Community Living Program A 14-week program that provides structured independent living skills training to individuals whose goal is to manage mental health symptoms, overcome related impairments, and strengthen 2

35 independent living skills applicable to living in the community. Individuals who successfully complete this program may be eligible for the Ibiza independent living apartments. Field Capable Clinical Services Programs (Adult and Older Adult) Two field-based mental health programs that provide the majority of mental health treatment services to adults and older adults outside the office. Services may be provided at an individual s home or at other locations that may better meet an individual s specific needs. Interventions may include: case management, medication support, crisis intervention, independent living skills training, dual diagnosis services, socialization opportunities, and individual, group, couples, and family therapy. Transitional Residential Services Program A transitional program that collaborates with local housing providers to offer comprehensive mental health and 24-hour residential care to homeless adults coping with severe and persistent mental illness. This program provides intensive mental health services to adults placed in transitional residential care for up to 6-months. Clients may transition from this program to other Alcott programs, and will be assisted to transition to more permanent housing options. Services may include: food service and housekeeping, assistance with benefits establishment, medication supervision, case management, comprehensive mental health services including rehabilitation and therapy, crisis intervention, medication support, dual diagnosis groups, peer support, and linkage to other needed community resources including medical care.
