8 What did we find when we asked our health professionals? Agree it is their role to screen for oral disease and diabetes. Better collaboration, and formal communication is needed between medical and oral health services to improve diabetes care. Barriers include limited time in consults, competing priorities, differing funding models and non-integrated data bases. More education is needed around diabetes and periodontal risk. A better understanding of the different health professionals roles is needed.
9 Barriers to co-management siloing of dental and medical practices incompatible medical record software formal referrals not routine oral health not included in Diabetes Cycle of care not included in Medicare EPC funding limited access to public dental services
10 Recommendations for Practice in the 21 st Century raise issue of oral health in diabetes risk screening create formal referrals, request feedback include oral health professionals in management plans understand eligibility criteria for community dental services referrals to private dental support good oral health practice
11 Continuing and future research PHASE 4 validation of Periodontal Risk Assessment tool PHASE 5 test the efficacy of a interdisciplinary education program FINAL PHASE Test the efficacy of a developed model of shared responsibility for diabetes management that includes oral health
