Welcome to our 6th edition of Shaping Health: Your Choice, Your Voice. This is the time of year when the dark nights start drawing in; I think our politicians are debating the benefits of putting the clocks back. Whatever they do we know that winter is just around the corner. As is always the case around this time of year, we begin rolling out our plans to help people keep well. So no surprises we are talking about flu immunisations and how to make sure people can get the right care, first time see articles on both in this newsletter. Inside you can see that, since our last edition, the CCG has been at the forefront of two important local campaigns. Dr Hamish Stedman The first involves the CCG, Salford City Council, Salford Royal NHS Foundation Trust and Greater Manchester West Mental NHS Health Foundation Trust coming together to form a historic Alliance called Salford Together to deliver integrated care for older people. Using this joined-up approach, we want to ensure that older people are offered the best care and that those providing it are all on the same page. The second campaign is the free distribution of vitamin D, and other beneficial supplements, to all pregnant women, women up to 12 months post-natal and children aged between six months and five years. 3 Meet The New Integrated Health And Social Care Engagement Team Salford CCG and Salford City Council have joined together to develop an integrated engagement team. This means that both engagement teams will be located in the same office and will develop joint plans to engage with Salford s communities, reducing overlap and duplication. Our aim is to improve engagement by working together to reach more communities, especially those seldom heard. The city council team have specialist skills in engaging with these communities in particular those with learning difficulties, physical and sensory impairments, people with mental health conditions and older people. We are also pleased to welcome Richard Whitehead, Engagement Officer who joined our team in October. Richard has extensive experience working with communities to understand their needs. The Engagement Team Amanda Rafferty, Engagement Manager: Amanda is responsible for managing the engagement team and developing plans and strategies to meet national and local requirements. Karen Kenny, Engagement and Experience Project Manager: Karen is responsible for implementing a number of projects including community reporters, Youth Ambassadors and Patient Participation Groups. Richard Whitehead, Engagement Officer: Richard supports the Engagement Team to develop mechanisms for engagement including the Citizen s and Patient Panel, focus groups and drama workshops in schools to develop an understanding of community needs. User Development Workers: Caroline Allport Lyndsey Brook Diane Critchley The User Development Workers are responsible for engaging service users in developing policy and services. They work with traditionally hard-to-reach groups. If you have any ideas how we can reach more seldom heard groups, please Richard Whitehead at richardwhitehead@nhs.net or telephone Spread The Word, Not The Flu The 2014/15 winter flu vaccination campaign was launched in October, and we are encouraging everyone to visit their GP and make sure that you are protected this winter. Did you know that you may qualify for a free flu jab? Pregnant women, carers, children aged two four, those aged under 65 with a life-long condition and anyone aged over 65 all qualify for a free flu jab. GP Practices in Salford have been hard at work preparing for what is always a busy time of the year. Linda Beder, Practice Manager at Limefield Medical Centre in Salford (pictured second right) said: We like to start our flu campaign as early as possible. We ring all of our relevant patients and invite them to book a flu jab over the phone, and then follow this up with another call for anyone who didn t attend, giving us the opportunity to discuss any concerns that they may have. We found letters to be a less effective method when two-way communication is needed. We also place reminder stickers on all relevant prescriptions reminding patients to book their flu jab. And we re conscious that flu clinics don t always take place at times that suit patients therefore we offer appointments throughout the working week so that we can reach as many people as possible. 4 Local Mum And Toddler Face Of New Healthy Start Vitamins Campaign A Swinton mum and her toddler are the shining faces of a bright new campaign aimed at ensuring pregnant women, their unborn babies and youngsters get off to a Healthy Start. Cheryl Butler and three-year-old Joshua Butler are featuring on the front of posters, leaflets and pop-up banners across the city promoting the Healthy Start scheme rolled-out in November by NHS Salford Clinical Commissioning Group and which sees the free distribution of vitamins to all pregnant women, women up to 12 months post-natal and children aged between six months and five years. The vitamins, previously only offered free of charge to those on benefits or low incomes, contain the right amount of vitamin D to help strengthen babies bones and vitamin C to look after healthy tissue as recommended by government guidelines. Cheryl said: I think the provision of these vitamins is an excellent idea and I d encourage all those who are pregnant, just given birth, and those with children between six months and five years old to take full advantage. I took the vitamins during my pregnancy, am continuing to take them now and ensuring the kids receive their supplements too, which come in the form of drops. The vast majority of vitamin D some 90 per cent is traditionally gathered through sunlight with the rest through eating foods, such as oily fish, eggs and fortified cereals. However, given the UK s unpredictable climate, it s fair to say a helping hand is sometimes needed. Free supplements for pregnant women also contain folic acid, helping prevent spina bifida: where the spine doesn t form properly in the womb. The children s vitamins which come in the form of drops contain also contain vitamin A for growth, good eyesight and healthy skin. Those wanting more information on the scheme are encouraged to speak with their Salford Health Visitor. NHS 111 NHS 111 provides a simple, free, non-emergency phone number for Salford residents to call when they need urgent medical advice. The 111 number is available 24 hours a day, seven days a week and 365 days a year, and should be used when it s not an emergency but medical help or advice is needed quickly. The line is staffed by a team of fully-trained advisers who will ask questions to assess the situation and then direct callers to the appropriate service. This could be an NHS walk-in centre, accident and emergency, a pharmacy or an out-of-hours doctor. If the advisor believes the situation is more serious, they will be able to send an ambulance. The service provides consistent clinical advice to ensure Salford residents can access the right service, first time. You should call 111 if: You need medical help fast but it is not an emergency You think you need to go to A&E or need another NHS urgent care service You don t know who to call or you don t have a GP to call You need health information or reassurance about what to do next For less urgent health needs, call your GP or local pharmacist. 5 Salford Together A Historic Alliance to Deliver Integrated Care Salford City Council, NHS Salford Clinical Commissioning Group, Salford Royal NHS Foundation Trust and Greater Manchester West Mental Health Foundation Trust have formed a historic alliance to deliver integrated care and invest millions of pounds to improve health and social care for older people in Salford. This new Alliance will manage a budget covering Health and Social Care Services of 100 million and in October it made the first of a number of new investments. This first investment worth 2 million will boost the number of district nurses and social care staff in the local community. Salford City Mayor Ian Stewart said: This Alliance is a significant moment in the implementation of integrated care for older people in Salford, and the investments the partners are making together will help to improve the health and social care offered in Salford to all our residents. With all four agencies working closely together, with considerable involvement from community groups and contributions across the wide range of existing health and social care services from GPs and social workers through to citizen groups, we believe we can make meaningful changes in developing the support offered in our communities. The integrated care programme, branded Salford Together, aims to help those who are healthy and well stay active and busy so they remain healthier for longer, and support those who have care needs to improve their lives and manage their own care. The programme has an overall focus on improving the health and wellbeing of all older people in Salford, bringing together the contributions of GPs, district nurses, social workers, mental health professionals, hospitals, care homes, community and voluntary groups plus many more. Meet Salford CCG s Clinical Lead for Information Management & Technology (IM&T) Dr Owain Thomas In each edition of Shaping Health Your Choice, Your Voice, we will introduce a different GP. This edition sees us turn our attention to Dr Owain Thomas, GP at Langworthy Medical Practice in Salford and the CCG s Clinical Lead for Information Management & Technology. Owain talks about what he enjoys about his job, the area he works in and interests outside of the medical profession. What do you enjoy about your job? I love the variety and meeting different people. I work at a practice with a really diverse population - about two thirds of our patients are from Salford with the rest being university students. Every day is different and I get to meet people from all walks of life, of all ages and even from different parts of the world. It s a really satisfying job, I think we all like helping other people so it s nice to be able to do that as part of your job. What do you like about working in Salford? I ve worked in Salford for over 10 years now and really enjoy how dynamic it is - things are constantly changing, developing and growing. We have some really good relationships with an excellent local hospital, Salford Royal, and with our city council. I m sure that these ties will mean we can collectively make a real difference to the health of the people of Salford. What does the role of being Clinical Lead for IM&T involve? Information Technology is such an important part of healthcare and nowhere more so than here in Salford. We have an integrated record which joins together the hospital and GP systems to mean that Salford patients get better more joined up care. In the next year or two we are hoping to develop this further so that patients receiving help from the local authority (for example carers or other support) will have even better co-ordinated care. You are the Salford CCG neighbourhood lead for Ordsall and Claremont, what are the top health issues and challenges in this area? Ordsall and Claremont is a relatively young part of Salford so we have particular challenges around young peoples issues like teenage pregnancy and drug and alcohol problems. 6 New Memory Assessment Service for Salford Residents of Salford are to see the benefits of a new service for the initial assessment of memory problems, after one of the biggest investments of its kind was put forward by local commissioners. NHS Salford Clinical Commissioning Group (CCG) announced in March that it would spend an additional 1.5m on the Memory Assessment Treatment Service (MATS), to be provided in the area by Greater Manchester West Mental Health NHS Foundation Trust (GMW). The memory service is set up to diagnose patients living with dementia, which has a wide range of symptoms associated with a decline in memory or other thinking skills severe enough to reduce a person s ability to perform everyday activities. The service will be provided at Woodlands Hospital in Little Hulton and the Meadowbrook Unit at Salford Royal Hospital. A launch event took place in September with contributions from the Memory Assessment and Treatment Service and its partner organisations, including the Alzheimer s Society, Age UK, Reach Beyond and the Carers Centre. Dementia has been prioritised by the government on a national level, with 835,000 people in the UK having a dementia diagnosis. However it is thought to affect significantly more people, as diagnosis rates vary across the UK from as little as third in some areas and up to 75% in others. The Alzheimer s Society s 2014 campaign which was launched on in September 2014 aims to continue to promote early diagnosis through recognising signs and symptoms of dementia and to encourage people to see their GP and talk to someone. As such, memory clinics have become a crucial part of diagnosing and caring for people with dementia in the community. Gill Drummond, Dementia Quality Lead for GMW said, Early diagnosis in dementia is incredibly important as a timely diagnosis means that person and their network of friends and family have access to more support and a greater chance to be able to manage the effects of dementia and retain a better quality of life. Early signs of dementia can include memory loss, confusion, mood changes and problems with speech. I would urge It s time to talk. Don t let dementia be the elephant in the room Talking about dementia can be difficult. If you re worried about your own memory problems or concerned about a loved one then speak to your GP who can arrange a referral for a diagnosis. Alternatively, visit alzheimers.org.uk or call the Alzheimer s Society Helpline on Opening hours: Monday- Friday 9am-5pm, Saturday-Sunday 10am-4pm Salford now has state-of-the-art facilities available to assist dementia diagnosis and provide postdiagnosis support for you, your family and friends. anyone worried or concerned about their memory or the memory of a loved one to speak to their GP. Around 750 people a year in Salford will develop dementia and GPs can refer patients to MATS for the assessment, diagnosis and post diagnostic support they need. Dr Tom Tasker, NHS Salford CCG Clinical Lead For Mental Health, said: Unfortunately, common misconceptions surrounding dementia still persist and act as barriers to those living with the condition. The idea that dementia is a natural consequence of growing older and that little can be done for those with the condition isn t the case. In reality, there are many things that can be done in the early stages of diagnosis that can assist in making life easier and improving the quality of life. Therefore, an early diagnosis really is key. With this added NHS Salford CCG investment, we will see more patients seen quicker than before and offered appropriate support plans. I really would encourage anyone who is concerned about a loved one to go ahead and have that difficult conversation about dementia. Time is valuable. 7 Patient Participation Groups Growing in Salford Since January Salford Clinical Commissioning Group has been working with GP practices and Salford residents to increase the number of Patient Participation Groups (PPGs). PPGs typically work with GP practices to offer the patient perspective on services provided. Nearly 90% of GP practices across Salford now have a PPG established which represents an increase of more than 30% since January While the increase in patient participation is encouraging work continues to support PPGs in there development. PPG Patient Chair at Ordsall Medical Practice, Mr J Ahmed explains: The Patient Participation Group at Ordsall Health Practice has gone from strength to strength. With the kind support of Practice staff, we have become an enthusiastic group with a real desire to make a valuable difference to the healthcare in our practice, neighbourhood and beyond. The opportunity to have an Input in improving services, on behalf of, and for the benefit of the wider community is welcomed by all our members and we have embraced this responsibility by ensuring our meetings, agenda and activities are patient led. We have been involved with conducting informal research, engaging with the patients to ascertain what they feel would best improve their experience of visiting the practice. We are committed, as representatives of the patients, to improving things for everyone. Over 70 people were in attendance including members of the public, patient groups, community reporters, stall holders, Salford s NHS Youth Ambassadors and even student police officers. Participants learnt about how the CCG monitors the quality of services in Salford including Salford Royal Hospital NHS Foundation Trust, GP services and community services. Everyone had the chance to talk about their experiences of Salford s services using a range of new and exciting methods including; community reporters, Twitter and video booths. Participants also helped to decide what improvements needed to be made to GP practices as part of the CCG General Practice Development Plan. Panel members fed back their comments about the meeting, saying: I have learnt a lot about what the CCG is trying to do. Our opinions were valued and listened to. The next event will be in March and we will publicise the date and venue as soon as it is available. 8 People who can t come to events should be able to get involved in different ways. People need appropriate literature and campaigns to encourage them to go for cancer screening. Improve access to GP services. GPs should provide more information to patients about their long-term conditions and signpost them to support groups. We like being able to engage with GPs at the Citizen Panel Event. Involve more young people at events and focus groups. Educate front-line staff on the signs and symptoms of cancer (not just clinicians). x140372_cbs_p4_nm.indd 8 Members of the public who couldn t come to the panel event on 22nd October, were invited to take part in the live debate via Twitter. People were able to send in questions ahead of the event and join in the debate even though they couldn t attend. A local blood in your pee campaign is about to be launched raising awareness of bladder and kidney cancer. More information about this campaign will appear in GP surgeries, pharmacies and public venues in the city. We are currently developing a general practice development plan which aims to improve access to GP appointments. We are working in partnership with patients and GPs to achieve this and have engaged with the public at our Citizen Panel event to ask for their views on what needs to change. In partnership with patients and the public, we have developed a patient charter between the patient and GP. Patients and GPs sign up to the charter and commit to a number of principles including ensuring that the GP provides adequate information and signposts to local support groups. We organised the October event to be at the same time and same place as the GP governing body meeting. This meant that the GPs were able to join us for the workshop part of the panel to improve GP practices. We have now established a joint Salford Royal Hospital and Salford CCG Youth Ambassador Forum. Young people from the forum attended the panel event in October and gave their views on a number of issues. The Public Health team have developed an educational programme for front-line staff at the City Council, Salford Royal and Salford CCG. This will ensure that staff who come into contact with the public as part of their role are aware of the signs and symptoms of cancer and can signpost people to appropriate services. 05/11/ :03
