5 About Egypt Egypt is probably the world's oldest civilization having emerged from the Nile Valley around 3,100 BC, historically. Egypt is probably one of the oldest vacation spots. Early Greeks, Romans and others went there just for fun, and to see the wonders of some of mankind's earliest triumphs. But Egypt is much more than Pyramids and monuments. It is also Red Sea scuba diving, hot night spots, luxury hotels and five star restaurants. It is romantic cruises down the Nile on festive river boats, a night at the grand opera and it is a cultural experience like none you have ever experienced. Egypt is a land bustling with life, sound, visual beauty and excitement. The stunning, colossal monuments of Ancient Egypt never fail to astonish. The heat, sight and the light of the desert invigorate. The cool, turquoise waters rejuvenate The Great Pyramids of Giza is an Ancient Egyptian landmark. Known as Khufu's Pyramid, it is the greatest pyramid of the complex: a truly overwhelming sight. Being one of the seven wonders of the ancient world, it is the only one still standing to this day! When gazing at this colossal structure, there s no way to escape the feeling of being dwarfed sisters.nearby, are the Great Sphinx, the Solar Boat Museum and the Sound & Light Show. About October 6 City This is a satellite city of Cairo. A new cosmopolitan city, it hosts students of many nationalities who seek education at its private universities. It was established in 1979 and Named in commemoration of the liberation of occupied Sinai and the successful crossing of the Suez canal by the Egyptian military in It is 17 km from the great pyramids of Giza and 32 km from Cairo downtown. It has a total area of (400 km2) and is expected to have 3.7 million nhabitants but current population is about It hosts one of the largest industrial zones in Egypt. It is also accompanied by a Banking Sector that groups branches of all banks in Egypt. The area is also home to over thirty universities, colleges and higher educational institutes. The neighboring residential area Sheikh Zayed city is notable with its several gated communities and luxurious compounds. It is connected to Cairo by a highway bridge (Mehwar). It is considered to be one of the most thriving and fruitful modern urbanisation projects. 2

6 Vision and Mission Orthopaedic Department Our vision is to improve the quality of orthopaedic service following international standards for quality control, accustomed to fit our local and national conditions. Our mission is to improve the quality of orthopaedic service following international standards for quality control, with slight modifications to fit our local and national conditions; to provide a high-quality clinical research and innovative methods that are relevant to our community and leveled to international standards; to pay back, by teaching others, what we have learnt using our skilled human resources, motivation and dedication; to educate our patients, students and allied health professionals; to teach our young surgeons the theory and the practice of modern orthopaedics. 7 Glance at October 6 University (O6U) The pioneer of private university education since its establishment according to the presidential decree number 243 for the year A member of the Association of Arab Universities. A member of the Association of African Universities. A member of the United Nations initiative on corporate social responsibility (UN Global Compact). Shareholders in O6U are institutions and companies, which adds a special institutional character to the university. All the degrees awarded by the university are approved and accredited by both Supreme Council of Universities of Egypt. The language of instruction is in both Arabic and English. The university is in a collaborative partnerships and agreements with many foreign and Egyptian academic institutions. The number of students is estimated at approximately 12000,one third of that number are international students drawn from more than 20 nationalities. The total area of the university campus is thousand square meter (excluding the library building which covers 9000 square meter). Green landscaping and playground occupy vast areas of the campus. The university campus houses four educational buildings and one administrative building, university hospital and girls hostel. As for the central library and boys hostel, they lie about 150 meters from the university campus. The total capacity of the teaching halls and rooms in addition to labs, workshops and special halls can accommodate up to (20) thousand students at a time. The university offers its services to students through the departments of cultural relations, international students, public relations, youth care, social solidarity program and transportations service. The emergency department works round the clock. A huge library which is considered a center of cultural dissemination with different halls that display a variety of Arabic and foreign reference books. 9 The Faculty of Medicine The Faculty of Medicine is dedicated from its beginning to offer high-quality medical education to medical students and graduate physicians. Teaching, health care service and scientic experience are offered by excellent well-selected medical staff recruited from the most reputable and leading Egyptian medical faculties. Our students are supported by a well-equipped hospital which is sufficient enough to provide the pre-requisite clinical training essential for the acquisition of clinical skills by medical students. They are in continuous contact and collaboration with the famous local and international medical institutes and professional centers. October 6 University is looking forward to achieve the international eminence as an academic institution for continuing medical education, clinical research and practice. The first batch of students was graduated on February Now, we are on the steps of getting the international accreditation supported by the World Health Organization (WHO) which confirmed the inclusion of the Faculty of Medicine, October 6 University, within the WHO World Directory of Medical Schools. Graduates of the college are eligible to apply to and sit for PLAB section of the British Council in Cairo and the General Medical Council of the UK and other professional tests after fulfilling the requirement of the internship year of training. 11 Orthopaedic Department Thanks to the effort of Professor. Baha El Serauy and Dr. Khaled Fouda who both served as the heads of the department during 2001 to They established a service out of nothing and paved the way for the forthcoming achievements. The department has a wide range of specialties including non surgical treatment for arthritis and artificial joint replacement for hip and knee. Other orthopaedic specialties supervised by a group of elite orthopaedic consultants from the UK and faculty members of Egyptian universities include sports injury and joints arthroscopy, vertebral column (neck and back) diseases, pediatric orthopaedic, various bone diseases ( e.g., osteoporosis, rheumatoid arthritis and tumors), acute and chronic pain relief, shoulder and hand diseases. The physiotherapy group within the department works on osteoarthritis of back and neck, deformities, joint stiffness, after fracture care, pre- and post-surgery rehabilitation, sports injuries, ligaments sprains, motor-retarded children (arranged reservation), paralysis, peripheral neuritis, labor injuries and geriatric rehabilitation. The group provides a gym facility to help improve physical fitness (especially for elder people) Total and comprehensive care at home during rehabilitation by the orthopedic surgeons and physiotherapists is also provided. 15 Center of Excellence The department has a wide range of services including nonsurgical treatment for arthritis, artificial joint replacement for hip and knee, total, primary, unicompartmental and revision joint replacement, performing operations using navigation, computer assisted surgery or custom-made cutting guides, follow up with previously made joint replacement as well as trauma and fractures. All specialties are supervised by orthopedic consultants within the hospital. It provides immediate care for road accidents and work injuries and special treatment for hip and pelvis fractures in elderly people. Dr Hafez (the Head of the department) established the centre of excellence based on his 17 years experience abroad. He was trained in the UK for > 10 years, passing through basic (SHO) & higher (SPR) surgical training and working as senior registrar and locum consultant. He had subspecialty experience in hip and knee arthroplasty gained from 3 international centres at: Wrightington Hospital, Wigan, UK (2004) Western Pennsylvania Hospital, Pittsburgh, Pennsylvania, USA (2005) St Michael s Hospital, Toronto, Ontario, Canada (2006) He had special interest in Navigation and CAOS, particularly in hip and knee arthroplasty He is a pioneer in patient specific instruments (PSI), as he introduced the concept and application of this novel technique. His interest in CAOS started in 1998 with a one-year fellowship in navigation and robotics, University of Hull, UK. In 2001, he initiated a laboratory project at the University of Leeds, UK that was formally registered as a PhD (MD) work and resulted in the development of the novel technique of PSI for total knee arthroplasty (TKA). 16 Prof. Hafez published more than 10 book chapters as well as several editorials and original papers in this field. He organized the first international symposium on PSI, Sharm El Sheikh, Egypt, January 2011, and has been organizing the yearly Mid-East CAOS conference since He was invited to organize or participate in talks, workshops, and instructional course lectures and to chair sessions in this field during international conferences (e.g., SICOT, AAOS, CAOS International and Cleveland Clinic Knee Summit). He is selected as a reviewer of PSI technology by ECRI and as a Consultant to Confor MIS, US. The Orthopaedic Department of October 6 University provides evidence-based practice and advanced medical service by consultants who have experience in Egypt and abroad. It builds a database and follow up for major operations. It utilizes hi-tech computerized surgical techniques and navigation surgery. It also collaborates with telemedicine consultation and colleagues at centers of excellence in the UK, USA and Canada. The department staff members are pioneers in patient-specific cutting guides for total knee replacement (TKR), navigation for TKR in the Middle East, total hip arthroplasty (THA) by computer-assisted surgery and community arthroplasty register in the Arab World. They provide primary and revision of total hip and knee replacements as well as unicompartmental knee and hip resurfacing. CLOSE UP Director of Medical Services: Dr. Hisham M.Farag Dr Farag is a Fellow of the Royal College of Anesthesiologists, UK. He served as an anesthesia and pain management consultant at San George Hospital, UK, and as an Associate Professor at King Faisal University and contributed to the foundation of the first pain management center in Saudi Arabia. He also worked as an anesthesia consultant in Aramco company hospitals in Saudi Arabia. He is the author/coauthor of many publications in the field of pain management in American, Canadian and British magazines. He is a specialist in vertebral column pain management. 17 Clinical Care Arthroplasty services in our unit are nearly comprehensive and unique as they include different types of traditional techniques and recent such as minimally invasive and computer-assisted surgery. The scope of our services is joint replacement: traditional techniques in arthroplasty, total knee arthroplasty, unicompartmental knee arthroplasty, hip hemiarthroplasty, resurfacing hip arthroplasty, total hip arthroplasty (cemented, hybrid, uncemented, metal on metal and ceramic on ceramic), shoulder hemiarthroplasty and total shoulder arthroplasty.we also target minimally invasive surgery (MIS), mini lateral approach for the hip, MIS total knee arthroplasty, navigation and computer-assisted surgery. The outcome assessment for our patients includes arthroplasty sheets (hip and knee scores) and arthroplasty register. The operating rooms are dedicated with tele medicine unit. We have international collaboration with cleveland Clinic, Ohio, USA (1st Arthroplasty Conference 2008, 3rd Arthroplasty Conference 2010, 4th Arthroplasty Conference 2011); UK (review course 2009, review course 2010 braddock visit). We also share consultation with colleagues at cleveland Hospital, Ohio, USA; San Michael s Hospital, Toronto, Canada: hospitals of the University of leeds, UK. 19 Case 1: Young patient with complex THR Examples of our cases Case 2: THR & correcting 6 cm shortening 22 years old female with bilateral fused hips following multiple surgeries for DDH at age 2. She has been unable to sit or walk properly and had hip flexion deformity 30 degree. She had complex bilateral primary THR in 2008 Case 3: Conversion of old fused hips to THR Computer assisted THR for longstanding bilateral fused hips in 52 years male who had bilateral fracture neck of femur and multiple failed surgeries 27 years ago. Case 5: Revision surgery for 4 joints Total hip replacement (THR) with computer aided planning for 6 cm shortening and a very small cup 38 mm. This 29 years old female had infection Lt hip at age 7. Case 4: Reconstruction of 6 joints in one patient 35 years old male with Juvenile Idiopathic Arthropathy (JIA). He presented with fused bilateral hips, knees & ankle & 40 degree flexion deformities of hips & knees He had 4 joints replacement for hips & and knees and pantalar arthrodesis for right foot & ankle. Every 2 joints were done in single sesssion due to difficult intubation Case 6: Bilateral simultaneous revision knee implants Computer assisted bilateral knee replacement following failed femoral osteotomies & revision of bilateral failed hip replacement. This rheumatoid patient was non walker due to knee flexion deformities and severe hip pain but was able to walk after surgery. Bilateral simulteneous revision knee implants for ligmenteous instability and bone loss. Case 7: Bilateral simultaneous uni-compartmental 16

20 ( ) Patient speci c instruments (PSI) for total knee replacement: The open platform technique Patient specific instruments (PSI) for TKR is now used almost routinely in our center by Prof Mahmoud A Hafez. The technique used is called an open platform PSI, as it can be used for any TKR implants. The personal experience of Dr Hafez indicate that this PSI technique can make a revolution in the management of TKR in the Middle East. 21 In addition, here are the objective findings based on our clinical outcome studies. Egyptian Community arthroplasty register (>300 TKR) For PSI: Blood transfusion rate 5%, walking within 3 hours postop, less compliction rate and hospital stay <3 days. A study on 100 bilateral simultaneous TKR for OA with severe articular deformities (>20 degrees) for PSI: Hospital stay 4 days, operative time reduced from % and walking same day. A study comparing conventional, navigational and PSI techniques on 150 TKR PSI accuracy is comparable with navigation but PSI has the shortest operative time and is the least expensive. 45 Al Mohsen Medical Charity Al Mohsen Medical Charity organization was established after the death of Dr.Mohsen Abu Zeid (Consultant Plastic Surgeon, UK) and Dr. Mustafa Taw q (Consultant Orthopedic Surgeon, Saudi Arabia), in December 2009, with the help and cooperation of Dr.Mahmoud Hafez, Dr.Sameh Ansara, Dr.Essam Hassan and Dr.Hussein Mousa Gomaa. It took about a year to establish the organization. Finally, stablishment procedures, headquarters and members are approved by the Ministry of Social Affairs. The capital of Al Mohsen comes from voluntary contributions of the founders and members of board of directors. 46 A Fund from Misr El-Kheir (MEK) Foundation to Perform Kneee replacement operations for needy patients from deprived and poor areas in egypt. We successfully managed to deliver our promise and to achieve our targets as follows: 1) We managed to perform 22 cases of knee replacement using a very limited funding (almost 1/3 of the usual cost) 2) We used NexGen (Zimmer, USA), one of the best knee implants (prosthesis) worldwide 3) We have 3 to 12 months follow up with good results (technical and functional) and no complications 4) We performed clinically indicated bilateral knee replacement, which is not usually used in Egypt 5) We used the top technology of computer assisted surgery; patient specific instruments (PSI), which is not available in any center in Egypt or the whole Arab countries. 6) We managed to reduce the need for blood transfusion down to 5 % for single knee replacement and to 16 % for bilateral cases. This is important in Egypt as it reduces the risk of hepatitis B and C 7) We trained more than 10 young surgeons (trainees). Some of these cases were used for 2 MSc theses and one MD thesis 8) We invited 13 experienced surgeons to perform knee replacement using PSI technique from Cairo university, Suez Canal University, Assiut University, El Sahel Teaching Hopsital, El Helal Orthopaedic hospital, October 6 University and others). Some of the surgeons are heads of the orthopaedic departments 9) We presented and published our results in international conferences 10) These case are included in the Egyptian arthroplasty register, which is listed in the International Society of Arthroplasty Registers (ISAR). 43 El Mohsen Medical Prize 2013

47 October Medical Magazine October 6th City is a fast growing new city on both industrial and residential levels. It has some universities, two medical schools and more than 10 general hospitals. Yet, there is no regular journal, magazine or newspaper to connect the medical community of doctors, nurses, paramedics and employees with the local population. October Medical Magazine has been launched to fill this gap. Moreover, there is no medical publication that connects the medical community in Cairo and the whole of Egypt. It is a challenge to establish and maintain a new medical publication that caters for both medical stuff and the public. In our first issue, we decided to combine a medical magazine in Arabic with a medical journal in English. Each has its own style and language, but they share the same readers, mainly doctors who study and write medicine in English and practice it in Arabic. 44
