1 International HIV/AIDS Alliance in Ukraine at the International AIDS Conference, Melbourne, July 2014 In 2014 and beyond the International HIV/AIDS Alliance in Ukraine continues to strengthen the responses to concentrated HIV epidemics. We support implementation of large-scale comprehensive HIV prevention and care programme, start-ups and revisions of existing interventions targeting people wh o use drugs and other key populations. We combine direct management of programme development and implementation with systematic and structure d technical support, as well as state-of-the-art monitoring and evaluation. We implement programmes and deliver support domestically in Ukraine, as w ell as internationally in a range of countries facing concentrated HIV epidemics in Eastern Europe, Asia and the Pacific, East and West Africa. Our Kyiv I nternational Technical Support Hub delivers high quality technical support through long-term programmes and short-term assignments delivered by ou tstanding specialists with extensive experience of developing, managing and supporting HIV prevention and care interventions among people who use d rugs and other marginalised communities. Our latest achievements and key priorities are reflected in a range of presentations and discussions at Melbourne conference and include the following: SCALE and FOCUS: Ensuring that interventions focus on key affected populations in order to maximise the impact of HIV prevention and care efforts: Further development and application of effective outreach methods, including peer-driven outreach models based on the exploration of natural social networks of key populations; Maximising the potential for strategic use of ART by making the essential testing and treatment service accessible to key populations in community settings; Engaging sexual partners of key affected populations such as sexual partners of people who inject drugs and clients of sex workers in order to prevent further HIV transmission and acquisition; Maximising the preventive effects of PMTCT interventions. QUALITY: Further development of the essential HIV prevention and care services such as:

2 Ensuring that needle and syringe programmes are fully tailored to the local drug scenes and address the whole spectrum of risks associated with the production, transportation, distribution and consumption of substances; Improving the quality of opioid substitution treatment and addressing challenges faced by the clients and service providers; Ensuring access of people who inject drugs and other key populations to HIV care and treatment. Acknowledging and strengthening the role of communities in the detection of HIV, timely progression to and retention in HIV treatment. SPECTRUM: Addressing related conditions affecting the outcomes of HIV prevention and care programmes: Integrating HCV prevention and care into HIV interventions; Introducing early TB detection and improving treatment efficiency among key populations. SUSTAINABILITY: Ensuring sustainability and increasing allocation of public funds to HIV Establishing strategic stakeholder dialogue and involving the governments in the development and funding of essential prevention and care interventions targeting key populations; Working with donors to facilitate transitions towards public funding of key interventions. EFFECTIVENESS: Strengthening the evidence base for interventions: Studying impact of HIV prevention programmes on HIV seroconversion and other essential epidemiological parameters. ETHICS and RIGHTS: Addressing complexities of working with key populations in order to increase the effectiveness of prevention and care, e.g. addressing ethical dilemmas of working with the most at risk adolescents; Addressing stigma and discrimination as key factors restricting access of key affected populations to effective HIV prevention and care services; Addressing emergencies in HIV programming (rescue operation among Crimean OST patients).
