2 FIELD GUIDE Always take enough time with every applicant to assure that they fully understand all application questions and terminology. Applications for coverage will be rejected for the following: Anyone other than the applicant supplies the answers to the questions and signs the application. The applicant did not know they applied for insurance. The applicant is unwilling to complete a telephone interview. (Telephone interviews will be completed with all applicants.) Please advise the applicant to expect a call. If the application was submitted with a check from a third-party payor that has no family (spouse/partner, child, etc.) or business relationship (business owner, employee or retiree of the business). No premium or electronic funds transfer information is submitted with the application. CHOOSING AN EFFECTIVE DATE The effective date must be on or after the date of the application and, normally, will be the date the policy is issued.
3 UNDERWRITING Applications are underwritten up until the time the policy is issued. If a declinable health condition emerges between the time the application was taken and the time of policy issuance, the application will be declined. A yes answer to any part of Question 3 will automatically disqualify the applicant and the application should not be submitted. Questions on the telephone interview are identical to the application questions; however, questions concerning current medications and physicians being seen are also included in the interview. While the application form does not include a medications question, it is always advisable on any application to ask the applicant for medications currently being taken. Listing the medications may prompt the applicant to remember a condition, included in the application, which he/she may have overlooked. Certain medications may indicate a condition that is included in the application. The following Drug Information List contains commonly prescribed medications that are prescribed for conditions listed on the application; however, new drugs are regularly introduced and may not be included but may be unacceptable. Any questions concerning questionable medications should be directed to the Underwriting Department. A discussion of physicians being seen is helpful in determining eligibility. An applicant who is seeing certain specialists may suggest a condition included in the application; for example, an Oncologist may indicate treatment for cancer or a Nephrologist may indicate treatment for an unacceptable kidney problem. Other specialties that may suggest an unacceptable condition are Cardiologist, Hepatologist, and Psychiatrist. For questions, the Underwriting Department can be reached at , option 3.
