7 7 RA Goals & Objectives Implement the HAQ Functional Status and RAPID 3 Assessment tools electronically into our EMR System. Increase utilization of the HAQ Functional Status and RAPID 3 Assessment Tools in Rheumatology Practice Implement program to increase early diagnosis of Rheumatoid Arthritis by our primary care providers Standardize patient education materials to support management of Rheumatoid Arthritis Identify disparities in care for our Rheumatoid Arthritis Population and develop action plans to address these disparities. Continue tracking DMARD use in Rheumatoid Arthritis
8 8 Where are we now? Currently using a paper based system to complete the HAQ and RAPID 3 Assessment tools. No standardization across all RA Providers No EMR Integration Completed the build of the HAQ and RAPID 3 Assessment tools electronically into our EMR System on July 1, Working with our providers to incorporate these templates into their daily workflow Currently have wide range of knowledge about Rheumatology from our primary care providers No standardized patient materials currently developed for our Rheumatology patients Currently, we are not looking for or addressing disparities in care for our RA patient population. We do not know if disparities exist.

10 10 What do we hope to achieve? Increase assessment of our Rheumatoid Arthritis patients via the HAQ and RAPID 3 Tools. Step up process We will look to increase utilization of the tools across all providers in 25 % increments each quarter, with the goal of achieving 100% utilization by 3 rd Quarter Look for disparities in care in the Rheumatoid Arthritis population and develop action plans to address any disparities identified 3 rd Quarter 2015 Develop an educational program for our primary care providers to aid in early diagnosis of Rheumatoid Arthritis Engage/educate our primary care population by January 2015 and track outcomes for Early Rheumatoid Arthritis diagnosing to infer effectiveness Develop patient educational materials to help with Rheumatoid Arthritis management Complete development and have ready for patient distribution during RA Awareness Month May 2015.

Patient engagement in completing the HAQ and RAPID 3 Assessment Tools Time to fill out the assessment tools Educating our providers and clinical staff on incorporating these tools into the daily workflow IT Development Development time for incorporating the assessment tools into our EMR System, creating training documents, and holding training sessions for staff Data Analytics Developing the data analytics to pull data from the assessment tools that can be analyzed to identify disparities in care
