3 1. EXECUTIVE SUMMARY Alameda Health & Safety Code, Chapter Report. A. and B. Alameda County Safe Drug Disposal Regulations, Section 15 Annual Reports C.i. Alameda County Safe Drug Disposal Ordinance ( Ordinance ) Section and Alameda County Safe Drug Disposal Regulations ( Regulations ) Section 15 require MED-Project ( MED-Project ) to prepare and submit an annual written report describing activities between January 1, 2017 and December 31, MED-Project submits this 2017 Alameda County, California MED-Project Annual Report ( Report ) on behalf of its participating producers and in compliance with the Ordinance and Regulations. The Alameda MED-Project Product Stewardship Plan ( Plan ) was approved by the Alameda County, Department of Environmental Health ( Alameda County DEH ) on February 25, In keeping with the Plan, MED-Project is implementing the Product Stewardship Program ( Program ) in phases. During 2017, priority was given to establishing additional kiosk drop-off sites throughout Alameda County ( County ), conducting take-back events in alignment with approved Plan goals, and performing outreach to eligible pharmacies and Law Enforcement Agencies ( LEAs ) throughout the County. As a result of these efforts, MED-Project s Program collected and disposed of Unwanted Medicine from 32 kiosk drop-off sites and 12 take-back events in Kiosks were activated in pharmacies and LEAs that meet all local, state, and federal requirements and that executed agreements with MED-Project prior to kiosk installation and activation. In addition, outreach and educational materials were distributed to all interested and participating sites, including signage and/or brochures. As outlined in the approved Plan, the ultimate goal of the Program is to provide County residents with convenient, safe, and accessible methods to dispose of Unwanted Medicine. MED-Project anticipates a steady increase in kiosk drop-off sites throughout 2018 as a result of Program activities in MED-Project continues to communicate with kiosk drop-off site hosts to distribute outreach materials, review procedures as needed, and improve overall Program design. MED-Project will submit revision requests and a revised plan in 2018 to address both improvement in collection efforts to reflect the data in this and previous annual reports, and also address challenges in program implementation, such as implementing changes to address the California Board of Pharmacy Prescription Drug Take-Back Services Regulations, Cal. Code Regs. tit. 16, 1776 et seq. (2017) (the Board of Pharmacy Regulations ). Page 3

Alameda County Safe Drug Disposal Regulations, Section 15 Annual Reports C.ii Collection Goals Long-term collection goals, as outlined in the approved Plan, include the expansion of kiosk placement to additional eligible LEAs and pharmacies during the third year of Program operations, with a goal of establishing up to 100 kiosk drop-off sites, assuming viable locations are available. Program goals set an ongoing target of 12 take-back events per year. MED-Project held 12 take-back events in In 2017, law enforcement oversight of events was performed by ACSO for all 12 of the events. For more information about this effort, please see Section 3. Agreements to host kiosk collection services in 2017 were executed with four (4) LEAs and ten (10) pharmacies and healthcare organizations, consisting of a total of 22 kiosk drop-off sites. Kiosks were subsequently installed in 22 sites and became available to the public, providing a total of 32 active kiosk-drop off sites in See Table 2 for more details. Page 4

Although MED-Project has not established up to 100 kiosk drop-off sites, it anticipates adding new kiosk drop-off sites in 2018 as potential kiosk drop-site hosts become more familiar with the Program. 6 2.2. Education and Public Outreach Goals MED-Project conducted 12 take-back events, performed installations at 22 kiosk drop-off sites, performed print ads, and distributed materials, including brochures and posters, to all 32 active kiosk drop-off sites in The toll-free number for the call center continues to be available in both English and Spanish. The MED-Project call center returns voic s within one business day as provided in the approved Plan. In an effort to increase website visits, MED-Project continued outreach in 2017 through social media websites such as Twitter and Facebook. Brochures and materials were distributed to nearby pharmacies, LEAs, clinics, veterinary facilities, hospitals, and community groups, and other media outlets were contacted to increase outreach prior to events. The website provides contact information County residents, kiosk drop-off site hosts, and producers can use to obtain Program information. Information regarding kiosk drop-off sites is available in both map and list format at Take-back events are listed at the following site: The Plan s long-term education and public outreach goals provide that MED-Project may revise and/or add communication materials based on changes to the Plan, and that MED- Project will review media and public outreach and collect feedback to improve operations. The methodology employed by MED-Project utilizes qualitative and quantitative techniques to evaluate outreach and promotional activities, including unstructured interviews, data collection at take-back events, and website statistics. MED- Project is also incorporating information collected, best practices, and lessons learned from other jurisdictions to improve its Program. Page 6

7 MED-Project performed unstructured interviews of residents, potential and existing kiosk drop-off site hosts, stakeholders, health systems, and local agencies in various settings. The purpose of these interviews was to elicit feedback regarding program awareness and activities. Information was gathered during take-back events, site visits, and regularly scheduled meetings. Data collection was performed during and after take-back events to capture attendance, collection, transport, and Unwanted Medicine disposal statistics. Website statistics were compared to the previous calendar year. Through its data collection, MED-Project learned that events held in combination with the County s HHW events showed increased consumer participation as compared to events hosted at fairs or festivals. While MED-Project advertised its events extensively on social media and the MED-Project website, consumer participation at events was highest when held in combination with HHW events. Events at fairs and festivals collected fewer than 69 pounds of Unwanted Medicine per event on average, while events that were promoted jointly with HHW collected more than 360 pounds of Unwanted Medicine per event on average. As a result of these lessons learned, MED-Project may be able to improve collections at 2018 events by focusing mainly on conducting events in conjunction with planned HHW events. MED-Project is working in collaboration with Alameda County DEH to explore conducting Unwanted Medicine take-back events at all Alameda County HHW events in MED-Project provides media release templates to host sites, which enable sites to promote events in combination with MED-Project s efforts. Regarding kiosk drop-off sites, MED-Project will continue to publicize these locations in 2018, and will provide each site with posters, brochures, and media release templates. Upon activation, all existing and new kiosk drop-off sites are promoted on social media and the MED-Project website. Fliers for residents are also produced to increase public awareness. Efforts to recruit additional health systems and chain pharmacy kiosk drop-off sites in 2018 may also increase awareness and allow MED-Project to implement numerous additional kiosk drop-off sites throughout Alameda County. MED-Project continues to increase awareness of its Program in Alameda County and to recruit additional kiosk drop-off sites by performing site visits and distributing outreach materials to potential kiosk drop-off sites. MED-Project is also distributing materials to health associations, systems, and organizations, as described in Appendix E of the approved Plan. Over 3,150 pounds of Unwanted Medicine were collected at the 2017 events and subsequently incinerated. MED-Project collaborated with ACSO and held events at 12 different venues. Eight (8) of the events were in collaboration with County HHW drivethrough events, and four (4) of the events were conducted at festivals or fairs. For more information regarding take-back events conducted by MED-Project in 2017, please see Tables 1 and 4. The location of each site and the status of the agreements are outlined in Tables 5 and 6 below. Agreements with LEAs that are pending review include sites that have not reviewed the documents to date or may not be able to host kiosks. Agreements that are under review include sites that have acknowledged receipt and are in the process of reviewing the document. 15 GOALS Alameda County Safe Drug Disposal Regulations, Section 15 Annual Reports C.iii Take-Back Events MED-Project is working closely with Alameda County DEH to conduct take-back events in collaboration with HHW drive-through events throughout the County in In addition, MED-Project is exploring several options for hosting take-back events in collaboration with LEAs upon request. Take-back event services agreements will continue to be issued to LEAs that are interested in overseeing take-back events Kiosk Drop-Off Sites MED-Project continues to work closely with LEAs, pharmacies, and hospitals/clinics with on-site pharmacies that are interested in becoming kiosk drop-off sites. Agreements have been distributed to LEAs and 32 pharmacies or hospitals/clinics with on-site pharmacies for review. See Tables 5 and 6 for more information. MED-Project continues to schedule kiosk installation and activation at eligible sites with executed agreements, with the goal of establishing up to 100 kiosks in Alameda County. Pharmacies that are targeted for continued outreach as potential kiosk drop-off sites are listed in Appendix A. An additional kiosk installation was performed in early 2018 for a kiosk drop-off site host that executed an agreement with MED-Project. Please see Table 7 for more information. Events and kiosk information will also be posted to the MED-Project website as they are confirmed and available to the public. MED-Project will continue to monitor the website and call center to assure quality and customer satisfaction, and to assess the impact of outreach and education efforts. As part of the 2018 communication and outreach goals, MED-Project will consider adding Chinese, Vietnamese, and Tagalog translations to the website and print educational materials. Page 16

17 4. COLLECTION PRACTICES Alameda Health & Safety Code, Chapter Report. A.5. Alameda County Safe Drug Disposal Regulations, Section 15 Annual Reports C.iv. MED-Project provided County residents a combination of take-back events and kiosk drop-off sites for the collection of Unwanted Medicine in Policies and procedures for collecting, transporting, and disposing of Unwanted Medicine, as described in the approved Plan, were followed during the reporting period, with exceptions as described below. Take-back events were conducted from April through November with events hosted at 12 separate venues. Over 3,150 pounds in total were collected during 2017, with an average of approximately 263 pounds collected per event. Oversight was performed by ACSO at all events in In August 2017, MED-Project submitted a request for exception to utilize common carrier for the transportation and disposal of Unwanted Medicine due to an unexpected disposal facility closure, which was granted by Alameda County DEH on September 1, Take-back events in September 2017 utilized common carrier transport and were disposed of at Heritage Thermal Services in Ohio. Kiosk services agreements were executed with a total of four (4) LEAs and ten (10) pharmacies and healthcare organizations, representing a total of 22 kiosk drop-off sites. Kiosk drop-off sites were implemented at LEAs and pharmacies in 2017 with a total of 22 kiosk installations and activations at four (4) LEAs and 18 pharmacies and/or hospitals/clinics with an onsite pharmacy. Kiosks were available to the public within one week of installation. Based upon the potential for loose Unwanted Medicine to accumulate in the drop-slot, MED-Project worked with the kiosk vendor to design a retrofit for the drop-slot chute. MED-Project completed the retrofit for all kiosks placed under the Plan in Alameda County on March 1 st, All new kiosk installations will be equipped with this retrofit. Additionally, in June 2017, MED-Project learned that certain Program inner liners were not marked appropriately. MED-Project subsequently corrected this issue with the appropriate vendor. MED-Project continues to capture data from take-back events and kiosk drop-off sites to evaluate performance on a routine basis. Page 17

MED-Project uses a combination of printed materials, online media outlets, public service announcements, and other methods, such as the call center, to increase awareness of the Program, and to educate County residents about safe and available methods for disposal of Unwanted Medicine. In 2017, brochures (Figure 6) and posters were distributed to all active kiosk drop-off sites (Table 2), to ensure materials and information are available for residents. A primary objective of outreach efforts in 2017 was to raise awareness of the Program. Materials will continue to be sent to active kiosk drop-off sites, community organizations, agencies, associations, and health systems to increase awareness and recruit additional sites for potential kiosk placement. In early 2017, MED-Project began a vendor selection process to produce a 30-second animation that will be utilized as a Public Service Announcement for residents regarding safe and available methods of disposal of Unwanted Medicine. The vendor selection process was completed in July of 2017 and MED-Project worked with the vendor over the next seven months to finalize the animation video. Upon completion, MED-Project anticipates placement of the animation video on social media websites, followed by placement on other media platforms, such as television. See Appendix C for more information regarding the timing for placement of the animation video as well as production and placement of a 30-second audio script on radio stations in Page 18
