3 Healthy Step to Wellness Rewards Step 1 Health Risk Assessment (HRA) Start your journey to wellness by completing your on-line personal Health Risk Assessment (HRA) within 30 days of enrolling with NetCare Health Insurance at This educational tool is designed to identify health risk factors you can control and provide you with suggestions for making positive lifestyle changes. Once you have completed the HRA, a personal wellness report is generated which can be printed or read online. The evaluation includes a summary assessment of risk in several categories (Nutrition, Tobacco Use, Physical Activity, Alcohol Use, Stress and Depression, Current Health Status, Family Health History and Biometric Measures) and key suggestions. The report can be reviewed with your primary care provider. The report also showcases two important things: your healthy habits and your areas of highest risk. The rest of the report contains information and suggestions on how to strengthen those healthy behaviors and work to change and improve your unhealthy ones. All the information contained in the report is individualized bases on your response. 11 Know Your Numbers BLOOD PRESSURE Uncontrolled high blood pressure can lead to stroke by damaging and weakening your brain s blood vessels, causing them to narrow, rupture or leak. High blood pressure can also cause blood clots to form in the arteries leading to your brain, blocking blood flow and potentially causing a stroke. Ranges Classification 120/80 Normal 121/81 139/89 Pre-Hypertension 140/90 159/99 Stag. 1 Hypertension 160/100 or higher Stag. 2 Hypertension BLOOD GLUCOSE Severely elevated glucose levels can result in a medical emergency (diabetic coma). Over time, hyperglycemia can lead to damage to organs and tissues. Long-term hyperglycemia can impair the immune response, leading to poor healing of cuts and wounds. It can also cause never damage, vision problems, and damage to the blood vessels and kidneys.
