4 The Status of the HIV Epidemic Approximately 1,000,000 PLHIV (HIV estimates) Country s HIV prevalence rate at 10% for the population aged (MPHIA, ) Women disproportionately affected- prevalence is at 12.8% among females compared to 8.2% among males (MPHIA, ) HIV prevalence among 25 to 29-year-olds is three times higher among females (14.1 percent) than males (4.8 percent) Southern Region is the epicenter of the HIV epidemic Prevalence hovers slightly over 15 % among adults aged (MPHIA, ) Prevalence in Blantyre City alone is as high as 18.2% As of December, 2016: there were 662,788 patients on ART and 89% of HIV positive mothers were on ART (Integrated HIV Program Report)

6 Investments in HIV and AIDs by Malawi Government Government contribution to the national HIV response has been rising, albeit incrementally, reflecting government s commitment to the fight against HIV and AIDS. Government contribution rose from 1.7% in 2010/11 to 8.1% in 2011/12 and 14.3% in 2013/14. There was a Government funding dip in the 2015/16 fiscal year; Government contribution was estimated at 9% (2017 GAM) Increase in Government spending on HIV is imperative To support expansion of treatment programme under Test and Treat Strategy Counterpart financing under the GF support

7 Country Response with respect to the MDGs HIV was prioritized in the MGDS II (under the key priority area: Public Health, Sanitation, Malaria and HIV and AIDS Management ), which was the country level operationalization of the MDGs Under the MDG dispensation, response evolved remarkably: Roll out and expansion of free ART program Roll out of the Option B+ Strategy (PMTCT) Decentralization of HIV services to communities (enhancing the public health approach to HIV programming)-making the scale up of ART manageable Over 50% reduction in new HIV infections in a decade ( ) An almost equally drastic reduction in AIDS related deaths.

8 Country Response to the SDGs: A Strategy Perspective The National HIV and AIDS Strategic Plan ( ) operationalizes the UNAIDS Fast-Track Agenda at country level UNAIDS global Fast-Track strategy, and thus NSP , envisages acceleration of the global response towards the goal of eliminating the AIDS epidemic by 2030 (enshrined in SDG 3) Country developing new MGDS which will domesticate SDGs and include HIV Malawi on course to achieving the targets, which form the centerpiece of the Fast-Track Strategy (among first countries to adopt targets) Increasing focus on vulnerable populations: AGYW, KPs (MSM and FSWs), and geographic locations (Blantyre is a Fast Track City) Some cautious optimism regarding the expanding role for domestic resources in the HIV response within the next few years

10 Concluding thoughts and Outlook Explore HIV program implementation efficiency as the next frontier for HIV financing World Bank supported HIV program implementation study already pointing to some potential levers for enhancing efficiency ODA will still constitute a sizeable and critical proportion of total HIV funding for the foreseeable future Further intensify domestic resource mobilization-explore more innovative financing mechanisms Having a lot of money is no guarantee for sustainability (Whiteside & Obertha, 2016). Need to emphasize other tenets of sustainability beyond financial: epidemiological, programmatic, political as well as human rights (ibid, 2016)
