1 Date: November 205 Chelsea and Westminster Hospital Information Governance Team Chelsea Harbour Harbour Yard Unit, st Floor London SW0 0XD Our Ref: FOI Dear Requester Thank you for your information request received by us on 5 October 205 This request has been handled under the Freedom of Information Act Please note that Chelsea and Westminster Healthcare NHS Foundation Trust merged with West Middlesex University Hospital in September 205, for this reason our response covers both sites. Your request Earlier this year we made a request under the Freedom of Information Act relating to the care of people affected by dementia in your hospital. Following this I would now like to ask some additional questions under the Freedom of Information Act. Please see these below. There is an objective in the National Dementia Strategy for Dementia for improved quality of care for people with dementia. We are therefore interested to know: and over who have fallen in hospital in 20-5 or the last year for which statistics Number of patients aged 65 and over with a coding of dementia who have fallen in hospital in year for which statistics and over discharged between the hours of 200 and 0600 in are and over with a coding of dementia discharged between the hours of 200 and 0600 in financial year for which statistics The number of delayed discharges and delayed transfers of care for patients aged 65 and over in The number of delayed discharges and delayed transfers of care for patients aged 65 and over with a coding of dementia in financial year for which statistics Page of 8
