20 COMMUNITY ACTION PLAN Current Activities 2011 Adult Obesity Interventions Medical Nutrition Therapy Services Available at Stokes Family Health Center/marketed in community and physician offices Provide Eat Smart/Move More/Weigh Less Classes Nutrition Education targeting community, seniors, and church groups Participation in Health Fairs and sponsor additional screenings such as BMIs, cholesterol and glucose as requested

23 COMMUNITY ACTION PLAN Current Activities 2011 Youth Tobacco Use Interventions Enforce ongoing nonsmoking policy in schools Provide smoking cessation classes for students who do not follow school policy (School Nurses, Stokes Family Health Center s Health Educator, and Partnership for Drug Free NC) Provide Tobacco Use Prevention Education in schools ongoing Implement peer education component in schools (Stokes County Tobacco Control Coalition) in progress
