2 Background 2002 Eye screening programme launched in Sunderland 2005 Foot screening included in screening encounter 2007 Sunderland and South Tyneside Programmes merged Eye & foot screening continued to be delivered by screening programme, all other checks undertaken in GP practice (fragmentation of tests, multiple appts for patients, variance in uptake) April 2015 New One Stop Service launched offering 9 checks in single appointment.
3 Why change? Patient preference survey = 94% of patients wanted a one stop service. To free up GP time in practice to afford more time to complex patients. To work in collaboration with CCG s and the Acute Trust to improve uptake of annual diabetic reviews. Practice variation exists of 30-80% uptake (NDA), screening uptake currently 84%. To deliver all 9 of the N.I.C.E. recommended healthcare processes for patients with diabetes in a single annual appointment. There are approximately 9,200 South Tyneside patients registered with the programme.
4 What tests are included? (1) Retinal Screening (2) Feet Assessment (3) Blood Pressure measurement (4) Smoker? (5) HbA1c (6) Cholesterol (7) Blood Creatinine (8) Urinary Microalbumin:Creatinine ratio (9) BMI Measurement
5 How does the appointment flow?
6 How is the information shared? All information is recorded into the existing diabetes register (Prowellness System). A clinical visit summary is handed to the patient and a copy sent to the GP on the day of the visit confirming the attendance and which tests have been performed. A full clinical result letter to GP follows and patient, detailing the outcomes i.e. blood test results, urinalysis etc. Blood tests reported via ICE system, shared with GP s within hours of screening.
7 Patients Take Home information (also shared with GP)
8 How does the GP know when to review their patient? A full list of diabetic patients held by the screening programme was sent per GP practice showing the date when screening recall was due. Practices can calculate when their patients are to be rescreened and can align the GP annual practice review to occur after screening is completed. The checklist visit summary document can also act as a trigger to the GP to arrange annual review as results will be imminent.
9 How will uptake be monitored? The screening programme uses existing bespoke eye screening software to monitor uptake. Reports can be generated to show uptake/dna rates per practice with patients identified to GP s for education, follow-up and encouragement to participate. Regular Patient Experience Surveys/Feedback show excellent responses to enhanced service. Reports from Prowellness Diabetes Register reflect more outcome data to monitor uptake per test.

11 Challenges? Not all patients will consent to all 9 tests and GP s acknowledge that there may be some gaps in data, for example; Those who refuse blood testing Bilateral amputees Wheelchair bound patients who cannot be weighed Patients who may not be able to provide a urine sample The service is only operational in South Tyneside, there are currently no plans to mirror the service in Sunderland.yet. The eye screening is commissioned by PHE and the remaining 8 tests are commissioned by South Tyneside CCG. One pathway must not compromise the other (Eye screening has pre-set national targets/kpi s to meet).
