We are pleased to inform you that your association has recently launched an annual recognition awards for our member hospitals in an endeavor to recognize excellence amongst hospitals. The PHA Awards Committee is composed of the following credible personalities to further ensure the integrity of this award-giving body: Honorary Chairman: PHA President Dr. Huberto F. Dr. Leo Olarte PHA, Dr. Rustico Jimenez Former PHA Executive, Mac Caliwara PHA Researcher Ryan Jay Cua Advisers: PHA President Emeritus Dr. Ruben Flores ASec Dr. Elmer Punzalan We have worked rigorous hours and brainstormed to determine the various categories of awards and come up with the formal mechanics for awards and recognition. We have likewise sat down with Council and Chapter Presidents to appeal for their full support to this latest PHA undertaking. There are three coveted recognitions that shall be awarded during the Fellowship Night of the PHA 69th Annual National Convention in November this year, namely: the Most Outstanding Hospital Award, the Most Outstanding Chapter, and the Most Outstanding Council Award. You may refer to the attachment for the Entry Mechanics and other contest details. Deserving members shall be selected via a strict screening on the basis of excellence in the fields of service, training, research, and governance, which shall be on a per chapter/per region basis. PHA hospital members from both the government and private sectors are invited to participate and submit their entries. Nos: (02) /75; Fax Nos: / Submission of Entries - PHA Member in Good Standing - No pending cases - Submit entry to PHA Chapter PHA Chapter Chapter Nominees - Checking of requirements - Chapter confirms information PHA Regional Council Regional Finalists - Council decides finalists - Council presidents, as a panel, decides on merits of nominees Final Selection ENTRY MECHANICS Interested institution submits completed entry form with supporting documents and/or pictures (to verify existing programs) to their chapter of membership Signed and attested to truth of submitted entry The local chapters determines eligibility, and submits vetted entries to their regional council of membership The regional council determines the top 2 nominees, and submits vetted entries of their nominees to the Awards committee for final tallying. The Awards Committee collects the vetted entries of the regional council nominees. The nominees points will be tallied. All the nominees will be credited and recognized, and the top 2 finalists per category (government/private, for infirmary/level 1/level2/level 3) shall be awarded Most Outstanding in their categories.

3 POINTS SYSTEM The PHA Annual Recognition Awards will be based on a system that counts merits of the institution as a whole. The results will be tallied and deliberated upon. This is to ensure that the recipients will be truly deserving of such awards, and will be recognized for their commitment to excellence in healthcare. For the Most Outstanding Hospital Award, this includes (but is not limited to) as follows: Corporate Social Responsibility (private) / Community Work (government) Excellence in Governance Accreditations o ISO o QMS o Awards/Distinctions received Infection control Disaster Preparedness Green Hospital Initiative Social/Sports Development Program Regular staff meetings (Medical and non-medical) Safety and Patient Care Continuity of care / Functional 2-way referral system Patient engagement and education Transparency in reporting of sentinel events/adverse reactions For the Most Outstanding Chapter Award, this includes (but is not limited to) as follows: Complete Roster of Officers Functional: o Regular meetings with other hospitals within the chapter o Joint activities with other hospitals within the chapter o Dissemination of information to members of the chapter o Relaying feedback from the members of the chapter Participation: o Annual National Convention of the previous year o Regional Conference of the previous year o National Hospital Week o Infection Control Program o Disaster Preparedness Program Membership and Recruitment of the previous year Participation in PHA-affiliated International Activities of the previous year For the Most Outstanding Council Award, this includes (but is not limited to) as follows: Complete Roster of Officers Functional: o Regular meetings with other hospitals within the council o Joint activities with other hospitals within the council o Dissemination of information to members of the council o Relaying feedback from the members of the c council Participation: o Annual National Convention of the previous year o Regional Conference of the previous year o National Hospital Week o Infection Control Program o Disaster Preparedness Program Membership and Recruitment of the previous year Participation in PHA-affiliated International Activities of the previous year
