The locations on this map represent a National Education Course or Symposium where orthopaedic surgeons and other Health Care Professionals (HCPs) can train on a variety of surgical procedures, techniques, and new technologies. 6

8 NATIONAL PROGRAMS Our courses provide an opportunity for orthopaedic surgeons to learn, connect and share knowledge with peers and a panel of renowned surgeon faculty. Key thought leaders share their clinical experiences through interactive discussions around disease states, patient outcomes, and medical devices related to hip, knee and shoulder arthroplasty. The various 2014 National Courses will spotlight key technologies and techniques, such as Anterior Approach to THA, Revision Solutions featuring RECLAIM Modular Revision Hip System, GRIPTION TF Acetabular Augments, TRUMATCH Personalized Solutions, the ATTUNE Knee System and the GLOBAL UNITE Shoulder System. Attendees will then have the opportunity to hone their skills with an ample amount of time for hands on cadaveric and/or sawbones training, reinforcing the principles and technologies covered during the lecture. 8

The interactive presentations will be led by faculty highly experienced using this technique. History, evolution and procedure review for Anterior Approach THA Preoperative planning and patient selection Planning incision and capsular exposure Understanding and interpreting the x-ray Dislocation, neck cut and working with neck fractures Acetabular reaming, preparation and implant positioning Achieving optimal femoral exposure Proper broaching and implant positioning through Anterior Approach Learning curve and practice impact with the Anterior Approach Hands-on cadaver training led by experienced surgeon faculty 10

11 Advanced Anterior Approach for Total Hip Arthroplasty as described by Joel Matta, MD Anterior Approach for Revision Hip Arthroplasty as described by Joel Matta, MD June 13 Atlanta, GA April 4 Long Beach, CA This course will be conducted in an open and interactive format and will cover challenging aspects of the approach including the following topics: Soft tissue dissection Anatomy of acetabulum Anatomy of the femur Using x-ray to ensure correct leg length and offset Hemostasis, pain management and blood loss This is an advanced educational program designed for surgeons who have adopted the Anterior Approach for Total Hip Arthroplasty technique. It will be open to those surgeons who have performed at least 25 THA cases using the Anterior Approach. October 17 Atlanta, GA This course has been developed for the orthopaedic surgeon who is advanced in primary Anterior Approach for total hip arthroplasty and is interested in exploring revision THA through the Anterior Approach. The interactive presentations will be led by industry thought leading Anterior Approach faculty. The following topics will be addressed: Revision opportunities with the Anterior Approach Proper exposure for acetabular revision Gaining access to revise the femur Using traditional and revision stems with and without ETO (Extended Trochanteric Osteotomy) Advanced understanding and use of x-ray to assess leg length and offset There will be extensive time devoted to case reviews, Q&A and cadaveric training for attending surgeons. Due to the advanced nature of the topics involved in this course, each attendee will need to have completed a minimum of 100 Anterior THA cases and be prepared to begin exploring Revision opportunities through the Anterior Approach. 11

Didactic presentations will include topics such as addressing unmet patient needs, kinematic advancements, and the current state of polyethylene. The course is highlighted with a live surgery event with the ATTUNE Knee System. Meeting attendees will also have the ability to get hands-on experience with the new ATTUNE Knee INTUITION Instruments and discuss with course faculty their preferred method of total knee balancing. ATTUNE Knee: STABLE MOTION EXPERIENCE Course January 31 March 28 May 30 July 11 Miami, FL Long Beach, CA Seattle, WA Raynham, MA September 12 Atlanta, GA October 17 Henderson, NV This course is designed for surgeons interested in advancing their knowledge in the techniques of primary knee replacement surgery with the new ATTUNE Knee system from DePuy Synthes Joint Reconstruction and how it can impact their patients and their practice. The goal of the course is to give an overview of the ATTUNE Knee, including AOX Polyethylene and INTUITION Instrumentation from DePuy Synthes Joint Reconstruction. 15 Revision Solutions in Hip and Knee Arthroplasty February 28 May 2 August 8 October 10 Long Beach, CA Atlanta, GA Chicago, IL Henderson, NV This hands-on program is designed for those surgeons who are interested in more advanced education on both hip and knee revision arthroplasty. The course is led by a world-class surgeon faculty, providing an opportunity to learn best practices, such as pre-op and intra-op efficiencies, that you can then incorporate into your surgical workflow. The agenda will consist entirely of either hip or knee revision topics, and will provide ample amount of time for both interactive discussion with course faculty and hands-on cadaveric training utilizing the most advanced revision technologies from DePuy Synthes Joint Reconstruction, including CORAIL Hip Revision, RECLAIM Modular Revision Hip Stem and GRIPTION TF Acetabular Augments for Revision THA. Revision knee topics will include the application of Rotating Platform bearings in the revision setting and utilization of metaphyseal sleeves for fixation. REVISION THA SESSION Preliminary Overview: Risk mitigation in revision THA Understanding implant options, patient selection and pre-operative preparation. Common surgical exposures and component removal options Fighting infection causes and treatment options Surgical considerations for femoral prostheses, including both monoblock and mid-modular options. 16 2014 National Programs SHOULDER ARTHROPLASTY Glenoid Exposure Mastery Course Anatomic to Reverse Shoulder Arthroplasty February 21 Atlanta, GA June 13 Orlando, FL Proper access to and exposure of the glenoid joint is a key element of success in shoulder surgery. This anatomic shoulder course is focused on sharing the latest techniques in achieving consistent glenoid exposure, glenoid preparation and glenoid component fixation. This course will provide surgeons with a step by step look at various exposure techniques as well as tips and pearls in Total Shoulder Arthroplasty. Didactic presentations will be given by key total surgeon experts, and the course will allow for a high degree of interaction with course faculty. July 11 August 15 September 26 October 24 Phoenix, AZ Raynham, MA Henderson, NV Denver, CO This course has been developed for the orthopaedic surgeon who is interested in Anatomic and Reverse Shoulder Arthroplasty, as well as exposure. The interactive presentations will be led by a renowned panel of faculty experienced in shoulder arthroplasty. 19 This comprehensive two-day meeting will address the latest advances in total Hip and Knee Arthroplasty. Thursday will focus on the Anterior Approach procedure as described by Dr. Joel Matta and other relevant hip topics. 20 RESIDENT & FELLOWS EDUCATION Since 2001, DePuy Synthes Joint Reconstruction has facilitated the training of over 2000 orthopaedic residents through our popular Resident Training Programs. We will offer a Junior Resident program, targeted to 2nd and 3rd year residents, and a Senior Resident Program, targeted to 4th and 5th year residents. The Junior Resident Program is an introduction to arthroplasty and the Senior Course focuses on more advanced topics and surgical solutions. Both programs have an experienced faculty panel, representing significant resident training credentials. The Professional Education Department funds travel, lodging and modest meals for residents who attend these programs. 22 Resident and Fellows Education RESIDENT EDUCATION 2nd and 3rd Year Residents Symposium 4th and 5th Year Residents Symposium September 4 6 Rosemont, IL September Rosemont, IL This two day program features the basics of total joint arthroplasty for hip and knee, and is designed for the second and third year resident. The course is a combination of faculty presentations, handson sawbones exercises, interactive debates, and Q&A. An experienced faculty will assist residents with developing fundamental skills and treatment options for patients indicated for primary joint arthroplasty. This is a two day program that features discussions and intensive reviews on hip, knee and shoulder arthroplasty and is designed for the fourth and fifth year Orthopaedic Resident. Participants will interact with a distinguished faculty that will address key orthopaedic issues including: history of product philosophies, surgical techniques, overview of biomechanics, product design rationale and panel discussions. The program is a combination of didactic lectures, small group discussions, case reviews and excerpts of live surgery footage. 22

Anterior Approach for Total Hip Arthroplasty Course May 8 Chicago, IL Invitation Only This invitation-only course has been specifically developed for the orthopaedic fellow who is interested in the exploring the Anterior Approach for total hip replacement. The course will be led by highly experienced faculty and will feature both interactive didactic presentations and handson cadaveric educational opportunities in a lab setting. Shoulder Fellows Meeting May 29/30 Henderson, NV Invitation Only This invitation only course is designed for the shoulder arthroplasty fellow interested in enhancing their knowledge of both shoulder arthroscopy and arthroplasty. A world-class faculty of experienced surgeons will explore an array of diverse topics including glenoid exposure, instability and fracture management and the core principles of shoulder arthroplasty via interactive didactic presentations Additionally, program attendees will ample time for hands-on experiences in a cadaveric lab setting. 23

26 Local, Personalized Education Programs SMART LABS Small, Market Appropriate, interactive, Training (SMART) labs are designed to be very focused in nature, taking place in a more intimate setting. They are facilitated by one DePuy Synthes Joint Reconstruction surgeon consultant educating a small group of health care professionals. SMART labs are more technique intensive with handson exposure utilizing cadavers and/or sawbones. This program is designed for surgeons interested in learning more about new surgical techniques for total Hip, Knee, or Shoulder Arthroplasty. Compared to our National Learning Centers, there is less emphasis on lectures and case reviews and an increased focus on hands-on instructional lab work. 27 OR VISITATIONS OR Visitations provide you with the benefits of surgical observation of a DePuy Synthes Joint Reconstruction surgeon consultant in his or her operating room. As the visiting surgeon, you will have the opportunity to view multiple surgeries during your time in the O.R., based upon your desired educational focus. 28 Local, Personalized Education Programs LOCAL MEETINGS PEAK (Platform for Education and Knowledge) Events Allowing for a local education experience close to home, PEAK events feature a DePuy Synthes Joint Reconstruction faculty surgeon discussing educational topics of your interest. These include surgical techniques, material science, implants and instruments, and Operating Room efficiencies. PEAK Event programs typically consist of didactic presentations and product displays. The DePuy Synthes Joint Reconstruction surgeon faculty could be present at the dinner or participating via real-time video conferencing, with both scenarios allowing for live two-way communication with the surgeon faculty. Focused Interactive Telepresence (FIT) Program These programs create a quick and efficient learning opportunity utilizing our new Focused Interactive Telepresence (FIT) system. Working with Fleming s restaurants and Sensory Technologies, DePuy Synthes Joint Reconstruction has developed a defined educational dinner meeting program utilizing video conferencing technology that can be executed at any of the 64 Fleming s locations. FOCUSED agendas on multiple contemporary topics are currently available. INTERACTIVE environment provides real-time, two way communication with HD video quality, allowing for meaningful discussions between the DePuy Synthes Joint Reconstruction Faculty and dinner attendees. TELEPRESENCE technology by Cisco Technology, Inc. is utilized by both the DePuy Synthes Joint Reconstruction Faculty and the private dining rooms at Fleming s locations providing a virtual, controlled and quality educational environment. 29 WORLD WIDE CUSTOMER VISITS A World Wide Customer Visit is a program designed to provide Healthcare Professionals with an opportunity to experience the Warsaw campus and meet with the DePuy Synthes Joint Reconstruction professionals who bring passion and commitment to our products. Attendees primarily consist of orthopaedic surgeons, physician assistants, nurses, surgical technicians, hospital administrators, and other orthopaedic healthcare professionals. The Professional Education Department is responsible for coordinating and conducting World Wide Customer Visits. A member of the Professional Education team will facilitate pre-visit consultations to create a customized agenda that meets your unique educational needs. 30 DEPUY SYNTHES TRAINING FACILITIES DePuy Synthes Institute Raynham, Massachusetts The DePuy Synthes Institute is the largest educational facility of its kind on the East Coast, located in Raynham, Massachusetts. The Institute houses a sub-dividable auditorium, with seating for over 200; an additional six classrooms with seating for 25-60; and laboratory space to accommodate up to 19 stations for hands-on training. State-of-the-art communications equipment broadcast programs online or archive them for future use. Also, wireless internet is accessible throughout the facility. You are invited to explore the DePuy Synthes Institute, and advance your expertise with exceptional educational opportunities. DePuy Synthes Institute Chennai, India The Institute in Chennai is the first of its kind outside the United States. This 30,000 square foot facility caters to the continuing education needs of healthcare professionals in India. Training at the DePuy Synthes Institute in Chennai will allow surgeons to keep up with rapidly increasing patient demands. More importantly, the tiered, comprehensive programs will help surgeons learn and develop surgical skills, incorporate new technologies, and enable them to have a positive impact on patient treatment. Main features include an auditorium with seating for 100 people; classrooms, conference rooms, a reading area and a large sized cafeteria; a saw bones lab; and, an off-site state-of-the-art cadaver facility. DePuy Synthes Institute Palm Beach Gardens, Florida Anspach welcomes guests to its newly constructed medical education facility at the Anspach headquarters in Palm Beach Gardens, Florida.
