5 AIMS & OBJECTIVES To study effects of Vacha & local abhyang (with Bala taila) on speech disorders that are of neurologic origin 5
6 MATERIAL & METHODS STUDY DESIGN: Study was planned to be conducted on 25 in-door patients of KB Dept. of PD Patel Ayu. Hospital having speech disorders of neurologic origin. Duration of treatment: One month duration Examination of the patient: Especially for excessive drooling, difficulty in sucking, chewing &/or swallowing as well as difficulty in coordination of lips, jaw & tongue (both BT as well as AT) contd. 6
7 MATERIAL & METHODS DRUGS Vacha churna (Acorus calamus) - for internal administration Bala taila - for local abhyang on cheeks METHOD: Clinical exam of diagnosed cases of CP or ADHD with speech disorder Vacha chunra in the dose of 5 mg/kg body wt. on empty stomach was given for one month Abhyang of Bala taila on both the cheeks twice 7 daily contd.
8 MATERIAL & METHODS INCLUSION CRITERIA Diagnosed cases of CP/ADHD with speech disorder attending PD Patel Ayu. Hospital Patients up to completion of 16 years of age EXCLUSION CRITERIA Patients having speech disorders due to pathology other than neurologic conditions Malignancies of CNS Patients above 16 years of age 8

14 DISCUSSION & CONCLUSION CP is a disorder of motor dysfunction Muscles involved in sucking, chewing & swallowing are also involved in phonation and speech Weakness of muscles salivary drooling as well as in development of speech deficiency or disorder In the present study no abnormal findings in the MRI of brain of ADHD were reported. contd. 14
15 DISCUSSION & CONCLUSION However, in ADHD ~ 5-10% reduction in volume of specific brain structures such as prefrontal cortex and basal ganglia (both rich in dopamine receptors) is observed Also reduction of blood flow to caudate nucleus & striatum is found Significantly smaller splenium of corpus callosum is also one of the findings These may lead to delayed development in some patients along with speech disorders contd. 15
16 DISCUSSION & CONCLUSION Abhyang provides strength to weak muscles Vacha is said to vak-svar-prada Present study confirms these properties The study was for a short term duration and during that all the patients (except one, who was noncompliant and three drop out) got relief. 16
17 DISCUSSION & CONCLUSION Overall relied percentage in salivary drooling &/or other difficulties involved in sucking, swallowing etc was 100% (9 out of 9 patients) Overall relief percentage in speech was 84% (22 patients out of total 26) 17
18 DISCUSSION & CONCLUSION Apprehension about toxicity of Vacha Beta asarone, found in Vacha, is a procarcinogen that is neither hepatotoxic nor hepatocarcinogenic. 18
19 DISCUSSION & CONCLUSION To exhibit toxicity it needs metabolic I - hydroxylation in liver The carcinogenic potency of toxic metabolite (genotoxic epoxide) is very low due to rapid breakdown of epoxide residues with hydorlase which leaves these compounds inert. Additionally, the major metabolite of betaasarone is 2,4,5-trimethoxininnamic acid. This derivative is non-carcinogen. 19
20 DISCUSSION & CONCLUSION Also the studies for its toxicity involve animals The results found in animals may not hold true on humans. Ayurveda has been using Vacha since ages and no such adverse effects have been reported in ancient texts. Further studies involving a large no. of patients would prove this. 20
