11 Why Develop a Model?
12 One year ago I completed treatment for breast cancer. When I left the hospital the day of my last dose of radiation, my sister rang to congratulate me on having gotten through what was an horrific nine months She asked if I was going to celebrate and I burst into tears. Just when I expected to feel ecstatic, I felt lost and confused. Did I have cancer that morning when they strapped me into the radiation machine, and that afternoon I didn t? I d been a cancer patient for a year, who was I now?
13 I was more afraid at that moment than any other moment since my diagnosis During treatment I was so focused on fighting the disease. Surrounded by those whose job it was to help take on the cancer. I was too busy being sick to think about dying. Having leaned heavily on my family and friends throughout the previous year, I was also self conscious about sharing this new crisis with them how could I tell them the mental anguish had only just begun.

And how wide is the scope? 3.Who makes up the survivorship workforce? And should there be survivorship standards and competencies? 4.What does the transition process look like, throughout the cancer care continuum?
18 The workshop highlighted several key themes: Survivorship Care planning from diagnosis with the patient at the centre Coordinated services during and post treatment Workforce requirements to support survivorship Equity of access and culturally appropriate support
