For convenience, I am including them in the same . Please treat each as a separate request under the terms of the Act. The requests are as follows: 1. Please supply a list of the providers from which your Clinical Commissioning Group (CCG) commissions diabetes services. NHS Vale of York CCG commissions diabetes services from York Teaching Hospital NHS Foundation Trust. 2. Please confirm or deny if your CCG has within its CCG Outcomes Indicator Set an indicator relating to diabetes care. I can confirm that NHS Vale of York CCG has diabetes care within it s outcome indicator set. 3. Please list the indicator your CCG has within its CCG Outcomes Indicator Set which relates to diabetes. 2 Increase the rate of uptake of education for people with Type 2 diabetes; Increase self-efficacy for people with diabetes 4. Please confirm or deny if your CCG makes use of financial incentives to deliver improvements in diabetes care through the contracts it holds with providers. I can confirm that the CCG does not use financial incentives to deliver improvements. The CCG is currently working with partners to deliver service redesign utilising a shared outcome approach. 5. Please list the financial incentives to deliver improvements in diabetes care which your CCG makes use of in the contracts it holds with providers. Not applicable, please see answer to question 4 above. 6. Please state the number of people diagnosed with diabetes for which your CCG is responsible, broken down by type 1 and type 2 diabetes if possible. The CCG can only provide an overall estimate of diagnosis rate, including both Type 1 & Type 2, which is 66%. The latest figures the CCG holds indicate that there are 18,765 people diagnosed with diabetes in the Vale of York CCG locality. 7. Please provide an estimate of the number of people living with undiagnosed diabetes in the area covered by your CCG. I can confirm that the information you requested is not held by NHS Vale of York CCG. York Teaching Hospital NHS Foundation Trust may hold the information you have requested. Please provide an estimate of your total anticipated expenditure on diabetes services in , broken down by expenditure on type 1 and type 2 diabetes if possible. I can confirm that NHS Vale of York CCG do not hold the information requested broken down by type 1 and type 2 diabetes. However, current total expenditure is 888,000 (including associated dietetics referrals) with an estimated breakdown of 50% for both Type 1 and Type 2 diabetes. 9. Please provide an estimate of the proportion of your CCG s expenditure on diabetes services in which is required to treat the complications of diabetes. I can confirm that NHS Vale of York CCG does not hold the information you have requested. 2

3 10. Please confirm or deny if the providers of diabetes care which your CCG holds contracts with support the provision of diabetes care through the use of each of the following technologies: a. Insulin pumps I can confirm providers would use insulin pumps in line with NICE guidance. b. Smart glucose meters c. Continuous glucose monitors In relation to questions 10 b. and c., I can confirm that NHS Vale of York CCG does not hold the information you requested as decisions on which technologies to use is clinically based. 11. Please state, for each of the technologies supported by providers with which your CCG holds contracts (listed in #10), if your CCG favours the use of a specific brand or product. I can confirm that the CCG does not favour a brand or product. However, the CCG have recently undertaken an option appraisal review of Blood Glucose Meters (BGM) resulting in a preferred BGM machine to be used in 2014/15, (for predominant use by people with Type 2 Diabetes). 12. Please confirm or deny if your CCG has issued guidance to providers which encourage the use of specific brands or products used for the treatment of diabetes. I can confirm that the CCG will be issuing guidance to providers in relation to BGM machines for 2014/15, please see answer to question 11 above. This is being implemented as part of the service re-design project. 13. Please supply all guidance issued to providers which encourage the use of specific brands or products used for the treatment of diabetes. Not applicable, guidance is not yet available, please see answer to question 12 above. 14. Please state the total number of people living with diabetes currently in receipt of insulin pump therapy in your CCG area. I can confirm that the information you requested is not held by NHS Vale of York CCG. York Teaching Hospital NHS Foundation Trust may hold the information you have requested. In relation to questions 15-17, I can confirm NHS Vale of York CCG does not hold this information at this level of detail. Insulin pump starts fall under a High Cost Drugs budget. 18. Please supply any communications to providers with which your CCG holds contracts during the current financial year which concern the funding on insulin pump starts. Please confirm or deny if your CCG has made an assessment of its compliance with all items of extant NICE guidance on diabetes care. I can confirm that the CCG has made an assessment of its compliance with all items of extant NICE guidance on diabetes care and this has been taken into account during the current service redesign phase. 20. Please supply details of the assessment your CCG has made of its compliance with all items of extant NICE guidance on diabetes care. Please confirm or deny if your CCG funds the provision of information resources for patients living with diabetes. The CCG does not directly fund this but some information resources will be funded indirectly through community contract and Out Patient tariffs. 22. Please list the information resources provided to patients living with diabetes funded by your CCG. This is currently in the process of being redesigned. 4

5 23. Please supply all correspondence in between your CCG and the Academic Health Science Network (AHSN) which covers your CCG area which relates to diabetes. I can confirm that there has been no communication between the CCG and the Academic Health Science Network (AHSN) which covers the Vale of York CCG area which relates to diabetes. 24. Please list the total number of complaints about diabetes services which have been made to your CCG in the current financial year. No complaints have been received by the CCG in relation to diabetes services.
