The Family Wellness Suite 1826 ARTHUR AVENUE Suite 101 (Btw 175th Street & 176th Street) (718)508-0618 (718) 508-0629 The Purpose -The Family Health Suites at the Neighborhood Health Action Centers The health of mothers and babies are connected. Women s health before pregnancy is the most critical factor in having a healthy baby and preventing infant death. Thus, to achieve birth equity the elimination of racial and ethnic and economic disparities in maternal and infant outcomes we must reduce infant mortality and life threatening complications during delivery through a multifaceted initiative that touches all New Yorkers and prioritizes neighborhoods with the highest rates of infant death and severe maternal morbidity/ sickness. Our Plan DOHMH proposes a multi-level strategy to address driving factors of poor birth outcomes that affect both mother and baby, by aligning with the citywide focus on Safe Sleep & Housing, Women s Health, Toxic Stress and Trauma. We hope to achieve birth equity in Black and Brown communities through direct service and building neighborhood capacity. The Neighborhood Health Action Centers Through the DOHMH Center for Health Equity has a strong history of building trusting relationships through presence and partnerships as well as providing programming at the neighborhood level. In 2016-2017, seven Neighborhood Health Action Centers will launch in revitalized DOHMH buildings in the South Bronx, East and Central Harlem, and North and Central Brooklyn. In addition to DOHMH programming and staff, they will offer primary care services, and host community-based organizations (CBOs) and other NYC government agencies to support the integration of public health, primary care, urban planning, and community organizing
