It is used to treat respiratory disorders linked with viscid or excessive mucus. It is secretolytic. This means that it increases the production of serous mucus in the respiratory tract and makes the phlegm less sticky and thinner. This lead to

2 secretomotoric effect. This helps the cilia transport the phlegm out of the lungs. Cilia represents the tiny hairs that line the respiratory tract. For this reason, bromhexine is added to some anti tussive (cough) syrups. Uses To treat cough with phlegm. To treat acute and chronic diseases bronchus and lung violation othozdenia patients. The medicine should not be used for any other uses other than those mentioned in the product information section. However, Bisolvon (bromhexine) is at times recommended to treat respiratory disease in animals. Dosage Always take the medicine as directed by your physician or as mentioned in the packet. Usually the recommended doses are stated below: Children 2-6 years: ¼ tablet 2 times daily. Children 6-12 years: ½ tablet 3 times daily. Adult and children over 12 years: 1 tablet 3 times daily. Safety Tips Not recommended for children below 2 years of age without medical advice. If you buy bromhexine over-the-counter then for adults and older children who are normally in healthy condition, the medicine should not be consumed for more than one week. Do not use bromhexine if you are allergic to any ingredients of bromhexine or other medicines. Tell your doctor if you have liver problems, kidney problems, or stomach ulcers before taking the medicine. Bromhexine works by encouraging the body's natural process for clearing mucus from the respiratory tract. It does this by increasing the production of serous mucus which is a thin type of mucus which dilutes the phlegm making it less hard and sticky. This also helps to 'lubricate' that hard phlegm which is already there aiding the cilia (tiny hairs which line the respiratory tract) in sliding the phlegm out from the lungs. As well as being used on its own it is also included in a range of cough syrups. Function Bromhexine supports the body's own natural mechanisms for clearing mucus from the respiratory tract. It is secretolytic: that is, it increases the production of serous mucus in the respiratory tract and makes the phlegm thinner and less sticky. This contributes to a

4 secretomotoric effect: it helps the cilia - tiny hairs that line the respiratory tract - to transport the phlegm out of the lungs. For this reason it is often added to some antitussive (cough) syrups. Bromhexine is a synthetic derivative of the herbal active ingredient vasicine. It has been shown to increase the proportion of serous bronchial secretion, making it more easily expectorated. Bromhexine also enhances mucus transport by reducing mucus viscosity and by activating the ciliated epithelium. In clinical studies, Bromhexine showed secretolytic and secretomotoric effects in the bronchial tract area which facilitates expectoration and eases cough. It is indicated as secretolytic therapy in bronchopulmonary diseases associated with abnormal mucus secretion and impaired mucus transport. Bromhexine is contained in various formulations, high and low strength syrups 8mg/5ml, 4 mg/5ml, tablets and soluble tablets (both with 8 mg bromhexine) and solution for oral use 10 mg/ 5 ml), adapted to the need of the patients. The physiosology varies with the age & weight, but there are products for all age groups from infant on. Bromhexine is a well established and well tolerated product in its indication. Sometimes it is replaced by its metabolite ambroxol, as in Mucosolvan or Mucoangin. Pharmacodynamics Bromhexine is an oral mucolytic agent with a low level of associated toxicity. Bromhexine acts on the mucus at the formative stages in the glands, within the mucussecreting cells. Bromhexine disrupts the structure of acid mucopolysaccharide fibres in mucoid sputum and produces a less viscous mucus, which is easier to expectorate.

5 Pharmacokinetics Bromhexine hydrochloride is rapidly absorbed from the gastrointestinal tract and undergoes extensive first-pass metabolism in the liver. Its oral bioavailability is stated to be only about 20%. It is widely distributed to body tissues and is highly bound to plasma proteins. About 85 to 90% of a dose is excreted in the urine mainly as metabolites. It has a terminal elimination half-life of up to about 12 hours. Bromhexine crosses the blood brain barrier and small amounts cross the placenta. Precautions Since mucolytics may disrupt the gastric mucosal barrier, bromhexine should be used with caution in patients with a history of gastric ulceration. Clearance of bromhexine or its metabolites may be reduced in patients with severe hepatic or renal impairment. Special Precautions History of peptic ulceration; asthama patients.
