4 Silver&Fit Basic Network ASH Fitness offers fitness facilities the chance to attract a new stream of members by making access to physical activity easier for older adults. There are two ways to participate: Silver&Fit Basic This network allows eligible Medicare members (65 years of age and older or on disability) to obtain a no-cost or low-cost membership at participating fitness facilities. Members health plan coverage provides access to cardiovascular equipment, resistance and strength training equipment, and any standard classes or other amenities. No special programming is required. Plus, your facility will be listed on the award-winning SilverandFit.com website. 4 page

Class options include: Silver&Fit Signature Series Classes (Explore, Experience, Excel) Senior Dance Senior Yoga Tai Chi Aqua Fall Prevention Strength and Cardio Classes ASH Fitness compensates your fitness facility for Silver&Fit memberships on a monthly basis as long as your facility meets the necessary criteria for ASH Fitness. This includes submitting a monthly record of member visit dates. Priority listing in network directories (for facilities that offer at least two classes weekly) Reimbursement by ASH Fitness within 30 days of receipt of invoices Access to FitnessEngagement.com for downloadable promotional posters and fliers

FitnessCoach Network The FitnessCoach program allows Medicare/Medicaid eligible members to obtain a no-cost or low-cost membership at participating fitness facilities.

Through FitnessCoach, health plans provide dual-eligible members with access to all of the standard amenities and services of the fitness facility, including non-fee-based group exercise classes. Dual-eligible members may include the following: Members that qualify for both Medicare/Medicaid Members with disabilities or chronic conditions ASH Fitness compensates your fitness facility for FitnessCoach memberships on a monthly basis as long as your facility meets the necessary criteria for ASH Fitness. This includes submitting a monthly record of member visit dates. It is designed for health plans that are not yet ready to offer a full benefit program, but want to expose their members to fitness-based health improvement programs. ASH Fitness ChooseHealthy members are responsible for paying their own enrollment/ initiation fees and monthly dues to the fitness facility. Similar to ChooseHealthy, the program is designed for health plans or employer groups that are not yet ready to offer a full benefit program, but want to expose their members to fitness-based health improvement programs. ExerciseRewards members are responsible for paying their own enrollment/initiation fees and monthly dues to the fitness facility. Members receive reimbursement back directly from ASH Fitness for participation and/or utilization of a fitness facility. A minimum of a 10% discount off enrollment/initiation fees and/or monthly dues is required by the facility. (Nonprofit facilities please contact ASH Fitness for more information.) Fitness facilities are required to submit utilization to ASH Fitness for ASH Fitness to reward the members for participation and/or utilization of a fitness facility.
