KEARS, Director AGENDA April 28, 2009 March 31, 2009 AGENCY ADMIN. &FINANCE 1000 San Leandro Blvd., Suite 300 San Leandro, CA Tel: (510) Fax: (510) The Honorable Board of Supervisors County Administration Building 1221 Oak Street Oakland, CA Dear Board Members: SUBJECT: Approval of the Ryan White Care Act Part A Fiscal Agent Agreement Amendment between Contra Costa County and Public Health Department, Office of AIDS Administration RECOMMENDATIONS: Approve and authorize the President to sign in original signature five (5) copies of the Fiscal Agent Agreement Amendment between Contra Costa County and Alameda County Public Health Department, Office of AIDS for Ryan White Part A program, an increase of $21,796 from $1,225,000 to $1,246,796 for the period March 1,2008 through February 28, 2009, Procurement Contract #2848 and Purchase Order #PHSVC4667 to provide comprehensive HIV/AIDS Care and Treatment services. SUMMARYIDISCUSSIONIFINDINGS: The California Department of Public Health (CDPH) has designated the County of Alameda as Fiscal Agent, or "Grantee", for the purpose of administering the Ryan White Care Act Part A and B funds in Contra Costa County. The Ryan White Part A and B grants provide funding for case management, mental health, medical and other support services to persons with HIVIAIDS and their families. On July 22,2008, your Board approved the Ryan White Part A Fiscal Agent agreements between Contra Costa County and the Public Health Department, Office of AIDS Administration, in the amount of $1,188,009 for the period March 1,2008 through February 29, On December 9, 2008 your Board approved the Ryan White Part A Fiscal Agent Agreement Amendment from $1,188,009 to $1,225,000, an increase of $36,991. In order to maximize the Ryan White Part A funding, the Alameda/Contra Costa HIV Collaborative Community Panning Council has approved a reallocation of Quality Management savings of $21,796 on February 10, 2009 to Contra Costa County. This additional funding will be used to develop and implement on-going Quality Management processes, plans, and provide technical assistance to CBOs to ensure quality data processes. The Public Health Department is therefore requesting a second amendment to the contract for an increase of $21,796 from $1,225,000 to $1,246,796.

Page 1, Paragraphs 3 and 6, shad be modified respectively to read: "WHEREAS, the Secretary of the United States Department of Health and Human Services, acting through the Health Resources and Services Administration, has awarded $ to the Oakland Transitional Geographical Area in accordance with the provisions of the HIV Emergency Relief Grant Program of the Act, to be disbursed over the period of March 1, 2008 through February to provide essential services, and;" "WHEREAS, both the Health Resources and Services Administration and the Alameda/Contra Costa HIV CoUaborative Community Planning Council stipulate that percent of the funds awarded to the Oakland Transitional Geographical Area for direct services be allocated to Contra Costa County and, further, that Contra Costa County has been allocated $ 1,246, from the period of March 1,2008 through February " 2. Page 1, Item 2-A, shall be modified to read: "A. Remit payment to Contra Costa County within ten (10) working days of receipt of each invoice. The total amount payable to Contra Costa County shall not exceed $ 1,246, for the period of March 1, 2008 through February 28, 2009," 3. Exhibit A shall be replaced by the attached Exhibit A (Revised). 4. Exhibit B shall be replaced by the attached Exhibit B (Revised). Budget and Service Priorities I. Core Services Early Intervention Serivces To provide HIV early intervention services including HIV counseling and testing at high risk clinics to at least 400 individuals in Contra Costa. $ 30,860 Case Management To provide medical case management services, including: coordinating access to medically appropriate levels of health care, treatment adherence, partner counseling and referral, case conferencing, etc., to at least 400 people with HIV/ AIDS in Contra Costa $ 700,489 Mental Health To coordinate and provide mental health assessments, counseling and referral follow up services to at least 75 people with HIV in Contra Costa. $ 72,303 Substance Abuse To coordinate access to care and provide assessments, substance abuse counseling and treatment for at least 75 people with HIV in Contra Costa. $ 127,767 Primary Medical Care To provide ambulatory medical care to eligible undocumented and indigent individuals. $ 10,000 The maximum allowable for cores services is 11-$ 9_4_1_,4_1_9...,j1 EXHIBIT A, PAGE 2 Contra Costa Health Del:lartm.~nt

Support Services Food March 1, February 28, 2009 Revised December 17, 2008 To provide food bank, congregate meals, vouchers and home delivered $ 94,654 meals to at least 200 eligible Contra Costa residents. Housing Assistance To provide emergency housing assistance payments consistent with standards of care for at least 6 Contra Costa residents $ 6,000 Respite Services To provide community-based, drop-in support to relieve caretakers by offering patient education, risk reduction, medication adherence, access to other services $ 50,001 and support for caretakers for at least 50 people with HIV in Contra Costa. Legal Advocacy To provide allowable legal services to ensure ongoing benefits coverage for at least 80 people with HIV in Contra Costa. $ 62,001 Emergency Financial Assistance To provide emergency payments for utilities in accordance with established protocols to 7 approx. $300jperson when no other resources are available. $ 1,991 Transportation To provide transportation vouchers and van transportation assistance to medical and social service appointments to at least 55 eligible Contra Costa residents $ 33,933 The maximum allowable for support services is 248,580 I III. 7 BACKGROUND: The U.S. Department ofhealth and Human Services has designated the County ofalameda as "Grantee" for the purpose of administering the Ryan White HIV/AIDS Treatment Modernization Act of2006, Part A funds for coordination of essential services to County Costa County residence with HIV disease and their families. On June 3, 2008, the Board of Supervisors approved Agreement # (as amended by Amendment Agreement # ) with the County ofalameda, as the Grantee of Federal Ryan White HIV/AIDS Treatment Modernization Act of2006, Part A funds, for the period from March 1,2008 through February 28, 2009, for coordination of essential services to County Costa County residence with HIV disease and their families. Approval of this Agreement (Amendment) # will increase payment to the County, in amount not to exceed $21,796 for these services, make technical adjustments, and aliow services to continue through February 28,2009. Four sealed/certified copies of this Board Order should be returned to the Contracts and Grants Unit for submission to the County of Alameda.
