4 OSS OVERVIEW Biomet Oncology Salvage System Featuring The Finn Knee Introduction We are pleased to introduce the Biomet Oncology Salvage System, featuring the Finn Knee. Offering the surgeon unparalleled modularity and surgical latitude, this system fills a viable niche within the Biomet reconstructive family. Developed to provide superior stability and complete reconstructive management for defects of the femur and tibia, the OSS is designed for, but not limited to, the following indications: Significant Bone Loss Ligamentous Deficiencies Bone Tumors Primary or Metastatic Trauma Multiple Revision Arthroplasties Connective Tissue Disorders The Finn Knee is classified by type as a Class III, or salvage knee prosthesis. If you are not already familiar with this market segment or, just as importantly, if you are not yet familiar with this system, invest a few minutes of your time the return on your investment will be very worthwhile. Until recently, a salvage reconstruction was just that. It literally meant that the surgeon was looking for some way to salvage what he/she expected to encounter in the operating room. Those days have changed. As you review the Biomet Oncology Salvage System, you will quickly recognize that the design rationale and product offering have come a long way since the earliest days of similar replacement surgeries. Your surgeon now expects contemporary design, proven biomaterials, precise instrumentation and, most importantly, complete modularity within the system. History of Hinged Knees In 1951, Waldius developed a constrained, hinged prosthesis that was initially made of an acrylic material and, later, metal. During the same decade, Shiers developed a uniaxial metal hinge, and Guepar followed with a design that featured an axis that was more posterior than the earlier designs, and offered femoral resurfacing for patellar articulation. Why the interest in hinged prostheses? They are technically similar to an intramedullary design from a technique-demand standpoint. The hinge design is self-aligning, and the ligament constraints can be sacrificed, as the prosthesis is self-stabilizing. 4

5 In 1989, Henry A. Finn, MD, Professor of Clinical Surgery, Medical Director - The University of Chicago Bone and Joint Reconstructive Center at Weiss Memorial Hospital, developed, in conjunction with Biomet, Inc., the Finn Rotating Hinged Knee. Now, with the advent of the twenty-first century, we are proud to introduce the Biomet Oncology Salvage System (OSS). Created around the original concept of the Finn Knee, the OSS focuses on expanded modularity, reduced implant profiles and system-specific instrumentation. Incorporating the ongoing design changes dictated by the difficult salvage and oncologic reconstructions performed by today s surgeons, the OSS will be the system of choice. Keep in mind, however, that the early designs were simple metal door hinges. Make certain that there is no comparative association made between those early offerings and the contemporary, rotating hinge Finn design found within the Biomet Oncology Salvage System. A distal femoral component articulates with a tibial component through an axle-and-yoke mechanism. Approximating the surface geometry of the distal femur, the femoral component allows your surgeon the choice of either a resurfacing or segmental replacement. The posterior femoral condyle of both styles accepts an axle that rotates within UHMWPE femoral bushings. All tibial components accept a reinforced yoke that rotates with an UHMWPE tibial bushing. A weight-bearing interface through a unique tibial bearing creates the articulation. Thoughtfully developed to accomplish both optimal patellofemoral kinematics and the distribution of contact forces throughout the entire prosthesis, the Finn Knee, along with the entire Biomet Oncology Salvage System, offers many unique design features unmatched by any competitor. 7 Feature: Benefit: Reduced A/P and M/L dimensions of the distal femur. Minimal resection of remaining bone stock. Smaller component size also helpful with wound closure especially for the patient who has had several revision surgeries. Feature: Benefit: Anatomical center of rotation. The center of the posterior-femoral condyle is the axis of flexion-extension. The result is a restoration of normal knee kinematics. Feature: Benefit: Deepened patellar groove in femoral component. 10 Product Description 3cm Resurfacing Distal Femoral Component , The 3cm Resurfacing Distal Femoral Component is utilized for the most basic reconstruction of the distal femur. Similar to a traditional total knee replacement, this implant replaces 3cm of the distal femur, accepts any of the OSS IM stems and may be used in conjunction with any of the OSS tibial options. 5cm Resurfacing Distal Femoral Component , The 5cm Resurfacing Distal Femoral Component is utilized for larger defects of the distal femur when some portion of the distal condyles remain intact. Similar to the 3cm Resurfacing component, this implant replaces a total of 5cm of the distal femur, accepts any of the OSS IM stems and may be used in conjunction with any of the OSS tibial options. 10

11 7cm Elliptical Segmental Distal Femoral Component , The 7cm Elliptical Segmental Distal Femoral Component is utilized for complete replacement of the distal femur, but only when no additional external femoral shaft replacement is required. The elliptical collar of this implant is designed to contact the remaining flared portion of the distal femoral shaft. This implant replaces a total of 7cm of the distal femur, accepts any of the OSS IM stems and may be used in conjunction with any of the OSS tibial options. 7cm Segmental Distal Femoral Component , The 7cm Segmental Distal Femoral Component is utilized for complete replacement of the distal femur when more than 7cm of reconstruction is required. This implant is designed for use with any of the diaphyseal segments or total femur couplers; when used in conjunction with a diaphyseal segment, any of the OSS IM stem options may be employed. 11

12 Non-Modular Tibial Baseplate Short , , The Non-Modular Short Tibial Baseplate is used for those tibial reconstructions where a small tibial canal prohibits use of a modular baseplate. These non-modular baseplates are 63mm, 67mm and 71mm in the M/L plane, and have a 65mm boss stem length. No modular stem capability is available for these baseplates, and no tibial augmentation is typically suggested. Non-Modular Tibial Baseplate Long , The Non-Modular Long Tibial Baseplate is used for resurfacing tibial reconstructions where a small tibial canal prohibits use of a modular baseplate, but distal stem length is required. These nonmodular baseplates are 63mm and 67mm in the M/L plane, and have a 160mm x 10mm stem. OSS tibial sleeve augments and OSS tibial block augments may be used if necessary. Modular Tibial Baseplate , , , , The Modular Tibial Baseplate is used for resurfacing tibial reconstructions where a modular tibial stem may or may not be required. These baseplates are 67mm, 71mm, 75mm, 79mm and 83mm in the M/L plane, and have a 79mm modular stem boss. These baseplates will accept any of the OSS stems, OSS tibial sleeve augments and OSS tibial block augments. 12

13 3cm Non-Modular Proximal Tibial Component The 3cm Non-Modular Proximal Tibial Component is used for replacement of the proximal tibia. Suture holes are present in both the M/L and A/P planes for reattachment of the extensor mechanism; the non-modular stem is 240mm x 11mm. 5cm Non-Modular Proximal Tibial Component , , The 5cm Non-Modular Proximal Tibial Component is used for replacing 5cm of the proximal tibia. Suture holes are present in both the M/L and A/P planes for reattachment of the extensor mechanism; the non-modular stem is 150mm in length and is available with stem diameters of 9, 11, 13 and 15mm. 13

14 7cm Non- Modular Proximal Tibial Component , , , The 7cm Non-Modular Proximal Tibial Component is used for replacing 7cm of the proximal tibia. This implant has suture holes in both the M/L and A/P planes for reattaching the extensor mechanism; the non-modular stem is 150mm in length and is available with stem diameters of 9, 11, 13 and 15mm. 9cm Modular Proximal Tibial Component The 9cm Modular Proximal Tibial Component is used for replacing a minimum of 9cm of the proximal tibia. This implant may be coupled with any of the diaphyseal segments to reconstruct the proximal tibia. While any of the OSS I/M stems may be used with these configurations, the 150mm straight I/M stems may provide the most consistent fit. 14

15 Finn Modular Proximal Femoral Component 7cm , The Finn Modular Proximal Femoral Component may be used with either a diaphyseal segment (for reconstructing a proximal femur), or with a total femoral coupler when reconstructing a total femur. This implant offers a low-profile design, medial / lateral suture holes, an auxiliary soft tissue claw and, when coupled with a diaphyseal segment, may utilize any of the OSS IM stems. Letson Modular Proximal Femoral Component 7cm , Used in the same surgical setting as the Finn Proximal Femoral Component, the Letson offers the same design characteristics with the addition of a raised lateral flange. Soft Tissue Claw , , , , , , , Used in conjunction with either the Finn or Letson Proximal Femoral Component, the soft tissue claw provides an additional method for reattaching soft tissue. 15

21 3cm Elliptical Diaphyseal Segment Used in conjunction with the 7cm Segmental Distal Femoral component, 9cm Modular Proximal Tibial component or either style of Modular Proximal Femoral implants to achieve extramedullary cortical seating at a flared resection. Diaphyseal Segments thru , The Diaphyseal Segments are designed in 2cm increments (beginning at 3cm) to add extramedullary length in either distal femoral, proximal tibial or proximal femoral applications. A 4cm Diaphyseal Segment is also available to provide 1cm incremental buildups when used alone or when coupled with another diaphyseal segment. Segmental Stacking Adapter The Segmental Stacking Adapter is utilized when a surgery demands segmental reconstruction in 1cm increments, and it is necessary to join two diaphyseal segments for the required replacement length. Take for instance a surgery requiring 13cm of distal femoral replacement: Example: (7cm Seg Distal Femur + 3cm Diaph Segment + 3cm Elliptical Diaph Seg) The Segmental Stacking Adapter is threaded into the diaphyseal segment that will be accepting the other diaphyseal segment; this permits the Small Head/Small Thread Locking Screw to connect the two diaphyseal segments. Total Femur Diaphyseal Coupler , For use in total femoral replacements with the 7cm Segmental Distal Femoral component and either style of the 7cm Modular Proximal Femoral implants (Finn or Letson). Available in 10cm and 30cm lengths, these implants are typically added to a single Diaphyseal Segment to achieve proper femoral length replacement. 21

22 Poly Lock Pin The compression molded lock pin engages the Reinforced Yoke and Axle. Poly Femoral Bushings The ArCom femoral Bushings provide a low-friction interface of the Axle and Reinforced Yoke. Poly Tibial Bushing This implant provides a low-friction interface for the distal portion of the Reinforced Yoke. Poly Tibial Bearings thru The ArCom bearing inserts provide a low-friction interface on any OSS tibial surface. 22

Resurfacing Femoral Sleeve Augment (3cm Resurfacing Distal Femur only) , Designed for use with the 3cm Resurfacing Distal Femoral components; provides a conical build-up for canal deflicts at the level of resection. 24
