2 Introduction Dr. Judie Goodman, Oncology Program Medical Director The Oncology program at St. Joseph Mercy Oakland brings high quality cancer care to our patients in their own neighborhoods. Our multidisciplinary tumor boards ensure prospective case evaluation and a team approach. Access to national clinical trials provides cutting-edge treatments. The program encompasses the full spectrum of treatment: prevention and screening; treatment to survivorship. We are devoted to bringing our patients the highest quality care possible. Outcomes Prevention & Screening Programs, Clinical Trials and Survivorship Prevention & Screening The Pink Ribbon Trailblazers is a grassroots organization that raises funds to support the St. Joseph Mercy Oakland free mammogram program. This service is available to residents of Oakland County, 40 years of age or older, and who are uninsured or underinsured. BREAST: Eight free mammogram clinics will be held this year with six free mammogram clinics held to date. Services include additional imaging and ultrasounds. Persons needing a biopsy are referred to the Breast and Cervical Cancer Screening program (Title 15) and, if found to have a positive diagnosis, are assisted with obtaining a payer source. Six clinics held to date: 126 persons served. LUNG: Low dose CT Screenings for Lung cancer at a cost of $99 are offered at SJMO. January 1 through July 1, 2014: 35 persons served. Free Smoking Cessation classes have been initiated and offered this year in March, June and October. 16 persons served. ORAL, HEAD, AND NECK: March 2014 Free Head and Neck Cancer Screening: 39 persons served. COLORECTAL: Free Colorectal Cancer Screening - 1,053 colorectal screening kits returned for testing, 81 positives identified. Oncology Research Program St. Joseph Mercy Oakland (SJMO) is actively involved in cancer research, which is a major component of our comprehensive cancer program. One of the final steps in the long process of cancer research, clinical trials are research studies in which individuals join with physicians to find new, more effective ways to prevent, diagnose or treat cancer. Our affiliation with the Michigan Cancer Research Consortium s (MCRC) Community Clinical Oncology Program (CCOP) and Mercy Cancer Network continues, as does the process of transitioning to the NCI Community Oncology Research Program (N-CORP) which will reorganize the format and focus of our research efforts. We met the American College of Surgeons commendations benchmark for an academic teaching hospital of 6 of the analytical cases enrolled in clinical trials. In 2013, 64 patients were enrolled in oncology trials through various routes (Cooperative group studies, MSU reflexology study, Surveillance and Epidemiology studies, etc.), which totaled 7 of our analytical cases. Distress Screening Susan Yun, RN, MA, LPC, as our oncology program psychosocial coordinator facilitates our Orientation Class for new patients with cancer. This class provides an overview of cancer and treatment with a goal of helping patients and their family members identify personal and family concerns, find resources within the hospital and community and acknowledge fears and emotions. Classes are held monthly. To register for a class please call Supportive Care Clinic In the Fall of 2013 we began our out patient Supportive Care Clinic at the Cancer Center. The focus of the clinic is to prevent and relieve the symptoms of a disease, as well as provide psychosocial support in order to enhance the quality of life for patients and their families dealing with cancer. Led by a nurse practitioner, the clinic visit also includes chaplain and social work assessment and support. Additional resources such as dietitian, financial and nurse navigators and home care are also available to patients and their families as needed. Studies have shown that when palliative/ supportive care is provided at the same time as traditional cancer treatment, that the patient actually lives longer and has improved quality of life. We are pleased to offer these services to our patients and have served over 85 patients in our first year. For more information, call

3 MEET OUR CANCER SUPPORT SERVICES TEAM NURSE NAVIGATORS Andrea Briefs-Ferris, RN, MHSA, CBPN-IC, Breast Cancer Nurse Navigator, Andrea serves as an advocate for breast cancer patients and assists both patients and families with various resource needs. She assists in coordinating care, provides emotional support, provides education, coordinates counseling when indicated and partners with physicians and their office staff members. She also leads two support groups: Breast Cancer Support Group and Look Good Feel Better, offered in cooperation with the American Cancer Society. Patti Moore, RN, BSN, Lung Cancer Nurse Navigator, Patti serves as an advocate for lung cancer patients and assists both patients and families with various resource needs. She coordinates care, provides emotional support, provides education, coordinates counseling when indicated and partners with physicians and their office staff members. GENETIC COUNSELING Kelly Kennelly, MS, CGC, Our genetic couselor evaluates and communicates the risk of an inherited disease to individuals who are identified as abeing at higher risk of developing cancer during their lifetime. Testing to identify these individuals means that they can take steps to reduce their risk of cancer by pursuing vital screening and risk reduction strategies. Patients can access our service through their physician or self-referrals or by requesting the MammoPlus risk assessment at the time of their digital mammogram at SJMO and then opting to participate in the RiskPlus program. SURVIVORSHIP ISSUES Suzanne Jermstad MSN, FNP-BC, AOCNP, Advanced Oncology Certified Nurse Practitioner, Suzanne is a board-certified, advanced oncology nurse practitioner who has cared for SJMO cancer patients for more than 15 years. She provides chemotherapy teaching, helps to manage treatment side effects, and assists with pain and palliative care. She also serves as the clinical practitioner for the RiskPlus program, making recommendations to primary care physicians for patients found to be at greater risk for developing breast cancer. In her role as the survivorship nurse practitioner, Suzanne formulates a post-treatment care plan and offers continued management of side effects and issues related to cancer diagnosis and treatment; including referrals for therapy, psychosocial support, and rehabilitation. FINANCIAL NEEDS Debra Averill, CPC, Oncology Financial Navigator, Debra assesses financial needs of new and established cancer patients with the intent of removing financial barriers to care and improving coverage for care. She provides patients and the cancer care team with financial education and advises on the most optimal insurance coverage. NUTRITION Lisa Lewis, RD, CDE, Outpatient Registered Dietitian, Lisa in an outpatient registered dietitian who partners with patients on an appointment basis to offer individualized nutritional counseling, helping patients identify their nutrition goals and planning ways to meet those goals. She advises patients on the nutrients they need to help fight cancer and heal. Lisa helps them manage side effects to keep up strength and energy.

5 American College of Surgeons Cancer Program Practice Profile Reports (C3PR) Breast, Colon and Rectum Cancer Measures BREAST 2011 Radiation therapy is administered within 1 year of diagnosis for women under age 70 receiving breast conserving surgery for breast cancer Combination chemotherapy is considered or administered within 4 months of diagnosis for women under 70 with AJCC T1c N0 M0 or Stage II or III ERA and PRA negative breast cancer Tamoxifen or third generation aromatase inhibitor is considered or administered within 1 year of diagnosis for women with AJCC T1c N0 M0 or Stage II or III ERA and/or PRA positive breast cancer COLON Adjuvant chemotherapy is considered or administered within 4 months of diagnosis for patients under age of 80 with AJCC III colon cancer At least 12 regional lymph nodes are removed and pathologically examined for resected colon cancer RECTUM Radiation therapy is considered or administered within 6 months of diagnosis for patients under the age of 80 with clinical or pathologic AJCC T4 N0MO or stage III receiving surgical resection for rectal cancer BCS/ RT 92.6 MAC 100 HT 93.3 ACT RLN Adj RT Cancer Center Supporters Pink Ribbon Trailblazers This grass-roots organization that raises funds to support the offering of free screening mammogram clinics at St. Joseph Mercy Oakland (SJMO). Up to eight free clinics are offered each year to women residing in Oakland County who are either uninsured or underinsured. The group was founded by Pat Nolf and since 2007 has raised more than $161,000 to support the free mammograms Dan Dewey Dan s late father was a chemotherapy patient at SJMO and Dan accompanied him to every visit. One day during a chemotherapy session, when going to get coffee for this father, Dan asked Anybody want anything? So started Dan s Coffee Run, a tradition of going to the local Starbucks and purchasing coffee each Thursday morning for chemotherapy patients and their caregivers. American Cancer Society SJMO has a strong partnership with the American Cancer Society. Besides providing educational material to support our cancer patients, the American Cancer Society provides the volunteers who staff our Cancer Research and Support Center located within the Alice Gustafson Center on the hospital campus and provides the instructors and materials for the monthly Look Good Feel Better session at the hospital which provides information and cosmetic advice to women battling cancer. Game for a Cure Each May, Clarkston Lacrosse hosts its annual Game for a Cure Lacrosse Tournament, an all-day Lacrosse event where girls and boys Lacrosse teams grades 2-12) play to raise money for the SJMO Cancer Center for cancer research. In the Pink Derby Each August the Huron Valley Equestrian Committee hosts their In the Pink Derby, their annual all-day horseshow, at Milford High School. They donate the proceeds to SJMO to benefit breast cancer detection and support programs. A Mother s Wish A Mother s Wish embraces and supports Oakland County women and families impacted by breast cancer by easing the day-to-day burdens associated with a diagnosis while offering opportunities for hope, comfort, family togetherness and healing. Many of our programs are supported by donations.
