Salford Community Lifestyles is part of Salford Community Leisure (SCL), which is responsible for the provision and management of sport, leisure and cultural services in the city. SCL manage a wide variety of facilities across the 8 neighbourhood areas within the city, including sport and leisure facilities, community centres, libraries (plus 2 books @ home libraries), museums and community use high schools. SCL s vision is Everyone Active and all SCL employees work towards achieving this vision. Welcome to the 1st quarterly report for 2013 for the Active Lifestyles Team. My name is Nicky Winkley and I am responsible for the management of the Active Lifestyles Team and the strategic development of health related activity within Salford Community Leisure. I am responsible for the following areas of work: Exercise on referral Get Active programme Healthy @ Heart classes Health walks and allotment scheme Gym inductions Being Well Salford Pulmonary Rehabilitation Pilot Healthy Hips and Hearts Postural Stability Park Runs The aim of this quarterly report is to highlight the work that has taken place during during April, May and June within the Active Lifestyles Team and demonstrate how Salford Community Leisure is performing. Introduction Quarter 1 (April - June 2013) has been a very successful quarter, some key highlights include: Kurling group established at Fit City Pendlebury Since starting, Worsley Wood Park runners have completed an equivalent distance of running around the world 6 week Get Active programme delivered in Swinton Attendance at The London Commissioning Show Delivered Exercise for Older People workshop at North West Health and Physical Activity Annual Conference Attendance at Integrated Care for Older People Collaborative Several Healthy Hips and Hearts workshops delivered and spring tea dance Several Leisure Team Members completed a GP referral course to support gym inductions at Fit City sites One of the Active Lifestyles Development Officers completed ARKE training Being Well Salford launch Attendance at a Natural England Conference 2 additional health walks delivered for Get Walking week, which attracted 33 attendances

Exercise on referral: The exercise referral scheme is for people with long term health conditions. Each individual is offered a tailored menu of activities, which can range from chair based exercises, health walks, Tai Chi, the Get Active programme, gym access at Fit City centres or Healthy @ Heart classes. Number of people referred: 209 Performance against target this quarter: Over performance of 84 referrals above target of 125 Get Active programme: The Get Active programme is for people who have, or who are at risk of developing Coronary Heart Disease. The programme is designed to increase people s confidence to exercise independently by enabling them to try a range of activities in a supportive group environment as well as providing information on staying active and healthy for life. This quarter the programme only ran in one area, however next quarter a further programme will be delivered in Ordsall. Number of programmes running across the city: 1 Number of attendances in quarter 1: 17 Healthy @ Heart classes: Healthy @ Heart classes are low level circuit classes designed specifically for people who have experienced cardiac problems such as a Heart Attack or who have been diagnosed with Angina or Cardio Vascular Disease. It is also suitable for those with Cardio Vascular Disease risk factors such as high blood pressure, high cholesterol or type 2 diabetes. Attendances to these classes have decreased slightly this quarter. This could possibly be attributed to the poor weather conditions in spring and also the fact that there were 3 bank holidays, which would have reduced the number of classes delivered this quarter by 12. There has however been a slight increase in the number of referrals received from cardiac rehabilitation and therefore figures may rise next quarter. Number of weekly classes delivered across the city: 13 Number of attendances recorded at classes: 2093 this quarter Health walks & allotment scheme: Health walks are available citywide in Salford; they are short doorstep walks no more than an hour in length which are led by volunteer Health Walk Leaders. They provide the opportunity for people to become more active and healthy in a social group within their local area. There were also two additional walks this quarter for Get Walking week, which saw 33 attendances. Number of health walks delivered weekly across the city: 8 Number of attendances recorded weekly at health walks: 2684 Number of active health walk leaders: 23 Attendances at allotment sessions this quarter: 240 I believe the Active Lifestyles Programme has helped me with my mild osteoarthritis. My knees and hip joints feel much more supple and less painful. I m also hoping to bring down my cholesterol by further increasing my exercise. The big plus is that you are in the right environment with support and no distractions.

Gym inductions People with long term health conditions require programmes of exercise to be carefully prescribed. SCL have Level 3 GP referral Gym Instructors in a number of the fitness suites who can deal with individuals who have mild to moderate health conditions. Also the Active Lifestyles Team are all level 4 qualified and specially trained to work with people who have complex / multiple health conditions. In many instances these inductions will result in the customer taking up a fitness membership. This quarter 9 additional Leisure Team Members completed a GP referral course to enable them to support this element of the service. Gym Inductions carried out by level 3 and 4 instructors this quarter: 69 Pulmonary rehabilitation pilot The Active Lifestyles Team have been commissioned in partnership with the SRFT Respiratory Team to deliver the exercise element of the Pulmonary Rehabilitation service for clients with lung conditions such as Chronic Obstructive Pulmonary Disease. The Pulmonary Rehabilitation Team is monitoring and reporting on this service. Number of classes (per week): 4 Average number of clients per session: 12 Healthy Hips and Hearts (HHH s): HHH s is a project for over 50 s in the city which aims to reduce social isolation and improve health and wellbeing. There are over 80 chair based exercise sessions established across the city in residential and sheltered housing run by staff in the sites who have received the HHH s training. There are also sole volunteers, who are members of the community wishing to enhance the lives of older people by delivering community sessions on SCL s behalf. The HHH s team provide mentor training then continue to support the mentors and sole volunteers in establishing sessions. Several workshops were delivered this quarter which offer existing mentors additional exercises, techniques and activities to incorporate into their Healthy Hips and Heart classes. The Better Balance workshop taught mentors additional standing exercises. The movement to music workshop offered gentle dance based exercises. Another workshop was delivered around the basic movements of Tai Chi, and a Shea Basu workshop which taught people about breathing techniques, being able to deal with anxiety and health problems such as COPD. Number of HHH s sessions delivered across the city: 29 community classes weekly. Number of events hosted: 1- spring tea dance (approximately 100 attendances) Postural Stability Postural Stability is a 12 week programme for those who are at risk of falling or who have suffered a fall previously. People are referred to this service from the Falls Clinic at Salford Royal Hospital. There are currently 2 sessions a week across the city. 23 people were referred to the service this quarter. Working in partnership with the Falls Clinic, the team is currently looking at a restructure to improve the quality and efficiency of the service. Number of people referred: 23 Number of attendances at sessions: 111

Park Run: Park Run is an international network of free, timed 5 kilometre runs. The first park run in Salford was set up in March 2012 at Worsley Woods. This popular park run starts at the Monton end of the Loop line and participants complete the 5 kilometre course at their own speed. On average there have been 104 people at each run. Since starting the Park Run, Worsley Wood Park runners have completed an equivalent distance of running around the world. Number of park runs this quarter: 13 Number of attendances this quarter: 2433 Being Well Salford: Being Well Salford is a new service commissioned by Salford City Council, managed by Big Life Centres which aims to work with people in their community to develop self help initiatives. The service aims to improve the health and wellbeing of people in Salford by helping them to make several long term behaviour changes. To be referred to the service a client must want to change two or more of the following things; low mood, weight, getting more active, smoking and/ or reducing alcohol intake. Being well aims to provide quality centres for the delivery of statuary services alongside community and voluntary sectors. Active Lifestyles will work alongside the service and hope to generate referrals as a result. Additional Activity: In additional to the core services described above, the Active Lifestyles team also supports other areas of work, which ultimately improve opportunities and increase participation in physical activity, particularly by older people, therefore encouraging a more healthy lifestyle. Examples of additional activity this quarter include setting up a Kurling group at Fit City Pendlebury and attendance at a number of relevant key conferences and workshops including: London Commissioning Show To understand more about the latest commissioning trends and changes within the Clinical Commissioning Group (CCG)/ NHS England and networking amongst other Health Professionals. Integrated Care for Older People Collaborative An ongoing project made up of seven subgroups, which aims to reduce hospital admissions in older adults, enable people to maintain independence, to die in their preferred place, and to feel socially connected with better access to services. Delivered workshop at North West Physical Activity Forum The presentation discussed how older people were included in the Olympics, the physical activity needs of an ageing population, a summary of the Go for Gold Challenge 2012 and future plans for the project and how provision could be improved. Natural England Conference Assisting in the restructure of cascade training for all trainers and walk leaders. ARKE Training The aim of the ARKE solution is to create a training environment where users of all fitness levels are able to (re)learn and improve their movement skills. Active Lifestyles hope to integrate this equipment into some of its delivery.

Case studies Name of Participant: MT Age: 78 years Name of Session: Healthy Hips and Hearts Name of Lead Officer: Louise Ashworth MT started attending the Healthy Hips and Hearts class at Boothstown Community Centre around twelve months ago. MT lives on her own in warden-controlled housing. Eighteen years ago she had a stroke which left her with a left sided weakness. Unfortunately MT was born with a deformity in her left leg and the stroke exacerbated the problem. As she got older the problem became worse and this affected her emotionally and her general health suffered too. She had attended exercise classes on and off since the stroke because she knew they were important, but as she got older she felt they were becoming too strenuous for her. MT then came across a Healthy Hips and Hearts leaflet and decided to come to a class. She was delighted with the level of activity, it was just right for her and she copes very well with it. MT has found that her co-ordination has improved, along with her balance, and consequently she is feeling more confident. She has made lots of new friends and really looks forward to Tuesday afternoons when the class takes place. Name of Participant: SM Age: 71 years Name of Session: Healthy Hips and Hearts Name of Lead Officer: Ibrat Naeem SM joined the Healthy Hips and Hearts group at Wardley Community Centre; she has been attending the exercise sessions for the last fifteen months. SM has been suffering from COPD for a numbers of years, however prior to this was always active. SM lives quite close to the community centre and now enjoys walking to the centre. Several years before she wouldn t have found this possible as she used to become out of breath even after taking 8 to 10 steps. SM stated; By attending these exercise sessions they have helped me tremendously both with breathing and generally keeping me mobile. There were so many things which I couldn t manage to do in the past. I now feel I can do household chores a lot easier. SM also enjoys meeting up with the other clients and has exclaimed how grateful she is to SCL for running these classes and how she hopes that they keep running in the future. Name of Participant: LC Age: 66 years Name of Session: Get Active Name of Lead Officer: Gareth Davies In June 2012, LC was referred to the Active Lifestyles Team by her GP to help her become more active and to help her lose some weight. LC was contacted to discuss the various options in helping her to achieve her goals. After a brief discussion, LC agreed the Get Active

Programme would be the best place to start. 12 month s on, LC has completed the Get Active Programme, and now acts as a mentor to other people attending the course for the first time. She also attends the Swinton Healthy @ Heart Class and Kurling session on a weekly basis and has even managed to encourage one of her friends to join her. LC recently expressed how much she enjoys attending the classes and commented about the positive difference it has made to her fitness, health and general wellbeing. She was also pleased to mention that she has lost half a stone in weight and has made lots of new friends.
