2 WHAT IS THE ALLERGY TOOLKIT? Atopic dermatitis is a diagnosis of exclusion. With this in mind, the allergy toolkit was created to help systematically rule out common causes of pruritus in dogs and cats, and thus complement the food and environmental testing for which Avacta Animal Health is known and trusted. The flowchart on page 5 shows the complete toolkit now being offered. Designed to be completely flexible to the needs of different veterinary professionals, it enables a comprehensive work-up to be carried out, testing for; bacterial and yeast infections, ectoparasites and fungal infections, food and environmental allergy. Alternatively, a specific section or individual test can be selected dependent on case requirements. Our detailed results pack has been devised to make test interpretation simple and clear. It includes client friendly versions of the results, with bespoke microscopy images from their own pet where possible, aiding their understanding and thus supporting enhanced treatment compliance. WHY SEROLOGICAL TESTING? Serological tests can aid diagnosis and treatment selection in cases of atopic dermatitis by providing rapid and easy identification of potential offending allergens. Although intradermal testing (IDT) was considered the gold standard for the diagnosis of allergic disease, recent data has suggested that the sensitivity and specificity of IDT vs serology is comparable 1. Furthermore, there are limitations to IDT such as; extensive patient preparation, sedation often being required and interpretation requiring both experience and expertise. ELISA technology has the following advantages: Quick and easy serum sample sent Excellent reproducibility to laboratory Not influenced by existing skin Requires minimal patient preparation pathologies or most medications No adverse reactions (see our allergy withdrawal guide at No need for referral withdrawal for more information) Standardised procedure WHY CONSIDER ALLERGY? It is suggested that 10-15% of dogs in the UK are affected by allergic disease, and it is being recognised with increasing frequency in cats. Consequently, there is a huge demand for aids in diagnosis. Allergy can manifest itself in a wide range of clinical signs. Those most commonly seen in companion animals are shown on the diagram below. 3 THE ALLERGY THRESHOLD The concept of an individual animal having an allergic threshold above which clinical signs are seen, has been long established. However, there is now a greater understanding that the allergens contributing to the build-up are often multifactorial. WORK-UP CHART FOR ATOPIC DERMATITIS If the only presenting sign is chronic ear disease, ectoparasite and/or fungal infection, tests may not be required. If completely controlled the clinical signs should resolve. The same would apply if food allergens were present in isolation and above the threshold. The combined allergenic stimulation is above the threshold so clinical signs are seen. Clinical signs could be managed by either controlling food or environmental allergens, but by only considering one aspect the pet is left close to the threshold and therefore vulnerable to compounding factors. In this case, if any intermittent compounding factors such as ectoparasites or seasonal pollens were added, the allergenic stimulation could easily go above the threshold and cause clinical signs. 4 The allergenic stimulation is above the threshold, so clinical signs are seen. It is not only in combination that this will occur as both environmental and food allergens individually are above the threshold. Even if one component is completely controlled there may not be a reduction in clinical signs. Both must be identified and controlled for resolution. DIAGNOSIS OF ATOPIC DERMATITIS If negative or partial response to food trial or seasonal SENSITEST ENVIRONMENTAL TEST FLARE-UPS Check for common causes of secondary infections. 4 Hypersensitivity also occurs to Malassezia and Staphylococcus, as it does against allergens. In these cases you often see a clinical response disproportionate to the number of organisms present on the animal s skin. BACTERIAL AND YEAST INFECTIONS Most cases of relapsing pyoderma or Malassezia dermatitis are secondary to an underlying condition/disease 2,3,4. STAPHYLOCOCCUS IgE/IgG Sample requirements: serum EXAMPLE RESULTS MALASSEZIA IgE/IgG Sample requirements: serum SKIN OR EAR CYTOLOGY (including ectoparasites, see also pages 8 & 9) Sample requirements: tape strips, impression smears, superficial and deep skin scrapes (5 slides)* Cytology is useful in the diagnosis of bacterial pyoderma/otitis or bacterial overgrowth, Malassezia dermatitis/otitis, eosinophilic granuloma complex and pemphigus foliaceus. Interpreted and reported by a certified dermatologist with images from actual slides submitted included in the report where possible (see page 15 for sample results). CULTURE AND SENSITIVITY Sample requirements: charcoal swab from the skin or ear* One swab is included in the individual tests and packages. Additional charges will apply for any extra swabs. Please see our website for further guidance on when culture and sensitivity is indicated. DOGS WITH ATOPIC DERMATITIS HAVE AN INCREASED TENDENCY TO SUFFER FROM SECONDARY SKIN INFECTIONS. SEE PAGE 14 FOR INFORMATION OF FLARE-UPS. STAPHYLOCOCCUS IgE 1 IgG 3 Positive IgE scores suggest a hypersensitivity response to staphylococcal antigens may be involved, with the organism acting as an allergen. In these cases you often see a clinical response disproportionate to the number of bacteria present 5. Positive IgG levels represent an antibody response proportional to the extent and duration of exposure to the bacteria. Most clinically healthy dogs will have come into contact with Staphylococcus, so a positive response is expected 5. Negative IgG scores in dogs displaying signs of severe staphylococcal infection may indicate an underlying immunodeficiency. THERAPY FOR IDIOPATHIC RECURRENT PYODERMA For further information about therapy for idiopathic recurrent pyoderma, due to hypersensitivity or otherwise, please contact Customer Services. TREATMENT OPTIONS MALASSEZIA IgE 3 IgG 4 Positive IgE scores suggest that a hypersensitivity response to Malassezia may be involved, with the organism acting as an allergen. In these cases you often see a clinical response disproportionate to the number of Malassezia present on the animal s skin 6,7,8. Positive IgG is expected to be present in clinically healthy dogs, as most will have come into contact with the yeast at some point. Negative IgE and IgG scores may indicate that the Malassezia has been eliminated. ALLERGEN-SPECIFIC IMMUNOTHERAPY (ASIT) AND TOPICAL TREATMENTS When positive IgE scores are present, ASIT can be considered to desensitise the dog to Malassezia and reduce the associated allergic response 9. Long-term topical treatment is also indicated. Regular monitoring of Malassezia levels using cytology is beneficial in cases with recurrent infections. *For further information and guidance on sampling techniques, go to 6 7

5 ECTOPARASITES AND FUNGAL INFECTIONS It is important to rule out these organisms during the diagnostic workup of a pruritic dog or cat. DERMATOPHYTE PCR Sample requirements: hair plucks, scale/crusts or toothbrush sample. Sensitivity 87%, specificity 100% Recommended for initial detection of dermatophytes. SARCOPTES SEROLOGY Sample requirements: serum Sensitivity 92.1%, specificity 94.6% Recommended for initially detecting exposure to Sarcoptes scabiei. EXAMPLE RESULTS SARCOPTES PCR Sample requirements: superficial skin scrapes from predilection sites over a skin area of 10 x 10 cm* Sensitivity 95%, specificity 100% Recommended follow up to positive serology results for monitoring response to treatment. EXAMPLE RESULTS DERMATOPHYTE PCR NEGATIVE/POSITIVE Positive scores indicate the presence of dermatophytes. DNA sequencing is advised to determine the species of fungi. Fungal culture is recommended for monitoring response to treatment. Negative scores indicate dermatophytes were not detected. SARCOPTES SEROLOGY IgG NEGATIVE/BORDERLINE/POSITIVE Positive scores indicate exposure to the mite Sarcoptes scabiei. Negative scores indicate that no IgG antibody levels were detected: it may take up to 4 weeks following infestation before positive levels arise. Borderline result may mean the IgG is increasing, or that the IgG is declining after treatment. ECTOPARASITE SKIN SCRAPES (part of skin cytology, see also pages 6 & 7) Sample requirements: Deep and superficial skin scrapes from 2-3 different locations relevant to predilection sites (5 slides)* Useful to detect Demodex, Cheyletiella, Trombiculae (harvest mites), lice, flea dirt and Sarcoptes (Sarcoptes mites are difficult to SARCOPTES PCR NEGATIVE/POSITIVE Positive scores indicate Sarcoptes mites are still present on the dog. DERMATOPHYTE CULTURE Sample requirements: Hair plucks or toothbrush sample. Recommended follow up to positive PCR results for monitoring response to treatment. Polymerase chain reaction (PCR) is a technique used to amplify DNA which enables positive identification of an organism from a small sample. The result is dependent on the presence/absence of the organism in the sample submitted, so ensuring good quality sampling is paramount to increase the performance of the test* FLEA ALLERGY DERMATITIS (FAD) If the pruritus persists after treatment of obvious infestations/infections, or no obvious cause was identified on initial investigation, the distribution of pruritus and/or lesions can be helpful for further investigations. Presence of recurrent dorsolumbar dermatitis, usually seasonal, is highly suggestive and a unique feature of flea allergy dermatitis (FAD) 10. Visualisation of fleas or flea faeces can be difficult in very pruritic, overgrooming animals. Pruritus and lesions usually resolve with diligent flea control. Intradermal or serology testing is available, but false negative results are possible and do not rule out FAD 10. *For further information on sampling techniques to increase the chances of locating specific parasites, go to 8 9

6 FOOD ALLERGY TESTS An aid in the diagnosis and management of food allergies, to assist in the selection of appropriate foods for a dietary trial to rule out/diagnose adverse food reactions. SENSITEST FOOD TEST Sample requirements: serum IgG is included in addition to IgE as not all adverse food reactions (AFRs) are IgE-mediated, and the determination of anti-food IgG can be helpful here. A food screen is also available which provides simple positive/negative results which can then be expanded to get full results if required. CYNO-DIAL /FELI-DIAL Sample requirement: serum and 15g food sample. Choose up to four diets for assessment. For a comprehensive list go to com/telechargement/ This is the next step after a SENSITEST Food Allergy Test. Cyno/Feli-Dial uses a western blot technique to detect IgEmediated reactions (hypersensitivities) against commercial diets. As with SENSITEST, it is the negative predictive value that is important. The comparative analysis provided can be used to help identify those complete diets which are more likely to be tolerated by the animal prior to a food trial. INTERPRETATION AND TREATMENT OPTIONS TO READ THE LATEST SENSITEST RESEARCH PAPERS REGARDING FOOD ALLERGY TESTING GO TO HEALTH.COM/RESOURCES Both the IgE and IgG SENSITEST serology tests have a high negative predictive value 11, so ideally foods that are negative to both should be selected for a dietary trial, as highlighted in this example. IgE reactivity is classically associated with hypersensitivity, however the IgG data can be useful in cases of suspected adverse food reaction which are not IgE-mediated. If the results yield no foods negative to both IgE and IgG, the foods with negative IgE scores and the lowest IgG reactivity should be selected for the exclusion diet. The exclusion diet should be conducted for at least 8 weeks (or until a good clinical response is seen), then challenged with components of the original diet to identify relevant allergens and confirm the diagnosis of food allergy

7 SENSITEST ENVIRONMENTAL ALLERGY TEST Sample requirements: serum An aid in the diagnosis and management of environmental allergies. Results should always be interpreted in relation to the clinical history. In addition to the comprehensive environmental indoor and outdoor panels, an environmental indicator screen is available. The screen provides simple positive/negative results for grasses, weeds, trees and indoor panels, which can then be expanded to get full results if required. INTERPRETATION AND TREATMENT OPTIONS If the correct diagnostic work-up has been followed and relevant clinical signs are present, positive IgE scores are compatible with an atopic animal. In this example the highlighted allergens are potentially significant. These should be interpreted alongside history. Avoidance - Our results packs give clear and practical detailed guidance of how to help with avoidance/reduce exposure to environmental allergens for which the animal has shown a hypersensitivity reaction. Allergen-specific immunotherapy (ASIT) - ASIT is a practical option where there are a number of clearly defined hypersensitivity responses. Studies show that between 50-80% 12 of cases improve on ASIT and that concurrent therapies can often be reduced and in certain cases eliminated completely. ASIT can be ordered via Avacta Animal Health

8 ADDITIONAL TESTS WHAT WILL I GET IN MY RESULTS PACK? EARS It is now well recognised that hypersensitivities such as atopic disease, food hypersensitivity and contact hypersensitivity can cause otitis externa. Allergic diseases are the most common cause of otitis, especially chronic otitis, responsible for 43% of cases 13,14. In dermatology referral practice, up to 75% of chronic otitis cases were associated with atopic disease 15. Chronic ear infection may be the only sign of atopic dermatitis in some dogs. This is the reason we include allergy testing, Malassezia IgE/IgG and staphylococcal IgE/IgG as part of our routine ear package in addition to the standard cytology and culture and sensitivity. FLARE-UPS Dogs with atopic dermatitis have an increased tendency to suffer from secondary skin infections, with Malassezia and staphylococcal infections recognised as two of the major causes of flare factors contributing to pruritus. The flare-up package is therefore extremely helpful when dogs diagnosed with atopic dermatitis, that have been doing well, regress. It helps to rule out some of the common causes that may be responsible. If an underlying condition, such as allergy, has not been diagnosed and repeated flare-ups are occurring, it may be worth considering further testing. Most cases of relapsing pyoderma or Malassezia dermatitis are secondary to, or associated with, an underlying condition or disease. DERMATOHISTOPATHOLOGY Sample requirements: 1-3 tissue biopsies. This is available as an individual test for complicated cases requiring further investigation beyond the standard work-up. As a general guideline, biopsies should be performed on; any dermatoses that are not responding to rational therapy, persistent ulcerations, vesicular dermatitis, neoplastic or suspected neoplastic lesions, severe life-threatening conditions, any condition readily diagnosed by histopathology and any dermatoses with an unusual presentation. For example, the serological environmental allergy testing includes client support on how to avoid individual allergens. In our cytology report we incorporate microscopy images from the actual sample submitted whereever possible. Our aim is to make the results as user friendly, clinically relevant and client inclusive as possible, in order to maximise understanding and therefore compliance of whatever treatment option is appropriate

When testing for food allergy make sure the animal has not been on a restricted or hypoallergenic diet for at least the last two months. Remember we can store blood, here at the lab, free of charge for 3 months. This enables you to send a sample when you first suspect allergy. You can then do the full work up and obtain test results safe in the knowledge no drugs/diets/other treatments may have influenced the results.
