1 Calhoun County Task Force Meeting Summary The Calhoun County Task Force met on Thursday, March 26, 2015 from 1 p.m.-2:10 p.m. at Stewart Memorial Community Hospital in Lake City. Attendance: Jeanette Sargent-SMCH, Shirley Naughton-Parent, Pam Haberl- Parent, Emilee McNace- Counseling Services, Dave Staver-Opportunity Living Executive Director, Heather Cain-SMCH CEO, Cindy Carstens- SMCH, Leisa Mayer-RHCS Coordinator, Deb Gimer-CCPH, Karla Manternach-Counseling Services, Dawn Mentzer-RHCS CEO and Patrick Schmitz-PAMH CEO 1. Update from Counseling Services-Karla Manternach and Emilee McNace: Continuing to see individuals one day a week in the Rockwell City office. They placed an ad in the Tri-County paper and will also do so in The Reminder so they cover the entire Calhoun County area. Presentations about Counseling Services and general mental health information, signs and symptoms of mental illness and emergency services were presented to Opportunity Living staff and the Rotary in Rockwell City. They also did a mailing to police departments and emergency services. SMCH staff reported a glitch in using the emergency after hours phone number; they were never able to reach anyone at Foundation II. Karla is looking into this issue. Emilee reported most referrals are coming from Calhoun County Public Health. 2. Update regarding psychiatric services-cindy Carstens, SMCH and Patrick Schmitz, PAMHC: Once the lease agreement between PAMH and SMCH is completed by SMCH (hopefully by April 1 st ) it will be forwarded to CMS to approve credentialing/services. How long this will take once it is in CMS s hand is very uncertain. There is a very tentative start date for David Getner, ARNP to start providing face-to-face services at SMCH on April 23 rd. 3. Mental Health Services-are we where we want to be, any gaps or unmet needs/issues: Deb Gimer, CCPH shared the lack of children s services with a need for psychiatric, in-home, play therapy and trauma informed care for children from toddlers on up. Dave Staver, Opportunity Living shared his concern for lack of services for individuals with disabilities other than mental illness and the need for transitional services for children to adults and concern with schools not being able to meet the needs in the school setting. Discussion surrounded the need for what AEA s and IVRS can offer. Discussion also included the current funds available in the region, Dawn shared current financial information, how the coming managed care organization may affect regional finances, crisis services in the region and funding of these services, the current inability to bill Magellan for crisis beds/stabilization type services and that the current plan is for the region to fund these

2 costs and the regions being directed to follow SF2315 rules regarding establishing/funding of core services and core plus services. Dawn briefly shared information regarding the 2 proposals submitted for Crisis Stabilization Services in the community and Transitional Living Services in the community. Other proposals pending for the governance board to rule on are Community Employment proposals. These will all be discussed and voted on at the next Regional Governance Board meeting on April 15 th. Dawn shared the current Family to Family Support Group that is being held at Calhoun County Public Health and facilitated by 2 of their staff. The staff are trained NAMI family members. It is a free 12 week course for family and friends of individuals with serious mental health illness. Next Meeting? The next meeting is scheduled for June 2 nd at 10:30 a.m. at Stewart Memorial Community Hospital in the Private Dining Room.

3 Calhoun County Task Force Meeting Summary The Calhoun County Task Force met on Thursday, October 30, 2014 from 1:00p.m.-2:30p.m. at Opportunity Living in Lake City. 4. Welcome/Introductions: Those present were Pam Haberl, Family Representative/Governance Board Member, Heather Cain-CEO Stewart Memorial Community Hospital, Cindy Carstens, Stewart Memorial Community Hospital, Tracey Toms, Opportunity Living Director of Health Services, Karla Manternach, Counseling Services, LLC, Leisa Mayer, Rolling Hills Coordinator of Disability Services, Dave Staver, Opportunity Living Executive Director, Jane Condon, Administrator for Calhoun County Public Health and Patrick Schmitz-CEO for Plains Area Mental Health 5. Update from Counseling Services, LLC - Karla Emille McNace from Counseling Services, started providing outpatient mental health services in the Rockwell City satellite office at Calhoun County Public Health offices on October. They will consider adding additional days as needed. The after hours on call number for Counseling Services will be changing on October 31 st and connect people to the Foundation 2 emergency number just as Plains Area Mental Health also will. Foundation 2 will take the initial call and if a more local contact is necessary they have the information of who is on call for Plains Area and Counseling Services and will contact them. This number can be used by the public and providers of emergency services. Karla scheduled with Opportunity Living and Stewart Memorial for inservice training. 6. Update regarding Telepsych Cindy Carstens /Patrick Schmitz Cindy shared that she and Patrick Schmitz have been communicating re: setting up telepsych services at the Stewart Memorial/McCrary Rost location for the public and for the residents of Opportunity Living. Patrick says they have telehealth equipment available for the hospital to use. After this meeting Patrick, Cindy and Heather will go to the hospital and look at the logistics to get this set up. Patrick said that their current ARNP, Chrystal Reinhart, will be available for face to face services and they are looking to recruit for the telepsych services. There is not an exact date but will hope to start as soon after January 1, 2015 as possible.

4 7. Community Health Needs Assessment & Health Improvement Plan Unmet Needs Grant-Jane Condon Jane provided information on the above named grant and asked for input from the group about mental health needs for Calhoun County. The group shared ideas about possible upgrade needs for telepsych equipment, transportation needs for psychiatric appointments, staff time and salaries for the Family and Peer Support groups, staff time for hospital staff to receive in service information on mental health issues/services and Mental Health First Aid classes in the community. The group agreed on pursuing this grant and Jane agreed to write it. Jane will contact the participants around the table for information she needs to complete the grant and will keep the group up to date on what she learns. Thank you! Next Meeting? The next meeting was scheduled is scheduled for 12/4/14 at 1:00 p.m. at Stewart Memorial Hospital. Cindy will let us know what specific location will be.

5 Calhoun County Task Force Meeting Summary The Calhoun County Task Force met on Thursday, September 25, 2014 from 1:00p.m.-2:00p.m. at Opportunity Living in Lake City. 8. Welcome/Introductions: Those present were Pam Haberl, Family Representative/Governance Board Member, Shirley Naughton, Family Representative, Cindy Carstens, Stewart Memorial Community Hospital, Tracey Toms, Opportunity Living Director of Health Services, Karla Manternach, Counseling Services, LLC, Leisa Mayer, Rolling Hills Coordinator of Disability Services, Dave Staver, Opportunity Living Executive Director and Dawn Mentzer, CEO Rolling Hills Community Services Region 9. Update from Counseling Services, LLC - Karla Counseling Services, LLC has entered into an agreement with Calhoun County Public Health to rent office space in their building in Rockwell City. Karla informed the group that Emilee McNace will be providing therapy services every Thursday initially and as the need increases they will consider adding additional days. They will be starting to provide services in October. Karla explained how the after-hours crisis services will be provided by Foundation II which is a call center based out of Cedar Rapids. Staff from Stewart Memorial will be able to contact Counseling Services during business hours for commitment assistance/pre-screening services and then will be able to utilize the 800 number for Foundation II after hours and on the weekends. Dave wondered whether Counseling Services, LLC would be available to provide presentations to their staff for in-service training as needed and Karla was willing to do so. 10. Review of progress toward next steps from 8/28/14 meeting -Dawn will contact New Hope Village to inquire as to whether they have a need to share this resource if developed with Calhoun County. Update: Dawn received information from Dottie Dorman, Allied Health Services Director at New Hope who indicated that they have adequate psychiatric coverage. Kathy Richardson, ARNP from St. Anthony s Mental Health currently comes to the New Hope Campus twice monthly to evaluate/treat their clients. Kathy Richardson, ARNP and Dr. Philip Muller from St. Anthony s Outpatient Mental Health Clinic also treat and serve HCBS clients from the community on an outpatient level. New Hope also has HCBS clients who choose to see Sue Gehling, ARNP at Family and Specialty Medical Center. Currently they have no clients going to Plain s Area but that is also an option for them. -Dave will reach out to the hospital in order to engage them as active partners in the task force and planning process.

6 Update: Hospital was represented by Cindy Carstens and they will continue to be an active partner on the task force. SMCH may be the venue for Telehealth services. -Dawn will contact Patrick Schmitz to discuss options for Calhoun County and what resources would be needed and how the region could assist. Update: Dawn contacted Patrick Schmitz from PAMHC to inquire about meeting the psychiatric needs in Calhoun County. Patrick believes that this is a possibility through Telehealth with Chrystal Reinhart, ARNP. They are not able to provide it through Counseling Services, LLC since their liability insurance will not cover a subcontract for this service. Discussed best location for the equipment Opportunity Living or Stewart Memorial Community Hospital? All were in agreement that the hospital would be the best location for the general public as well as the clients of Opportunity Living. Cindy Carstens read an from the Berryhill Center indicating their willingness to also consider providing Telehealth at the hospital. The consensus was to follow-up on a firm commitment from the region s contracted provider, PAMHC before responding to the Berryhill Center. Dawn was concerned that there may be a funding request from the Berryhill Center and that we are already block granting a significant amount to PAMHC and do not want to duplicate. Dawn will further discuss this with Patrick. Dawn reported that Tony Larsen, Licensed Psychologist with PAMHC may be interested in providing services for the residents of Opportunity Living as Patrick reported that he is already traveling close to that area. He was going to check with Tony and report back. Other Needs/next steps: *Dawn will follow-up with Patrick Schmitz to discuss a firm commitment from PAMHC to provide Telepsych Services in Calhoun County. *Dawn will provide contact information to Patrick for Cindy Carstens. Cindy would like Patrick to attend a Med Staff meeting in the future to share information about PAMHC and the new services starting in Calhoun County. *If PAMHC is capable of providing the Telehealth Services, SMCH can respond to the Berryhill s request. *Dawn reminded the group of the upcoming Regional Advisory Committee meeting that is scheduled for October 23, 2014 at 3:00p.m. Dave Staver will be providing an update to the Committee on the progress that has been made to date in Calhoun County. All in attendance were pleased with the progress so far in a short amount of time. Next Meeting? The next meeting was scheduled for 10/30/14 at 1:00 p.m. at Opportunity Living.

7 Calhoun County Task Force Meeting Summary The first meeting of the Calhoun County Task Force was held on Wednesday, August 27, 2014 from 10:00 a.m.-12:00 p.m. at Opportunity Living in Lake City. 11. Welcome/Introductions: Those present were Scott Jacobs, Calhoun County Supervisor/Governance Board member, Pam Haberl, Family Representative/Governance Board Member, Karla Manternach, Counseling Services, LLC, Leisa Mayer, Rolling Hills Coordinator of Disability Services, Dave Staver, Opportunity Living Executive Director and Dawn Mentzer, CEO Rolling Hills Community Services Region 12. Identify Current Services and Strengths of Calhoun County What is available? Dave Staver provided information about their agency and what they have to offer to the individuals that they serve. Opportunity Living serves 68 ICF/ID individuals from 27 counties with one being served from Nebraska. They are currently serving one HCBS Adult and one HCBS child. Seventeen are on the waiting list for adult placement. They have on-site physician accessibility due to contracting with Stewart Memorial for Dr. Frate s time. Opportunity Living collaborates on a regular basis with the hospital and they have an established working partnership. 13. Identify Service Needs/Gaps Opportunity Living would like to expand services into HCBS however, the issues that prevent them from doing so include lack of transportation in the community, lack of services in Calhoun County which causes families hesitation should the ICF bed not be available if there would be a need and the need for additional employees in a rural setting. Dave identified the largest need for Calhoun County is the ability for individuals to access local psychiatric care. They have 23 individuals that are accessing psychiatric coverage at the Berryhill Center but have a need for at least 40. The Berryhill can not take any more of their individuals therefore, the remainder either have their medications monitored by an MD or they transport to Iowa City. The trips to Iowa City are difficult for the people that they serve due to distance and the amount of time that it takes. At one time, the Berryhill Center worked out an arrangement with Stewart Memorial Hospital for Telepsych but had to pull the equipment prior to service implementation when their psychiatrist became incapacitated, needed to work from home and required the equipment. Dave stated that their facility has 30 days to have an assessment completed upon admission and that as a best practice, it should be completed faceto-face and that telepsychiatry is not always effective with the clientele that they serve. Dave has requested that St. Anthony s from Carroll assist with psychiatric coverage but they feel that

8 their clientele do not meet the criteria for the services that they provide. Also discussed was the lack of psychiatric coverage for rural areas and the difficulty in attracting such specialty services when the need is just as great in urban areas that have more to offer. Outpatient Services were previously identified as a need in Calhoun County through the full Advisory Board. Karla provided an update regarding their plans to open an office in Rockwell City so that they have a physical presence in Calhoun. They hope to be up and running by the end of September and will be in Rockwell City one day per week. They are looking at renting space in the Public Health Department. Karla indicated the Stewart Memorial could contact Counseling Services for assistance when they have someone brought into their ER to determine whether they need admission to an inpatient setting or if they could be screened through their agency and scheduled for outpatient services. Other Identified needs: Mental Health First Aid Training for first responders / Dave also requested that ISAC partners with the IACP in lobbying for funding to re-open the office of the substitute decision maker. He is involved on the statewide task force and is advocating for the office to be funded by the legislature to fill an important need for individuals with intellectual disabilities as well as the elderly since Iowa has an ever-increasing elderly population. 14. Vision for one year from now? Psychiatric coverage in Calhoun County 3-4 days per month through a combination of face-toface psychiatry and telepsych. 15. Organization of Task Force future meetings/locations Dawn requested that Dave consider being the liaison between the task force and the Advisory Board and he was in agreement. Next steps: -Dawn will contact New Hope Village to inquire as to whether they have a need to share this resource if developed with Calhoun County. -Dave will reach out to the hospital in order to engage them as active partners in the task force and planning process. -Dawn will contact Patrick Schmitz to discuss options for Calhoun County and what resources would be needed and how the region could assist.
