Agenda In Sandy Milakovic s absence, Michelle Kelly chaired the meeting. Michelle called for and received approval of the agenda. 2. Previous Meeting Notes November 3, 2010 The group agreed that the notes of November 3, 2010 reflected the meeting discussions. Suzanne Robinson will circulate the notes and request they be posted to the LHIN s web site. 2.1 Business Arising In follow up to the September meeting, Suzanne Robinson circulated hospitalization data and a research article related to people with dual diagnosis from Trevor Lumb, Central West Network of Specialized Care. 3. Business 4.1 Vice-chair Suzanne Robinson informed that group that Scott Brooker was appointed Vice-chair by the Central West LHIN. 4.2 Ministry-LHIN Performance Agreement (MLPA) Indicators for MH&A Laurie Wolfson shared that while William Osler Health System s committee on data quality has convened only once, it is still the intent of Osler to assess the value of including community providers. Suzanne Robinson summarized discussions to date about the new indicators and circulated supplementary data as provided by MOHLTC. Jasdeep Sahota, the Central West LHIN staff responsible for the Stocktake report received input from providers to take forward to her cross-lhin colleagues. Supplementary information from the MOHLTC further clarified the definition of repeat visits to which providers did not express concern. From the perspective of community providers, the following requires further discussion and investigation: capacity to fast track clients from the Emergency Room to community programs, assess wait times to access services, review discharge planning practices, practice of and capacity to conduct facilitated referrals, clarify our shared understanding of client consent and personal health information sharing. Suzanne Robinson to gather and share information on the coordinated access model for housing in Toronto and report back to the Group 4.3 LHIN Collaborative Committee re: Care Transitions in Mental Health and Addiction Suzanne Robinson provided background on the committee. The Group briefly discussed ways in which it could provide to the Central West LHIN information about and samples of tools in use and processes in place that improve clients movement from one service to another. Suzanne Robinson to contact Lesley Nagoda of Supportive Housing In Peel regarding central intake, gather information on the resource matching and referral project, and discuss with Karen Parsons the transition within the addictions sector from counseling to case management 4.4 Update on New Initiatives Suzanne Robinson reported that the LHIN is proceeding with Supportive Housing In Peel (SHIP) and Peel Addiction Assessment and Referral Centre to plan for implementation of the Supportive Housing for Problematic Substance Use. MOHLTC letters have been received and the Central West LHIN is working through the process with SHIP. The priority client is one who has addiction, based on inventory the housing will likely be in Brampton, referrals are expected from withdrawal management programs, the Street Outreach team, and waiting lists, and managed uptake of clients is still to be discussed. The launch of this program is anticipated in this fiscal year. Total funding for support services is $168,000 and $111,400 for rent supplement. MOHLTC guidelines will be the basis for program policies. Suzanne reported that the Health Canada agreement with MOHLTC for the Youth Addiction Outreach Services two-year project is working its way through the authorization process at MOHLTC. The Central West LHIN continues to work with the Canadian Mental health Association/Peel Branch (CMHA/Peel) to plan implementation. As part of a six-lhin 2

3 project, Central West LHIN and CMHA/Peel will be part of a collaborative process to monitor performance targets and participate in a formal evaluation. 4.5 Psychiatric Sessionals Suzanne Robinson was pleased to share that the planned allocations submitted to MOHLTC were approved. While we await MOHLTC letters, the Central West LHIN is readying to issue letters to providers so as to provide access to the sessionals as soon as possible. Suzanne thanked the Group for their patience, citing that the inclusion of community health centres required more review at the MOHLTC level than anticipated. Conveying providers endorsement of and commitment to the plan as submitted seemed to have a positive impact. An outstanding matter is the need to plan for the allocation agreed to be held by William Osler Health System. Suzanne Robinson and Laurie Wolfson will follow up with Dr. Koczerginski on the discussions at the August meeting regarding coordinated physician recruitment 4.6 CAG Feedback Summary Suzanne Robinson reviewed the slide deck as distributed. Suzanne to circulate a summary of H-SIPs on file to prepare for a discussion at a future Core Action Group meeting 5. Project Updates 5.1 Community Care Information Management CCIM a. Ontario Common Assessment of Need (OCAN) Ashim reported that LHIN staff is meeting this afternoon and from that, an implementation approach and schedule will be decided. Communication to providers will follow shortly. b. Integrated Assessment Record (IAR) Community Care Information Management (CCIM) presented to the Chief Executive Officerlevel group known as GTA Connects and is awaiting additional information. They will decide the roll out of the IAR and while there is no schedule as yet, the Chief Information Officer (CIO) of the Central West LHIN is advocating for early participation. c. Business Management Solutions No update required. 5.2 MIS Workshop As per the handout, Suzanne Robinson provided an update on the series of workshops to be hosted in order to increase consistency in reporting and therefore improve data quality for planning. The committee agreed that it was important to include a session on data analysis so that is now the fourth of four workshops being planned. 4 6. Standing Reports 6.1 Consumer Survivor Network Michelle Kelly just issued their most recent newsletter, has scheduled January regional meetings in Brampton, Caledon, Orangeville, and Rexdale. Michelle is awaiting replies to the seeking response to the regional priorities report that was first shared at the August meeting. There is an upcoming Provincial Consumer/Survivor Network LHIN Leads meeting that Michelle Kelly will report on. Michelle Kelly will re-issue the about the Regional Priorities Report and encourage timely feedback 6.2 Diversity and Equity Baldev Mutta reported that at its last meeting the Core Action Group participated in a webinar on the Health Equity Impact Assessment. The Group s co-chair and LHIN staff are presenting this at the next Child and Maternal Core Action Group. An outcome of their January meeting will be a report of their progress in 2010 and the Core Action Group s plans for Concurrent Disorder Network Mohamed Badsha reported that the GAIN Short Screener, uploading of screening data to the Centre for Addiction and Mental Health, and discussions about the electronic solution are underway. To date, the specialized short-term case management staff has served clients. Meetings are planned with Network partners to educate their staff about the service and how to make referrals. Laurie Wolfson added that the staff presentation at Osler s Etobicoke campus was well received. Laurie Wolfson added that in this fiscal year, a Phase I eight-week, psycho-educational closed group already operating in Brampton will be replicated at the Etobicoke campus. As well, a Phase II group will be started in Brampton. The groups are co-facilitated by mental health and addiction staff. 6.4 Dufferin Connects Eric Tripp-McKay reported that a new protocol for transporting clients discharged from William Osler Health System back to Orangeville is being pilot tested. With the initial capacity to serve 20 clients, Dufferin Connects allocated Dufferin County crisis funding to this initiative. Trellis staff is scheduled to present to William Osler Health System (Osler) about making referrals and Shelley O Grady added that the Transitional Care Worker at Headwaters will make referrals at the time of patient transfer to Osler. Laurie Wolfson reported that Headwaters, Osler, and Trellis met to discuss the function of the transitional care worker based at Headwaters. They want to meet with Dufferin Connects to ensure integration of multiple health service provider crisis workers to optimize services to residents. Laurie Wolfson will follow up with representative of Dufferin Connects 4

5 7. Other Business Susan Smither announced that she is transferring to another LHIN jurisdiction. With the change CAMH is dedicating the new consultant to Central West LHIN only. Susan previously worked in Toronto Central and Central West LHIN. Susan Smither to contact Suzanne Robinson to discuss the transition and the opportunity it presents for the Central West LHIN 8. Next Meeting The next Mental Health and Addictions Core Action Group meeting will occur on Wednesday, January 5 th, 2011 from 9 a.m. to 11 a.m. at the Central West LHIN. 9.
