5 About vaccine management It is a system of transporting and storing vaccines within the safe temperature range of +2 0 C to +8 0 C Begins from time of vaccine manufacture. Must be maintained throughout its transportation to the State/Territory vaccine distribution centres. Ends when the vaccine is administered.
6 Why is it important? Duty of care in ensuring clients receive potent and effective vaccines Vaccines expensive/short supply Quality assurance measure Problems can be detected before ineffective vaccine is used Exposure to heat or freezing temperatures has a cumulative effect on vaccine viability Eliminates re-vaccination of clients
7 Vaccine Management Protocol (VMP) The following information should be included:- Equipment - Fridge, thermometers, eskies Communication relevant contact details, PHU,QHIP, fridge technician Who is responsible for what Ordering, receiving disposing of vaccines Managing a CCB Education and Training in Vaccine Management
8 Purpose-built vaccine refrigerator Continuous temperature monitoring device Data logger
9 Modified domestic refrigerator Minimum/maximum thermometer Location in fridge important 1.5 volt battery (have spares on hand)

11 Be careful about what goes in your vaccine fridge!
12 Cold Chain Breaches (CCB) A cold chain breach is when vaccine storage temperatures have been outside the recommended range of +2 C and +8 C This does not include temperature deviations or excursions up to +12 C lasting no longer than 15 minutes when stocktaking or restocking
13 Implications of CCB Disruption to service provision in the practice No protection to vaccine preventable disease Increased risk of adverse reactions Loss of community confidence Loss of credibility of the staff Loss of self esteem Loss of revenue Extra work!
14 The Practice had a CCB, so I m waiting for viable vaccines!
15 Cold chain breaches Action to take when identified: Isolate the vaccine in the fridge between +2 C and +8 C and mark DO NOT USE VACCINES UNTIL FURTHER NOTICE Note and record the minimum and maximum temperatures at the time of the breach Contact QHIP as soon as possible after the breach during office hours by telephoning Do NOT discard any vaccines unless advised

17 Tips Store vaccines in PBVR Nominate a primary and secondary staff member to be responsible for vaccine management Ensure current protocols and procedures are in place for vaccine management Ensure all involved in vaccine management are trained Support of management
18 Tips Regular self audits and VMP update Annually Temperature monitoring twice daily Each facility should have plan in place for cold chain breaches and power failure Report temperatures outside 2 and 8 degrees to QIP ASAP Follow the guidelines regarding using ice packs, packing and monitoring eskies and coolers
19 Discarding vaccines According to the Environmental Protection (Waste Management) Regulation 2000 vaccines are classed as a restricted S4 drug under the Health Drugs and Poison Regulation 1996 and are therefore considered pharmaceutical waste High temperature incineration (ERA76(e)) is currently the only option that can be used for the treatment of pharmaceutical waste The incineration process renders the waste inactive and unrecognisable Check with your waste disposal contractor regarding how the waste is destroyed

21 Case study 1 You arrive at work at 730am to find that the fridge is alarming. The fridge temperature is 23 degrees. What would be the first step in managing this issue? What would be your follow up actions? After you have dealt with the CCB you realise that one of the other nurses has given an effected MMR vaccine to a child. What should you do? How could you prevent this happening again in the future?
22 Case study 2 Renovations are occurring in the shopping centre where your practice is situated; builders have advised that the power will be off for most of the day tomorrow. What is the first step? Explain how you would manage the situation tomorrow to ensure cold chain is maintained? You have private vaccines in your fridge what should you do with them?
23 Case study 3 It is 5pm and the power has just gone off and you have a glassfronted purpose built vaccine fridge. Energex has advised that the power not coming back on anytime soon. You know from experience that you only have 20 minutes until your fridge reaches +12C. What do you do first? You have reviewed your Vaccine Management Protocol list the steps you would take to manage this situation Explain what you would do when the power supply is restored?
