6 Characteristic of eyes with PAC: Hallmark: - shallow AC - narrow angle. Mean ACD in PAC eyes is 1.8 mm. Angle closure becomes a rarity when anterior chamber depth exceeds 2.5 mm Characteristic of eyes with PAC: Small AC volume Small corneal diameter Short axial length 6

7 The AC shallowing is mostly related to the age-related increase in lens thickness and more anterior position of the lens. The axial lens thickness is greater than in normal subjects, and the thicker lenses are significantly more anteriorly positioned than in normal eyes. Current Surgical Treatment Options for PAC 7

8 Trabeculectomy Higher risk of failure Postoperative AC shallowing Malignant glaucoma Cataract Because the anterior chamber anatomy is a predisposing condition in the PAC spectrum, a surgical treatment to deepen the anterior chamber for these eyes may prevent progression of the disease. 8

11 Lens Extraction The potential importance of this definitive procedure to correct persistent pupillary-block and angle crowding in both the treatment and prevention of acute and chronic angle closure glaucoma cannot be overestimated. IOP reduction not observed to the same extent in POAG eyes Some studies found that IOP control in POAG eyes are largely unaffected by cataract surgery Others found a reduction in IOP that is smaller, and less sustained than that seen in PACG eyes. The mechanism for IOP reduction in POAG eyes is unclear. 11
