1 Marion County Board of Health Meeting Minutes March 16, :00 P.M. Board Members Present: Dr. Stan Morrison, Mike Morton, Dr. Aziz Rahman, Dr. Matt Stedelin and Paula Strother Others Present: Lori Ryan, Administrator; Melissa Mallow, Environmental Health Director; Shelly Yoder, Director of Nursing; Tara DeWitt, Health Educator; Crystal Russell, Administrative Secretary; and Bruce Kropp, WJBD Radio I. CALL TO ORDER The Marion County Board of Health met on March 16, at the Marion County Health Department, Salem office. Dr. Stedelin called the meeting to order at 7:00 p.m. II. QUORUM A quorum of five board members was present. III. MINUTES Mike Morton made a motion to accept the December 15, 2009, quarterly Board of Health meeting minutes as presented; Dr. Aziz seconded the motion. All were in favor; none opposed. The minutes were approved. IV. FINANCIAL REPORT Stedelin called for a review of the Financial Report. Lori Ryan reported that the cash flow balance at the end of the first quarter for Fiscal Year 2010 was $779,773.70; the revenue was $415,277.71, a positive variance of $136, Expenditures for the end of the first quarter were $276,926.28, or 24.11% of the first quarter budget. Ryan stated the Health Department currently has four CD s at Iuka Bank totaling $350,000; 3 - $100,000 and 1 - $50,000. Stedelin made a motion to accept the financial report; Dr. Aziz seconded the motion. All were in favor; none opposed. The Financial Report was approved. V. 2 VI. NEW BUSINESS a. Activities: i. Health Education: Tara DeWitt, Health Educator DeWitt presented information in January to the 50-Plus Group for National Influenza Vaccination Week. Kick Butts Day is Wednesday, March 24. Approximately 535 students from four junior high schools and Salem High School will view a power point presentation educating how tobacco companies target youth to use tobacco, current Social Norm of Marion County high school students, learn the truth that most don t use tobacco, and also learn that students can choose not to be manipulated by tobacco companies. A Social Norm Survey will take place in all six Marion County High Schools the week after Easter Break. DeWitt hopes the perceived norm is closer to the actual norm. DeWitt will show how the Marion County Health Department s Social Norm Marketing Campaign has affected the actual and perceived norm at the Regional Tobacco meeting later in March. ii. Environmental Health: Melissa Mallow, Director of Environmental Health Smoke-Free Illinois Act: The Health Department has received 68 complaints for 38 businesses from the Illinois Department of Public Health complaint line, as of March 16. Nineteen facilities were restaurants/bars and 19 were offices/workplaces. The Health Department has received no complaints since the last board meeting. Tattoo and Body Piercing: The Environmental Health Division of the Health Department completed three body art facility inspections in Marion County and saw no violations. All three facilities were very welcoming and pro-inspection. At each facility, a tattoo or body piercing was observed. According to the IDPH grant contract, Marion County Health Department is to receive 75% of the permit fee upon completion of field work. Each permit fee is determined by the number of stations at each facility. Solid Waste: As Solid Waste Coordinator for Marion County, Mallow offered information concerning the Centralia Recycling Center, who recycled over 736 tons of material in In April, the facility will start an Electronic Recycling Program, which allows the facility to accept computer monitors, desk top printers, free standing printers, computer towers, keyboards, copy machines and televisions. A date has not been set for the first drop-off for this free public service. Dr. Morrison questioned the fencing statute for the salvage yard on Route 37 South of Salem. Mallow noted that the company is not required to have a fence surrounding the perimeter of the salvage yard. iii. Nursing: Shelley Yoder, Director of Nursing Effective December 2009, the WIC Caseload increased from 1,454 to 1,511. WIC caseloads for the first quarter were 1,373 for December, 91%; 1,323 in January, 88%; and 1,313 in February, 87%. Yoder noted surrounding counties were experiencing Marion County Board of Health: March 16, 2010 Page 2 of 6

3 similar decline of caseloads. The reduction of WIC caseload could potentially mean that people are moving from the respective local counties to other areas. The Health Department will continue to monitor the caseload counts closely to see if these trends continue. HIV Tests: The Health Department completed 15 HIV tests with no positive results in the first quarter for Fiscal Year Due to funding cuts, the HIV CTR Grant for 2010 was not received. The Health Department is offering HIV testing through the STD Program for $10, to cover staff time for counseling and follow-up. STD Testing: Sixteen urine tests for gonorrhea and chlamydia and 16 syphilis tests were completed the first quarter. One positive test for chlamydia, the remaining tests results were negative. During 2009, 87 Risk Assessment Surveys were completed for the IDPH STD Program through the Health Department s STD testing services. Surveys completed by 63 males contain the following responses: 73% had sex while drunk or high in the last 30 days; 11% had a previous STD; 14% had sex with someone they met over the internet; 19% never use condoms; and 35% have had sex with an anonymous partner. The remaining surveys were completed by 24 females with the following responses: 33% had sex while drunk or high in the last 30 days; 33% had a previous STD; 8% had sex with someone they met over the internet; 13% never use condoms; and 13% have had sex with an anonymous partner. Lead Poisoning Prevention Services: The Health Department completed 151 lead tests during the first quarter, with 3 children currently being followed for lead case management services, and 2 children are pending a confirmatory lead venous test after a high capillary lead test. No lead nursing home visits were conducted this quarter. H1N1 Clinics: Since starting the H1N1 Clinics in October, the Health Department administered 8,646 doses of H1N1 vaccine to Marion County residents. The Health Department transferred 5,830 doses of vaccine to other healthcare providers for administration to staff, employees and residents. The demand for H1N1 vaccine tapered greatly since the end of February. The vaccine clinics will continue to be offered by walk-in clinics every Monday in Salem and every Wednesday in Centralia though the end of April or until the demand stops. According to the latest IDPH H1N1 Briefing Report dated March 5, an average of over 4-million doses of H1N1 vaccine have been allocated for Illinois, over 3-million doses ordered, and approximately the same amount of vaccine has been shipped to providers in Illinois. An average of 77,000 doses have been administered to the 6-23 months group, 140,000 doses administered to the months group; 482,000 doses administered to the 5-18 year group, 92,000 doses administered for the year group, 365,000 doses administered to the year group, 242,000 doses administered to the year group, and 195,000 doses administered to the 65 and older age group. Approximately 1,591,000 H1N1 vaccinations have been administered to Illinois residents Flu Clinics: The Health Department began the flu planning process in January by sending out first requests for potential off-site locations, with 21 sites Marion County Board of Health: March 16, 2010 Page 3 of 6

4 responded with request for clinics. The Health Department is focusing on locations that are out-of-town for off-site locations and limit some off-site locations that were previously held in the direct vicinity. The Health Department has received large turnouts for walk-in flu clinics at both offices, and is now focusing efforts to provide on-site locations to clients. The WHO has determined the components of the Influenza vaccine, which will contain the following: A/California/7/2009 H1N1 (Novel H1N1 strain), A/Perth/16/2009 H3N2, and B/Brisbane/60/2008. Immunization Outreach: Kay Crooks, Coordinator for Grandparents Raising Grandchildren Support Group at Marion County Senior Services, requested Yoder to present information regarding vaccines. Yoder distributed handouts and vaccine information, an overview of vaccine preventable diseases and information on how vaccines work. The 2010 child, adolescent, and adult vaccine schedules were also reviewed with the group in February. Labs: Due to multiple requests from clients, T3, T4, erythrocyte Sedimentation rate, C-reactive protein, and 25-hydroxy Vitamin D labs have been added to the Health Department s standing orders. [A Lab Services brochure was distributed to Board of Health members.] A total of 68 labs were drawn the first quarter, including 6-liver panels, 7-CMP, 3-PT, 2-PSA, 4-TSH, 10-lipid panels, 22-executive profiles, 1-TB Quantiferon Gold test, 2-Herpes test, 4-HGB A1c, 2-CBC, 1-Hep C Ab, 2-HbsAg, 1-T4, and 1-T3. 12-Week Health Challenge: The first step to being healthy is to know what health challenges to work on, and moving forward to improve them. Everyone needs to take an active approach in obtaining his or health goals and working to stay healthy. At the end of February, the Health Department ran promotions over the radio and in the newspaper to create awareness of the new program. Beginning March 1, the Health Department is offering an executive profile, blood pressure check, weight check and health information for $45. Residents are challenged to be aware of their health issues and work to improve them. Over a 12-week period, residents are encouraged to return every week for a weight and blood pressure check, depending on what goals they have identified to improve on. It is recommended to repeat the labs in 12 weeks to see if goals were met. Currently, 42 appointments have either been done and/or are scheduled through the end of March. [A Health Challenge packet was distributed to Board of Health members.] Communicable Disease: Beyond the 2 cases of active TB counted in Marion County in 2009, the Health Department also has a Spanish-speaking client who is positive for TB. Health Department staff is working with a DHS translator to communicate with the client. The client completed initial therapy on February 28 and has started continuation therapy on March 1. Completed TB treatment is anticipated for July 4. Marion County had two H1N1 deaths reported in January. Both cases were not vaccinated against the H1N1 virus. Surveys: Ninety-nine general surveys were completed in January, with the following responded as Very Satisfied : 91% with the convenience of the office location; 88% with the length of time they spent waiting in the office; 91% with the amount of privacy available at the visit; 95% with the courtesy and respect shown by staff; 94% with the Marion County Board of Health: March 16, 2010 Page 4 of 6

5 skills and knowledge of the staff; and 94% with the overall services they received. Results for the 155 Maternal Child Health Surveys include: largest percentage of clients stay at home was 48%, followed by working part-time, 22%; friends remain the primary referral source for WIC services; 100% of clients responded that the staff are helpful and respectful; 90% responded that appointment times are very good; the top two reasons for missing appointments remain work or school, and transportation; 89% responded the food instruments are easy to use; 98% responded the handouts are readable; 86% responded they had enough food to eat, followed by 11% responded they had enough to eat sometimes; and 66% responded that home visits were very helpful. iv. Administration: Lori Ryan Janet Bryant, an employee at the Centralia office for 11 years, retired on March 9. Ryan has received 76 applications, interviewed 16 for the front desk clerk position. Ryan is interested in applicants with medical background and plans to make a selection by the end of the week. The 12-Week Health Challenge addresses the priorities of the IPLAN: obesity, cardiovascular disease, smoking, and substance abuse. The $45 charge is for only the lab work. The Health Department will no longer receive funding for the HIV Grant, which has been received 10 years. For the past couple years, the Health Department has received $7,000 each year. Instead of eliminating the testing and counseling altogether, the Health Department will continue to offer the services, but without state funding. A back-up generator has been purchased with the Pandemic Flu funds for the Salem office. The generator will power the entire building. Ryan is looking into having a generator installed at the Centralia office as well. b Annual Report Ryan reviewed the 2009 Annual Report. All information within the report closed the previous fiscal year, November 30, Ryan highlighted the TB testing and active cases, blood pressures, STD s, vaccinations, international travel, lead screenings, HIV testing, paternity testing, food inspections, sewer inspections, and tanning inspections. Ryan also highlighted the H1N1 article which stated the Health Department vaccinated 7,680 people against the H1N1 virus. Ryan added a state report shows that a grand total of 11,312 Marion County residents have been vaccinated, which is 29% of Marion County. The Annual Report Financial Statement for 2009 concluded with the total revenue of $1,164, and total expenditures of $984,695.18, which completes the fiscal year with revenue over expenditures of $180, Ryan highlighted the revenue income, with 47% from grants, 32% from fees of service, 19% from taxes and 2% from other sources. 6 c. IPLAN 2011 The Health Department is to complete a community assessment every five years, which is part of the certification process as a local health department. This assessment is the IPLAN, Illinois Project of Local Assessment of Needs. Community members from police departments, schools, doctor s offices, and general public meet and review Marion County statistics then list three top priorities for Marion County. The IPLAN is a community effort to make a healthy difference. All Board of Health members are welcome to be part of the committee. The first meeting is March 25. Ryan s goal is to have the IPLAN completed by June 15. d. OPEN MEETING ACT Ryan reviewed the new Open Meetings Act handout. The Marion County Board of Health will allow a Board of Health member to electronic attend a board meeting if a quorum is physically present at the location where the meeting is to be conducted and if the member is prevented from physically attending because one of the following reasons: personal illness or disability employment purposes or the business of the public body a family or other emergency Dr. Morrison made a motion to accept the Open Meetings Act Policy as stated above to be added to the Marion County Board of Health s Attendance Policy. Dr. Stedelin seconded the motion. All were in favor; none opposed. The Policy was adopted. Closed Session verbatim tapes are kept on file. Ryan stated minutes are then to be typed from the closed session and approved by the Board of Health. The Board of Health will then meet bi-annually to decide if the closed sessions can be opened to the public or remain closed. e. SALARY SCHEDULE Ryan stated the Health Department s current clerical starting rate is $9.23. The Marion County Courthouse starting clerical rate is $9.56. Mike Morton made a motion to accept the courthouse rate of $9.56 for clerical starting rate. Paula Strother seconded the motion. All were in favor; none opposed. The new rate was approved. VII. ADJOURNMENT Mike Morton made a motion for adjournment at 7:37 pm; Dr. Morrison seconded the motion. The next meeting is scheduled for June 15, 2010, at the Salem office.
