I have lost count of the number of times where scaffolder's have been allowed to inspect their own work only for me to identify serious faults such as low guardrails, missing and poorly installed anchor ties, at my inspection. The structures have always been signed off as safe and fit to use by scaffolding supervisors. No wonder we have them collapsing. They should also be made to show a green inspection tag as a lawful requirement.
