The materials employed range from materials used to obturate the root canals namely gutta-percha points and root-canal sealers to root-end filling and perforation repair materials. The treatment success depends on how well these materials performed clinically to meet the needs of the particular procedure performed. Classically resin based sealers used in conjunction with gutta-percha points have been the gold standard and although no bonding is reported between self and gutta-percha the sealer ends micro mechanically to the dentine by penetrating the dentinal tubules and creating sealer tags. High success rates have been reported. For perforation repair and root-end filling the modified zinc oxide eugenol has shown considerable success. The introduction of mineral trioxide aggregation clinical dentistry has been revolutionary as the material is hydraulic and thus suitable for the particular clinical applications where control of moisture is impossible. Furthermore the highly alkaline environment created by hydration results in adequate antimicrobial properties and induction of cell growth and proliferation. Mineral trioxide aggregate has a number of disadvantages namely the claims of material impurity, tooth staining resulting form bismuth oxide, poor handling and long setting time. The new generation of tricalcium silicate-based cements known popularly as Bioceramics have improved properties and thus promise grater clinical success. Course description: The aim of this comprehensive course is to highlight the emergence of the novel biological based approach used to manage immature non-vital teeth. For many decades, apexification has been the routine practice, and despite a literature replete with discussion, including recent artificial barrier methods, there has been no major breakthrough to improve this treatment. Recently, two new clinical concepts have emerged. One involves a revitalization approach to achieve tissue generation and regeneration. In this method, new living tissue is expected to form in the cleaned canal space, allowing continued root development in terms of both length and thickness. The other is the active pursuit of pulp/dentine regeneration via tissue engineering technology to implant or re-grow pulps. This course will overview these recent concepts with emphasis on the tips and tricks for the clinical technique used.
