4 About Us Hospital is a community hospital in, Massachusetts and a member of the Steward Health Care System, a community-based accountable care organization and community hospital network with more than 17,000 employees serving more than one million patients annually in more than 150 communities in Massachusetts, New Hampshire and Rhode Island. Hospital is a 215-bed acute care community hospital with state-of-the-art medical technology and a highly skilled staff. The hospital s major clinical services include advanced surgical services, obstetrics, cardiology, neurology, orthopedics, gastroenterology, psychiatry, cancer care and pediatrics. As the only acute care hospital in, Hospital operates the principal emergency department for the town of 30,602. Hospital () also serves the Massachusetts towns of Canton, Dedham, Foxboro, Franklin, Mansfield, Medfield, Millis, Norfolk, North Attleboro, Norton,, Plainville, Sharon, Walpole, Westwood, and Wrentham. 5 Community Benefits Mission Statement Hospital is committed to collaborating with community partners to improve the health status of community residents. We accomplish this by: Addressing root causes of health conditions and disparities Educating community members around prevention and self care, particularly for chronic diseases such as diabetes Addressing the social determinants of health including education and access to healthrelated resources Community Benefits Statement of Purpose Hospital acts in compliance with the Massachusetts Attorney General s Guidelines for Non-Profit Hospitals as promised to our community and government at the time of our transition to Steward Health Care System, LLC in Our community benefits purpose is to: Improve the overall health status of people in our community Provide accessible, high quality care and services to all those in our community, regardless of their ability to pay Collaborate with staff, providers and community representatives to deliver meaningful programs that address statewide health priorities and local health issues Identify and prioritize unmet needs and select those that can most effectively be addressed with available resources Contribute to the well-being of our community through outreach efforts including, but not limited to, reducing barriers to accessing health care, preventative health education, screening, wellness programs, and community-building Regularly evaluate our community benefits program 4

6 Needs Assessment Hospital conducted a community health needs assessment in December 2015 with the goal of identifying the most imminent concerns in the hospital primary service area and the areas where the hospital could have the greatest impact in improving population health. The Community Health Needs Assessment results are based on data from primary and secondary sources, including: Survey of community health and human services providers, including hospital staff Local community health and human services providers and resident focus group Publicly available databases, including the U.S. Census Bureau and the Massachusetts Community Health Information Profile (MassCHIP) Internal health system informatics Meetings with hospital leadership to assess the needs of the hospital in terms of quality, patient experience, and costs Areas of concern were identified by two or more of the following criteria: Rate higher than the state average Rate increasing over time Identified as concerns by focus group participants, community provider survey respondents, or the Community Benefits Advisory Committee 5

Circulatory disease, cancer, and respiratory disease are the top three causes of death statewide and in our Primary Service Area. Input from survey respondents and focus group participants also reflected concern for chronic disease as a serious community health issue and remarked that it is significant and should not be overshadowed by the substance abuse problem in the area. Community members voiced the need for increased education and preventative interventions. 8 Chronic Disease Tactics / Short-Term Goals: Increase education on chronic disease prevention in community spaces. Utilize small media to inform and remind patients to get screened for chronic disease. Share information on chronic disease support groups that would be most effective for the population. 10 November Canton, Adults RE: Exercise, Blood Pressure/Cardio Meditech Employee Wellness Fair Priority 2 Obesity There are higher proportions of overweight or obese children and young adults in the communities surrounding Hospital than within the state population. Approximately 15-40% of children in Grades 1, 4, 7, 10 in the hospital s service area are overweight or obese, with higher concentrations of obese children in Attleboro, Canton, Dedham and. Obesity contributes to the incidence of circulatory disease, diabetes and other chronic ailments. Accordingly, community input mentioned obesity as a top 5 community health concern. Interventions cited to address obesity were nutrition education programs, exercise programs, and programs promoting lifestyle changes. 11 Attleboro Canton Dedham Foxborough Franklin Sharon Walpole Westwood Wrentham Massachusetts Total Obesity increases the risk of a number of health conditions including hypertension, adverse lipid concentrations, type 2 diabetes and bone and joint disease. According to stanfordhealthcare.org, obesity s far-ranging negative impact on healthcare costs over 150 billion dollars annually and causes an estimated 300,000 premature deaths in the United States. Chef s Table Nutrition Demos October Canton All/Families Re: Exercise, Nutrition Reebok Canton Road Race 5k Road Race Priority 3 Behavioral Health The behavioral health problem is evident to both community service providers and residents, with some towns in the Primary Service Area having hospitalization rates higher than the state average. A contributing factor to the behavioral health problem is mental health stigma. Mental health stigma has been identified as one of the major obstacles to accessing behavioral health resources. Suggestions to combat this problem included education regarding mental health issues and more accessible behavioral health resources for those with behavioral health concerns as well as their friends and families. Behavioral Health Tactics/Short-Term Goals: Provide education to frontline caregivers, community leaders, and residents on the signs of mental illness and how to access necessary resources. Share information on behavioral health support groups that would be most effective for the population. Support an environment of open dialog and acceptance of behavioral health issues to destigmatize the affliction of mental illness Behavioral Health Tactics/Long-Term Goal: Educate community on available treatment resources and support groups, with special emphasis on resources for youth. 13

16 Priority 4 - Access to Health Care Health insurance coverage for most of the Hospital service area is relatively good with uninsured rates below the state average, with the exception of Attleboro showing an uninsured rate of almost 5%. When stratifying the population by sex, a larger proportion of males are uninsured when compared to females. 17 Survey respondents cited financial concerns, language barriers, transportation, lack of primary care physicians, and the inability to get time off of work as the top 5 obstacles to accessing healthcare in the community. When the question of what populations were most underserved in regards to healthcare was posed to survey participants, they cited the mentally ill, low income, elderly, uninsured and disabled populations as the most in need of healthcare services. Access to Health Care Tactics / Short-Term Goals: Ease access to primary care physicians to encourage wellness and preventative healthcare. Provide information and assistance to patients on enrolling in insurance or seeking financial assistance. Increase community health education efforts utilizing multiple media channels. Access to Health Care Tactics / Long-Term Goal: Provide information on transportation resources for elders and others seeking treatment for chronic illness. Such conditions include family disruptions, financial problems, lost productivity, failure in school, domestic violence, child abuse, and crime. Moreover, both social attitudes and legal responses to the consumption of alcohol and illicit drugs make substance abuse one of the most complex public health issues. The data reflect an increase in alcohol or substance-related hospitalizations for about half of the towns from 2011 to 2012, including Dedham, Foxboro, Franklin, Westwood, and Wrentham. Clinicians and community stakeholders have seen significant increases in hospitalizations, injuries and overdoses since 2012, indicating that the problem is growing rapidly. 17

Focus group participants described the problem as epidemic and cited lack of services, easy access to e-cigarettes and marijuana, mental health issues and the normalization of drug use as contributive causes of the substance abuse problem. Participants suggested increased substance abuse education at an earlier age (late elementary and middle school) would be beneficial to combat the growing substance abuse problem.
