2 07 CANCER COMMITTEE Chairman s Report Over the last year, Cancer Treatment Centers of America (CTCA), Phoenix has remained committed to providing outstanding cancer care to current patients while working to serve even more patients in our community. At CTCA Phoenix, the Cancer Committee helps to ensure the quality of clinical care services, patient safety and development of innovative cancer programs. Each year, we set goals and perform studies to improve all cancer-related activities at our site. The membership of the cancer committee is multidisciplinary, representing physicians from diagnostic and treatment specialties and non-physicians from administrative and supportive services. This year, CTCA Phoenix rounded out its Medical Executive Team by electing and promoting the following doctors to leadership positions: Chief of Radiation Oncology Dr. Marnee Spierer was elected to the Chief of Staff position. Medical Director of the Lung Center and Medical Oncologist Dr. Ashish Sangal was promoted to Chief of Medical Oncology. Medical Oncologist and Hematologist Dr. Alan Tan was promoted to Clinical Research Medical Director. I am excited to announce that, over the past year, we have hired key members to our clinical care team including Medical Oncologist Dr. Vishangi Dave, Surgical Oncologist Dr. Vasilena Zheleva and Pathologist Dr. Shoib Sarwar. In this report, you will read more about the patients in our care, our community outreach efforts and the awards and accreditations we have been fortunate to receive. With the commitment of our Cancer Registry department, Committee members and leadership of President and CEO Rob Gould, we have been positioned to provide clinically excellent and truly compassionate care to our patient population. 0 Accreditations and Certifications About CTCA The information presented throughout the 08 Cancer Registry Annual Report reflects the data collected from January to December 3, 07 unless otherwise noted. 3 CANCER REGISTRY Report The Cancer Registry at CTCA Phoenix, Arizona has entered more than 6,36 cases into the database since its inception in 0. The Cancer Registry at CTCA Phoenix maintains a data system designed for the collection, management and analysis of data of individuals diagnosed with cancer. During 07,,08 new cases were accessioned into the Cancer Registry database. Of these cases, 486 were analytic and 595 were non-analytic. There were 485 male patients and 596 female patients. The Registry accessioned patients from 40 different U.S. states and territories, with the largest portion of patients (more than 35 percent) residing in Arizona. The American College of Surgeons Commission on Cancer mandates that A 90 percent follow-up rate is maintained for all analytic patients diagnosed within the last five years, or from the cancer registry reference date, whichever is shorter. A successful follow-up rate of 80 percent must be maintained on all analytic patients from the cancer registry s reference date. In 07, the Registry exceeded both of these standards with a 90 percent follow-up rate for both standards. The Registry staff performs the daily functions of the Tumor Registry and ensures data accuracy with continuous quality improvement reviews by Cancer Committee physicians. The Registry provides the Cancer Committee with information from which analysis of patient outcomes and evidence-based clinical decision-making can be performed. Cancer Registry Activities at CTCA Phoenix Submitted required data to the National Cancer Database, error-free, in the time frames required Submitted all required cases to the Arizona Central Cancer Registry (ACR) Coordinated and attended Cancer Committee meetings Played an integral role in preparing for the Commission on Cancer 08 survey Developed a registry abstract for each patient receiving cancer-directed therapy Established lifetime contact with each patient receiving treatment for newly diagnosed cancer through follow-up procedures within registry software Maintained all documentation for the accreditation of cancer program by the Commission on Cancer (American College of Surgeons) and the National Accreditation Program for Breast Centers Submitted breast and colon data to the Rapid Quality Reporting System (RQRS) Registry staff maintained membership in the Cancer Registry Association of Arizona (CRAAZ) and participated in an educational workshop, which was held at CTCA Phoenix in October 07 Registry staff maintained membership in the National Cancer Registrars Association (NCRA); one staff member attended the NCRA educational conference in Washington, D.C. in April 07 and reported information back to other staff members 3 CTCA Phoenix 08 CANCER REGISTRY ANNUAL REPORT

4 PRIMARY SITE: # of Cases Anus 4 Appendix 4 Bladder 3 Breast 9 Colon Esophagus 5 07 CANCER CONFERENCE Summary A multidisciplinary team including medical oncology, radiation oncology, radiology, pathology and surgery attended weekly Cancer Conferences. The nursing and ancillary staff also attended these conferences to ensure comprehensive representation. Together, they reviewed cases in detail and discussed stage, prognostic indicators, National Comprehensive Cancer Network Guidelines (NCCN), and plans for appropriate diagnostic studies, therapies and clinical trials. These prospective, patient-oriented and multidisciplinary physician care planning meetings serve as free consultations to patients and education for the medical and hospital staff. During 07, 44 General Cancer Conferences, 43 Breast Cancer Conferences and 3 Lung Cancer Conferences were held. A total of 38 cases were presented. 9 Screening and Prevention PROGRAMS Skin Cancer Prevention and Screening Event According to the Arizona Department of Health Services, Sun safety is especially important in Arizona, which ranks number two in the world in skin cancer incidence rates and has more than 300 sunny days annually. Because of these statistics, on May 0, 07, CTCA Phoenix, in collaboration with the Arizona Cardinals, held a free skin cancer screening event at the University of Phoenix Stadium. With doctors from four dermatology practices, 445 community members were provided a skin cancer screening. Of those, 99 required follow up. All partners also provided educational materials addressing sun safety and skin cancer awareness. Dr. David Boyd from CTCA Phoenix was available for further education and to answer any questions related to skin cancer prevention, detection or treatment. Prostate Pep Talk Prevention and Screening Event According to the Arizona Department of Health Services, prostate cancer is the most commonly diagnosed cancer among men. To raise awareness about prostate cancer and to encourage annual screening, on August 30, 07, CTCA held a free Prostate Pep Talk prevention and screening event in collaboration with the NFL Alumni Association and LabCorp. Approximately 00 guests attended this educational event, hearing Dr. Farshid Sadeghi, Medical Director of Genitourinary Center and Urologist, discuss the symptoms of prostate cancer and the different treatment options. Prostate cancer survivor and NFL Alumnus Nick Lowery also shared his story about how early detection saved his life, and encouraged male attendees to get screened. Through this national collaboration, LabCorp provided free PSA screenings from September to October 5 to the first,000 eligible respondents. Educational materials were provided to all attendees. Breast Cancer Screening Event According to the Arizona Department of Health Services, 3,400 Arizona women will be diagnosed with breast cancer in a year. This is the most common cancer among women making up nearly a quarter of all diagnoses. Additionally, Breast cancer is the second leading cause of cancer death among women in Arizona. If caught early, women have a 98 percent chance of beating breast cancer. To address the need for early detection, CTCA held a free breast cancer screening event at Golden Gate Community Center on November, 07. During the event, 49 screenings were provided, with two women receiving diagnostic follow-up. Both follow-ups were shown to be benign. The group was predominantly made up of women in underserved areas lacking health insurance coverage. In addition to screening, each attendee was given educational materials about conducting breast self-exams, information about mammography, and nutrition information, including a healthy eating guide and healthy recipe cards. 445 free skin cancer screenings 99 required follow up to those screenings,000 free PSA screenings provided from LabCorp 49 breast cancer screenings 9 CTCA Phoenix 08 CANCER REGISTRY ANNUAL REPORT

10 Accreditations AND CERTIFICATIONS CTCA Phoenix is accredited and recognized by several renowned professional health care organizations that assess and monitor the quality of patient care. The Joint Commission The Joint Commission s Gold Seal of Approval for Hospital Accreditation reflects a commitment to providing safe and effective patient care and a willingness to voluntarily undergo rigorous, unannounced onsite surveys. Accreditation requires compliance with standards related to areas such as patient rights, environment of care, infection prevention, leadership and medication management. Quality Oncology Practice Initiative (QOPI) The QOPI Certification Program, an affiliate of the American Society of Clinical Oncology (ASCO), recognizes outpatient practices that meet the benchmarks for breast, colorectal, non-small cell lung, non-hodgkin lymphoma, gynecologic and prostate cancers. This seal designates those practices that scored high on key QOPI quality measures and met rigorous chemotherapy safety standards established by ASCO and the Oncology Nursing Society. Commission on Cancer (COC) The COC recognizes cancer care programs for their commitment to providing comprehensive, high-quality and multidisciplinary patient-centered care. COC Program Standards require facilities to create meaningful processes for implementation of patient-centered care. National Accreditation Program for Breast Cancers (NAPBC) Accreditation by the NAPBC is granted only to those centers that are voluntarily committed to providing the highest standards of care to patients with diseases of the breast.
