5 Motivation Letter Dear colleagues: Cardiovascular medicine is currently at a stage of major transformations: The introduction of novel preventive interventions and medications which may halt or even reverse the progression of atherosclerosis The introduction of many subspecialities within the field of cardiovascular medicine, which reflect the dynamic development in heart failure, rhythmology, interventional therapy, and congenital heart disease to name just a few. As President of the EACPR I had the opportunity to experience the strength and vision of the ESC to lead our field into the future. During my presidency I launched the ESC textbook series on Preventive Cardiology and continue as editor of our European Journal of Preventive Cardiology. What motivates me to be a candidate for the office of Councillor of the ESC is to promote a patient-centered outcome-oriented perspective on cardiovascular medicine. The rapid sequence of technical and pharmaceutical innovations in cardiology call for a professional industry-independent and credible organization like the ESC to prioritize novel treatments based on prognostic benefits and cost-benefit ratio. Additionally, I believe that prevention is better than cure. Preventive cardiology needs to become an essential part of our clinical work rather than be delegated to rehabilitation institutions. With our clear lifestyle recommendations and the novel drugs on the horizon we are now closer than ever to drastically reduce the burden of atherosclerotic disease in Europe and beyond. I see the ESC as a leader in prevention and we should work towards this goal together with WHF and WHO. Digital technology will be key to leverage and promote prevention to professionals and patients, however, health apps should not only be industry-driven. We as professionals need to have our own agenda and priorities for the digital age and influence the developments in this fast-growing area through partnership and certification. Finally, despite all our subspeciality skills I would like us to preserve the unity of cardiovascular medicine. Subspeciality certifications are necessary but should be anchored around a core cardiovascular curriculum. Patients deserve disease- and outcome-focused treatment recommendations which require a broad general knowledge. The young cardiologists of tomorrow are our door to the next generation of cardiologists. The ESC has a unique opportunity to guide these young talents to become excellent general cardiologists before they focus on a subspeciality area. I envision the ESC as the international postgraduate cardiology school for the next generation of cardiologists and will work to harmonize and coordinate the multiple initiatives for young cardiologists within the ESC. Thank you for supporting my candidacy for the office of Councillor of the ESC. With best regards, Stephan Gielen, MD, FESC Apl.
