1 2018 EHDI Request for Proposals: List of Notice of Intent Forms Received ELIMINATING HEALTH DISPARITIES INITIATIVE CENTER FOR HEALTH EQUITY The Center for Health Equity received 164 Notice of Intent forms for the Eliminating Health Disparities Initiative (EHDI) Request for Proposals from the following applicant organizations. Please note that the Notice of Intent form was strongly encouraged but not required. This means that applicants may submit an application even if they did not submit a notice of intent.
