4 UPDATES AND RESOURCES Hurricane Preparedness Week is May George Foundation, RebuildTX, Episcopal Health Foundation, United Way, CDP s Hurricane Harvey Recovery Fund and others are meeting in Fort Bend/Wharton County Funders are also collaborating on a meeting for long-term recovery groups to be held in Galveston in June FEMA s IMAT are prepping for the upcoming hurricane season

7 Trauma and Grief Center at Texas Children s Hospital Primary Objective: Develop, implement, evaluate, and disseminate evidencebased treatments for traumatized and/or grieving youth Goals of the TAG Center: Trauma-Informed Services: Provide evidence-based assessment and treatment (free of charge) to youth ages 7 to 17 who have experienced traumas or losses. Training: Provide training to community- and school-based clinicians in trauma-informed best practices. Research: Identify risk/protective factors that we can use to build better, more effective interventions.

8 What is Trauma? A trauma is an exceptional experience in which powerful and dangerous events overwhelm the person s capacity to cope. - Child Witness to Violence Project Acute Trauma car accident natural disaster bereavement terrorist act Chronic Trauma domestic violence child abuse and neglect poverty community violence racial discrimination Posttraumatic stress symptoms can arise in response to trauma, and are more likely to arise in the context of multiple traumas.

9 Recognizing Signs of Traumatic Stress in Kids Can look like anxiety Fear and worry about the safety of self, family, friends Can look like ADHD Increased activity level, decreased concentration Can look like depression Increased irritability, changes in sleep Can look like conduct problems Aggression, angry outbursts Can look like physical illness Aches and pains, doctor s visits

10 Longer-Term Consequences of Unaddressed Trauma Smaller brain volume Depression Suicide risk School problems Problems with peer relations Substance/drug abuse Violent behavior Delinquent behavior Intergenerational transmission of trauma/traumatic stress

11 What is bereavement/grief? Bereavement = the experience of deprivation or loss by death Grief = the psychological or behavioral response arising from bereavement

12 Why Focus on Bereavement? Most frequently reported type of trauma in clinic-referred youth (Pynoos et al., 2014). Most common form of trauma worldwide (UNICEF, 2013). Most distressing form of trauma among adults and youth in the general population (Breslau et al., 2004; Kaplow, Saunders, Angold, & Costello, 2010). Strongest predictor of poor school outcomes above and beyond any other form of trauma (Oosterhoff, Kaplow, & Layne, in press).

13 How Do Children Typically React to Hurricanes and Their Aftermath? Majority of youth treated for PTSD post-katrina had experienced prior traumas and losses (Jaycox et al., 2010). 70% had experienced the prior death of a loved one. MMHPI estimates that, at 18 months post-harvey, rates of PTSD could be 5 x higher than pre-hurricane rates of PTSD.

15 Goals of HRRP Dissemination of Resources Resources regarding children s mental health needs Screening tools to conduct situation analysis/needs assessment Intervention and Access to Mental Health Care Provide ongoing in-house assessment and intervention free of charge Deploy trauma-informed clinicians through mobile units and TCP outpatient clinics Provide transportation (in partnership with Lyft) and free parking Community-Based Training of Practitioners Provide trauma- and bereavement-informed assessment training Provide tiered intervention trainings

17 Meeting the Needs of Undocumented Children/Families When the water started coming into our apartment, we were too scared to call for help. We thought they would take my Dad away year-old girl; father is undocumented We moved here to feel safe and get away from danger. Now we don t have a home and nowhere feels safe year-old boy from Mexico; father and grandmother were both murdered We lost our house because of the hurricane, so we might have to go back to El Salvador. I can t even think about it or I ll start to shake all over year-old girl who witnessed her cousin being tortured/killed

19 Evidence of TGCT s Effectiveness (Grassetti et al., 2015; Layne et al., 2001; 2008; Saltzman et al., 2006) Results of effectiveness studies showed: - reductions in PTSD - reductions in depression - reductions in maladaptive grief reactions - improved school behavior enhanced classroom rule compliance enhanced school grades enhanced positive peer relationships increased school interest decreased school anxiety/withdrawal decreased violent behavior
