Personal Statement I am an exercise and thermal physiologist with extensive training in integrative cardiovascular physiology. My research experiences have honed my skills and desire to conduct high-quality research on a variety of clinically relevant questions with greater impact to the general public. I have followed the traditional path of obtaining extramural funding at each stage of my early career. I obtained a prestigious American Heart Association pre-doctoral fellowship during my training at the Thermal and Vascular Laboratory at the Institute for Exercise and Environmental Medicine, University of Texas Medical Center, Dallas that focused on hyperthermia therapy on acute hypotensive response and glycemic control in adults with diabetes. I then joined a nationally recognized NIH T32 postdoctoral fellowship training program for exercise rehabilitation scientists at the University of California-Irvine, School of Medicine which provided training in pediatric exercise physiology and obesity. I then completed the postdoctoral translational burns fellowship program at the University of Texas Medical Branch and Shriners Hospitals for Children at Galveston, where I obtained an NIH Diversity Supplement funded by Eunice Kennedy Institute of Child Health and Human Development to Promote Diversity in Health- Related Research. This work allowed for exceptional training in randomized control trials, exercise science, cardiovascular and metabolism research in clinical populations. I subsequently was awarded a Ruth L. Kirschstein National Research Service Award (F32), funded by the National Heart Lung and Blood Institute. Further, I am a current scholar (cohort 6) in the Program to Increase Diversity in Cardiovascular Health-Related Research (PRIDE-CVD) sponsored by the National Heart, Lung, and Blood Institute, which aims to support minority scientist that focus on addressing cardiovascular disease health disparities research. My previous training has also involved exercise training and its adaptations at the whole-body level (body composition, strength, aerobic capacity) and at the metabolic and cellular level (mitochondrial function, glycemic control, and protein synthesis) in burned populations. Much of this work established exercise rehabilitation prescription guidelines for burn populations. At Texas Tech University I am a tenure-track Assistant Professor of Exercise Physiology and the director of the newly developed Exercise and Thermal Integrative Physiology Laboratory, in which we aim to study environmental factors and nutrition that affect cardiovascular health. 1. Contributions to Science Graduate Career ( ): My doctoral research occurred in two laboratories: 1) Exercise and Biochemistry Laboratory at Texas Woman s University focused on nutrition and exercise perturbations on metabolism in obese type 2 diabetic adults, and 2) The Institute for Exercise and Environmental Medicine, Thermal and Vascular Physiology Laboratory at UTSW focused on temperature regulation in healthy young and aged and burned adult survivors. 1. Nutrition, exercise and passive hyperthermia on metabolism. My research identified that omega-3 supplementation improved lipid profiles; however, did not contribute to post-exercise insulin sensitivity responses. We also reported that acute bouts of passive hyperthermia increased plasma leptin while acutely reducing arterial blood pressure in obese type 2 diabetics. This work highlights the importance of exercise and the novel use of hyperthermia therapy for improving metabolic and vascular function in obese and
3 diabetic populations. Lastly, we showed heat acclimation under uncompensible heat stress is mediated by primary alterations in metabolic heat production. a) Rivas E, Newmire DE, Ben-Ezra V. We reported that adult burn survivors have low aerobic exercise capacity that may increase the risk for cardiovascular disease. It is often recommended that burned individuals not exercise in the heat, due to damaged skin from burn and grafting. Our work showed similar abilities to tolerate heat stress and heat acclimation as non-burned individuals. Shock. 2016;46(3 Suppl 1): PMID: Postdoc Fellowship: : My post-doctoral training occurred in two laboratories: 1) The University of California Irvine (UCI), Galassetti laboratory focused on pediatric obesity and exercise, and 2) Shriners Hospitals for Children (SCH) and The University of Texas Medical Branch at Galveston (UTMB), Metabolism and Pediatric Exercise Physiology laboratory that focused on translational burns in children. 1. UCI: Exercise capacity in obese children. This training strengthened my understanding of obesity and pediatric exercise physiology. We found that peak exercise capacity test examined by allometric scaling and normalized oxygen uptake to kg of lean body suggest obesity reduced cardiorespiratory fitness independent of biological maturation (i.e., puberty) and sex. However, submaximal exercise is affected differently by puberty status in boys and girls. The following manuscripts are in preparation or currently under preparation: 1) Allometric scaling and normalization to lean body mass suggest obesity and metabolic syndrome are associated with reduced peak aerobic capacity in children, and 2) Central role of inflammation on cardiovascular disease progression in obesity, type 1 and type 2 diabetes from children to adults. a) Rivas E, Huynh H, Galassetti, PR Obesity affects relative peak oxygen uptake-heart rate relationship and exercise economy differently in pre and postpubescent boys and girls International Journal of Exercise Science 2. SHC & UTMB: Clinical translational research in burn children. This training provided an exceptional experience with randomized placebo-controlled trials that involved drug (propranolol and metformin) and exercise interactions in clinical populations, and understanding the pathophysiology of hypermetabolism from burn injury. We published several important findings from this work that may help advance exercise science and burn care in burn populations. 1) We found that propranolol (a drug that reduces hypermetabolism) did
4 not negatively affect skin blood flow response at rest in hot conditions or affect temperature regulation during exercise and therefore did not place children at greater risk for hyperthermia; 2) We are also the first to report that severely burn children have heart failure-like characteristics such as attenuated cardiac output and stroke volume during submaximal exercise and exercise intolerance; 3) For the first time, we found that glycemic control is improved after exercise training in burned children; and 4) Lastly, we reported that children with burn injury when compared to age-sex matched non-burned children, have attenuated strength and cardiorespiratory fitness 4 years post burn, suggesting a continued exercise rehabilitation may be needed years after burn injury. a) Rivas E, McEntire SJ, Herndon DN, Suman OE Resting β-adrenergic blockade does not alter exercise thermoregulation in children with burn injury: a randomized control trial.
