6 II. TESTING FOR DIABETES IN ASYMPTOMATIC PATIENTS

7 Criteria for Testing for Diabetes in Asymptomatic Adult Individuals (1) 1. Testing should be considered in all adults who are overweight (BMI 25 kg/m 2 *) and have additional risk factors: Physical inactivity First-degree relative with DM High-risk race/ethnicity Women who delivered a baby >9 lb or were dxed with GDM HTN ( 140/90 mmhg or on therapy for hypertension) *At-risk BMI may be lower in some ethnic groups. HDL chol level <35 mg/dl and/or a trig>250 mg/dl PCOS A1C 5.7%, IGT, or IFG on previous testing Conditions: insulin resistance (e.g., severe obesity, acanthosis nigricans) ADA. 8 Criteria for Testing for Diabetes in Asymptomatic Adult Individuals (2) 2. In the absence of criteria (risk factors), testing for diabetes should begin at age 45 years 3. If results are normal, testing should be repeated at least at 3-year intervals, with consideration of more frequent testing depending on initial results (e.g., those with prediabetes should be tested yearly), and risk status ADA. 9 III. PREVENTION/DELAY OF TYPE 2 DIABETES

10 Recommendations: Prevention/Delay of Type 2 Diabetes Refer patients with IGT, IFG, or A1C % to ongoing support program Targeting wt. loss of 7% of body wt. physical least 150 min/wk of moderate activity (eg, walking) F/U counseling is important for success Based on cost-effectiveness of diabetes prevention, such programs should be covered by third-party payers ADA. 11 Recommendations: Prevention/Delay of Type 2 Diabetes Consider metformin for prevention of type 2 DM if IGT, IFG, or A1C % BMI >35 kg/m 2, age <60 years, women with prior GDM Prediabetes: monitor annually for development of DM Screen for and treat modifiable risk factors for CVD ADA. 22 Correlation of A1C with Average Glucose Mean plasma glucose A1C (%) mg/dl mmol/l These estimates are based on ADAG data of ~2,700 glucose measurements over 3 months per A1C measurement in 507 adults with type 1, type 2, and no diabetes. The correlation between A1C and average glucose was A calculator for converting A1C results into estimated average glucose (eag), in either mg/dl or mmol/l, is available at ADA. V. Diabetes Care. 24 Glycemic Recommendations for Nonpregnant Adults with Diabetes A1C <7.0% * Preprandial capillary plasma glucose mg/dl * ( mmol/l) Peak postprandial capillary plasma glucose <180 mg/dl * (<10.0 mmol/l) *Goals should be individualized based on these values. 35 Recommendations: Hypertension/Blood Pressure Control Screening, diagnosis and treatment BP measure at every routine visit BP, confirm on a separate day Goal: 130/80 ACE inhibitors or ARB Multiple drug therapy (two or more agents at maximal doses) If ACE inhibitors, ARBs, or diuretics are used, monitor serum creatinine/egfr, K ADA. 38 Recommendations: Glycemic, Blood Pressure, Lipid Control in Adults A1C <7.0% * Blood pressure <140/80 mmhg Lipids: LDL cholesterol <100 mg/dl (<2.6 mmol/l) Statin therapy for those with history of MI or age >40+ or other risk factors *More or less stringent glycemic goals may be appropriate for individual patients. Goals should be individualized based on duration of diabetes, age/life expectancy, comorbid conditions, known CVD or advanced microvascular complications, hypoglycemia unawareness, and individual patient considerations. Based on patient characteristics and response to therapy, lower SBP targets may be appropriate. In individuals with overt CVD, a lower LDL cholesterol goal of <70 mg/dl (1.8 mmol/l), using a high dose of a statin, is an option. ADA. VI. Prevention, Management of Complications.
