2 COMMUNITY Bronx The Bronx Health Link, Inc. 851 GRAND CONCOURSE, ROOM 914 BRONX, NY (718) BRONXHEALTHLINK.ORG Infant Mortality Reduction Initiative This program promotes women s health and the health of newborn infants, partnering with community organizations to provide outreach and referrals to services and health education workshops. Doing Our Own Learning and Support This is a workshop for young pregnant women on prenatal care, breastfeeding, and other pregnancy issues. Healthy Women, Healthy Futures This citywide program aims to promote the health of women, children and families throughout NYC. It provides FREE birth and post-partum doulas (trained, experienced professionals who provide education, preparation, confidence, empowerment, physical and emotional support to mom and partner, while facilitating communication across the birth team) to the women of NYC. Community Health Needs Assessment This group gathers and analyzes statistics on diseases, pollution, health care access / services, and individual traits such as race, ethnicity, age, and income of Bronx residents. All data is used to prepare policy recommendations, and is shared with hospitals, social service agencies and elected officials. Assessing your experiences and opinions through focus groups, interviews, surveys and forums, its goal is to improve health care and reduce health inequality in the community.

3 COMMUNITY Brooklyn Caribbean Women s Health Association 3512 CHURCH AVENUE BROOKLYN, NY (718) CWHA.ORG Breastfeeding Workshops Learn about the importance of breastfeeding, how to breastfeed and overcoming challenges. Each registered pregnant woman will receive gift cards, Metrocards, a Pack N Play and refreshments. Club Momme Meet other moms at this monthly meeting. Get answers to breastfeeding questions, baby care and enjoy refreshments. The Incredible Years: Baby And Toddler Programs This program promotes effective parenting practices and strategies that strengthen your child s social / emotional competence & problem-solving skills, while reducing aggression at home and at school. Parenting Skill Workshops: Baby (0-12 months) & Toddler Program (1-3 years) Both programs are evidence-based, promoting positive, effective, research-proven (and developmentally age-appropriate) parenting practices / stragegies known to promote children s social competence, emotional regulation and academic skills. They also focus on reducing behavior problems both at home and at school. These programs address multiple risk factors across home and social settings known to be related to the development of conduct disorders in children. The 9-week Toddler Program and the 9-week Baby Programs are a both a combination of videotape vignettes, role playing, group discussions and home assignments. Additionally, these program have been approved by the Administration for Children s Services (ACS).

4 COMMUNITY QUEENS Queens Comprehensive Perinatal Council, Inc GUY R. BREWER BOULEVARD JAMAICA, NY (718) QCPC.ORG Downstate NY Healthy Start Project (DNYHS) DNYHS is a federal initiative funded by the Health Resources and Services Administration. The Queens Comprehensive Perinatal Council provides case management intervention services to women of childbearing age who are pregnant and/or parenting an infant/ toddler up to two years of age and reside in one of its three target communities: South Jamaica (zip code: 11433) Arverne (zip code: 11692) Rockaway Beach (zip code: 11693) Health Education workshops are conducted monthly at each of the DNYHS Project offices. Topics include breastfeeding, SIDS (Sudden Infant Death Syndrome) prevention, smoking cessation, and family planning. 5 COMMUNITY MANHATTAN Northern Manhattan Perinatal Partnership 127 WEST 127TH STREET, 3rd FLOOR NEW YORK, NY nmppcares.org/ Greater Harlem Healthy Start (GHHS) GHHS offers case management, health education and other services for pregnant and parenting women & men, as well as infants up to 2 years old. The goal of Healthy Start is to support families in raising a healthy baby and reducing infant mortality / morbidity. This program assists with advocacy and prenatal care access, as well as safe sleep and parenting education. In addition, GHHS facilitates referrals for housing, public assistance, WIC, counseling, domestic violence, HIV/AIDS and other health education, home visits and escorts to health and social services appointments. Within Healthy Start is the Thrive! Project. This project offers nutrition counseling / education, physical activity, and health and wellness interventions (e.g. wellness coaching and peer support groups) that effectively reduce the incidence of obesity and improve future pregnancy outcomes among women. Northern Manhattan Head Start/Pre-K This program serves preschool children ages 3 to 5 in a center-based setting. The primary goal is to promote school readiness by enhancing the social and cognitive development of children through the provision of educational, health, nutritional, social and other related services to enrolled children and their families. Additionally, Head Start provides: Full-Day Program, five days per week Nutritious breakfast, lunch & snacks Developmentally-appropriate curriculum Certified teachers Comprehensive evaluation services for children with special needs Educational dance & visual arts Enhanced language support

6 Healthy Families Central Harlem (Formerly Baby Steps) Healthy Families Central Harlem is a voluntary, primary prevention home visiting program providing intensive home visiting services to new / expectant parents and their families residing in Central Harlem. The goal of this program is to prevent child abuse and neglect through the promotion of child health and development, positive parent-child interaction and family self - sufficiency. Services are provided to those who are pregnant or have a baby under three months old. Maternal & Infant Community Health Collaborative (MICHC) This project focuses on increasing access to prenatal and postpartum care for pregnant and parenting women who reside in the Northern Manhattan community. This program also incorporates the Thrive! project, which offers nutrition counseling / education, physical activity, and health and wellness interventions (e.g. wellness coaching and peer support groups) that effectively reduce the incidence of obesity and improve future pregnancy outcomes among women. Maternal Intentions This program is funded by Merck for Mothers and seeks to reduce maternal mortality throughout Harlem. Maternal Intentions staff work individually (in one-on-one sessions) and in group settings to help women manage their chronic conditions, adopt healthier behaviors, plan for their future, and transform their lives. FAMILY HEALTH CONNECTIONS (IMRI) The Family Health Connections program provides case management, focuses on overall well - being, reproductive health, education, job / career readiness, life and family skills. It also provides group and one-on-one breastfeeding education and support to mothers, health education workshops and referrals for services. Services are available to families in the New York City area. HEALTHY WOMEN, HEALTHY FUTURES, (HWHF) The Healthy Women, Healthy Futures program is a citywide initiative whose goal is to promote the health of women, children and families throughout the five boroughs of New York City. The HWHF program provides FREE birth and postpartum doulas (trained, experienced professionals who provide education, preparation, confidence, empowerment, physical and emotional support to mom and partner, while facilitating communication across the birth team) to women. It also provides breastfeeding and mother - baby bonding support. MBR
