2 2 Greater Harlem Healthy Start (HCAN) Goals: Bring together community providers and stakeholders in a unified manner to help create fundamental community and systems change. The focus is not only in the Maternal Child Health arena but in the community at large. The HCAN advocates to mobilize us toward taking specific actions, and to create and implement solutions together or to benefit of the greater good.

6 Press Release: September 22, First Annual Community Awareness Day Issues: Infant Mortality, Maternal Mortality and Safe Sleep The Sudden Infant and Child Death Resource Center, reports that 109 babies died between 2013 and 2016 of sleep related accidents to empower and educate not just parents and caregivers but professionals in our healthcare systems to eliminate these deaths highlight the importance of mom s health and wellbeing

11 11 Challenge: Reaching Hardto-Reach Communities Healthy Mom Healthy Baby On Saturday November 12 th, 2016 the NMPP put on a Healthy Mom Healthy Baby event in the Polo Grounds Community Center This event was one of several planned out of a partnership between NMPP and the New York City Housing Authority (NYCHA) in order to bring services to the underserved. NYCHA Leaders very responsive, indicate that they need our services however the event had low turn out for pregnant and mom s of children under 2
