1 Making Every Contact Count In Bradford Department of Health & Wellbeing Public Health What is MECC? Making changes such as stopping smoking, improving diet, increasing physical activity, losing weight and reducing alcohol consumption can help people to reduce their risk of poor health significantly. Making every contact count (MECC) is an approach to behaviour change that utilises the millions of day to day interactions that organisations and people have with other people to encourage changes in behaviour that have a positive effect on the health and wellbeing of individuals, communities and populations. What is MECC? MECC uses the millions of dayto-day interactions that organisations and people have with other people to support them in making positive changes to their physical and mental health and wellbeing. Many long-term diseases affecting our population are closely linked to known behavioural risk factors, with 40% of the UK s disability adjusted life years lost being attributable to tobacco, hypertension, alcohol, being overweight or being physically inactive1. MECC is not about: What MECC is not Adding another job to already busy working days Staff becoming specialists or experts in certain lifestyle areas Staff becoming counsellors, providing ongoing support to particular individuals or sorting people s health out Staff telling somebody what to do and how to live their life. 2 Healthy Eating & Physical Activity To prevent obesity, manage risk factors and address the complications of obesity Excess Weight Obesity is a term used to describe somebody who is very overweight, with a lot of body fat Increasing recognition both in the UK and worldwide that there is an obesity epidemic In Bradford in 2015/ % of children in Reception were overweight or obese and 36.4% of Year 6 children were overweight or obese In Bradford in % of adults were overweight or obese NCMP data 2015/16 Excess Weight Evidence What Do We Look Like On The Inside? Prevention is better than cure Lose a little - gain a lot! Obesity reduces life expectancy by an average of 3 10 years and can lead to a number of serious and potentially life threatening conditions such as: Cancers e.g. bowel cancer and breast cancer Type 2 Diabetes Coronary Heart Disease Stroke Depression Losing just 5% of body weight can lead to health benefits for most people NHS Choices NICE Obesity: How Does it Cause Early Death? 3 Physical Activity Physical Activity Adults (19-64) should aim to: Be active every day Weekly 150 minutes of moderate intensity activity OR 75 minutes of vigorous intensity activity Do something to improve muscle strength on at least two days a week. Minimise the amount of time spent being sedentary (sitting) Department of Health 2016 Mecc in action film How to and how not to have a positive conversation about physical activity Alcohol Misuse To prevent harmful drinking and minimise risk from alcohol use for self and others Public Health, Leeds City Council Alcohol Information More than 10 million people in England drink above low risk levels Alcohol is 10% of the UK burden of disease and death, making alcohol one of the three biggest lifestyle risk factors for disease and death after smoking and obesity Alcohol: Understanding the Units Men and women should not regularly drink more than 2-3 units per day and no more than 14 units a week. This should include 2 alcohol free days The safest approach for pregnant women, or women planning to become pregnant, is not to drink alcohol at all, to keep risks to the baby to a minimum Regularly drinking up to a pint of premium lager or large glass of wine can increase the risk of mouth, upper throat, oesophageal, breast and bowel cancers There are 7 calories per gram of alcohol (fat = 9 calories per gram), which can be linked to weight gain / Source: Department of Health 3
4 Binge Drinking The definition of binge drinking used by the NHS is drinking lots of alcohol in a short space of time or drinking to get drunk For men, that's drinking more than 8 units during one session (about 3 pints of strong beer) For women, it's more than 6 units (2 large glasses of wine) Drinking Behaviour Adults living in households in the highest income quintile are twice as likely to drink heavily than adults in the lowest income quintile 22% compared to 10% Older people tend to drink more frequently than younger people Younger people tend to drink more heavily (exceeding 8 units for men and 6 units for women) on a single occasion than older people Every year in the UK, more than 10,000 fines for being drunk and disorderly are issued to young people aged Compared to non-drinkers, if you regularly drink above recommended units you risk higher rate of all of these issues: Smoking To identify smokers & prevent harm from smoking Department of Health PHOF Jan 2015 National Tobacco Use 15.5% of adult population in England smoke Majority of smokers start in their teens More men than women smoke, but the gap is narrowing Those from a lower social economic group are more likely to smoke Smokeless tobacco is mainly used by South Asian community groups Tobacco Use About half of all regular cigarette smokers will eventually be killed by their addiction Every year, over 100,000 smokers in the UK die from smoking related causes Tobacco use causes many types of cancers eg lung, larynx, mouth, throat, bladder, kidney, liver, stomach, pancreas, colon, rectum, cervix and acute myeloid leukemia Surveys show about two-thirds of current smokers would like to quit, but only 30-40% make a quit attempt in any given year Current smokers are up to 4 times more likely to quit successfully with support Jan
5 Benefits Of Stopping Smoking Second Hand Smoke (SHS) Affects Us All There is no safe level of exposure to second hand smoke Domestic exposure to SHS in the UK causes around 2,700 deaths in people aged and a further 8,000 deaths a year amongst those aged 65 and over Illness caused by SHS generates over 300,000 UK GP visits and around 9,500 hospital admissions every year, costing the NHS about 23.3 million Non- smokers exposure to second hand smoke In children increase risk of: Respiratory Disease by 50% Cot death by 50% Middle ear disease by 60% Double risk of Meningococcal Meningitis Ash Factsheets SHS, Feb 2014 In adults increase risk of: Lung cancer by 20-30% Heart disease by 25-35% Mecc in action film How to and how not to have a positive conversation about reducing smoking Mental Health Promote awareness of good mental health Public Health, Leeds City Council Mental Health Mental wellbeing describes your mental health how you are feeling and how well you can cope with day to day life. Our mental wellbeing can change from moment to moment, day to day, month to month or year to year. Our mental health can be impacted at any time in our lives, whether this is from a relationship breakdown, a bereavement or financial worries. 1 in 4 of us will have problems with our mental health at some time in our lives. 1 in 6 of us report experiencing a common mental health problem (such as anxiety or depression) in any given week. Mental Health Mental health problems often precede physical health conditions, and are important factors in recovery from physical conditions. Risk factors include: Having a Long Term Condition (46% of people with severe depression have a long term condition such as COPD or CHD) Smoking (50% of people who smoke have a mental health problem) Obesity, Alcohol and Drug use Psychological distress, discomfort and hopelessness can predict the onset of heart disease, and is also a risk factor for stroke Mental ill health also increases the risk of cancers, back pain, Irritable Bowel Disease as well as increases mortality from CVD, respiratory disease and infectious diseases. 6 Sexual Health Increase awareness of the importance of good sexual health and reduce the incidence of sexually transmitted infections Sexual Health Use a condom every time you have sex Test for all sexually transmitted infections after every new partner Emergency contraception (the morning after pill) is now effective for up to 5 days after unprotected sex, but is more effective the sooner you take it Ageing Well Promote health and wellbeing in older people Spot Signs of Fuel Poverty Cold home Damp Inactive householder Respiratory problems Portable heaters Lots of layers of clothes Falls About one third of all adults over the age of 65 fall each year Falls and fractures in people aged 65 and over account for over 4 million hospital bed days each year in England Falls Injurious falls, including 70,000 hip fractures annually, are the leading cause of accident related mortality in older people. (Help The Aged, 2008) After a fall, an older person has a 50% probability of having their mobility seriously impaired and a 10% probability of dying within a year. (Help The Aged, 2008) Falls reduces confidence, increase isolation and reduce independence with around 1 in 10 older people who fall becoming afraid to leave their home (NICE, 2013) A tailored exercise programme can reduce falls by as much as 54% (Spink et al, 2011) Royal College of Physicians,
7 Life Expectancy Male Female Life expectancy in Bradford Life expectancy in England Breastfeeding How long is breastfeeding recommended for? The world health organisation (2001) recommends that babies be exclusively breastfed until around 6 months. Just 1% of babies in Bradford reach this target (Infant feeding Survey, 2012) Public Health Outcomes Framework, figures Continued breastfeeding up until one year and ideally up until 2 years alongside the introduction of appropriate foods is also recommended by the WHO Obesity and not Breastfeeding One month of breastfeeding is associated with a 4% decrease in risk of obesity in childhood and later life (AHRQ, 2007) However, longer breastfeeding means even lower risk of obesity and the effect is strongest in children of primary school age Exclusive breastfeeding to 3 months is protective of preschool obesity up to 24% (Yan et al, 2014) Breastfeeding mothers benefit - An extra 500 calories per day are used when exclusively breastfeeding (DoH 2010) Benefits of Breastfeeding Lowers risk of: CHILDHOOD Sudden Infant Death Syndrome Gastroenteritis Allergies Asthma Hypertension Childhood cancer Poor dental health Diabetes type 1 Ear infections Eczema More GP consultations Lower IQ Obesity Respiratory illness/wheeze Septicaemia Urinary infections ADULTHOOD Hypertension Low bone density Breast cancer High cholesterol Diabetes Type 2 Heart disease Ovarian cancer Obesity Rheumatoid arthritis Research/Research/ Breastfeeding in Bradford In 2014/ % of mothers breastfed at birth. The England average was 74.3% This figure dropped to 41.6% at 6 8 weeks (giving any breastmilk). The England average was 43.8% So, to reiterate, MECC is around Having opportunistic and short chats with people up to 5 mins long Utilising effective communication skills Enabling positive change in an individual Increase an individual's motivation to make a positive change in an area of their lives May include signposting to a support organisation PHOF 7

The person listening cannot say anything but you still need to learn as much about the topic as possible. We are asking everyone no matter their role, to remember the following acronym to support your MECC conversations: The 3 AAAs Ask Seek to understand and listen before you speak Assist - Put the power for action in their hands and not yours!
