4 Health Status Index Series Vol XVII No II pg. i PREFACE This edition of the Leading Causes of Death in Chicago has a similar content and organization as the 00 version with the exception of changing from tabular to graphic presentations of mortality rates. This revision was introduced to better emphasize the differences between 00 and mortality. The report begins with data on the 00 leading causes of death for Chicago residents presented for different ethnic groups and sex by detailed age categories, and for different age groups by ethnicity categories. The report concludes with ten year comparisons of mortality rates for 00 leading causes and all-cause death by ethnicity and sex. It is hoped that this report will prove helpful in developing effective public health interventions in Chicago. Questions, comments or suggestions for improving future editions are welcome. 5 EXECUTIVE SUMMARY For all Chicago residents, regardless of gender or ethnicity, all-cause age-adjusted mortality rates decreased from to 00. The ten leading causes of death for Chicago residents represented % of all deaths, and 0% of all infant deaths. Although the positioning changed slightly, the 00 ten leading causes of death were the same as those for 00 and most of the 00 ten leading causes of infant death were the same. disease and cancer were the top two leading causes of death for all Chicago residents, also for both sexes and all ethnic groups studied. HIV/AIDS was a leading cause of death for only one population studied non-hispanic Blacks, whose age-adjusted mortality sharply decreased (%) during the study period. Disorders related to prematurity (short gestation and low birth weight) and birth defects (congenital malformations, deformations and chromosomal abnormalities) were the top two leading causes of infant death for all Chicago residents, also for both sexes and all ethnic groups. Infant mortality (reflected in the cause Certain Conditions Originating in the Perinatal Period) is one of the leading causes of death for Chicago s Hispanic population. For the - age group, homicides and accidents are the two leading causes of death for all Chicago residents, and for all ethnic groups except for non-hispanic Asians. th Homicide was the 0 leading cause for all Chicagoans, and the th leading cause of death for Hispanic and non-hispanic Black populations in Chicago. Age-adjusted mortality rates due to heart disease and stroke decreased from to 00 for all Chicago residents as well as for both sexes and all ethnic categories. Cancer mortality decreased from to 00 for all Chicago residents as well as for both sexes. However, age-adjusted mortality rates due to cancer only decreased over time for non-hispanic Whites, all other ethnic categories increased from to 00. Mortality rates for diabetes remained relatively stable or decreased from to 00 for all Chicago residents, both sexes and all ethnic groups, except for a subgroup of the Hispanic population. Diabetes mortality increased by % in Mexicans. Septicemia was the only leading cause of death for all Chicagoans to increase over the 0-year study period (%). The Hispanic population had the greatest increase, septicemia mortality rose by %. Chicago females and the non-hispanic White population saw dramatic increases in age-adjusted mortality rates due to Alzheimer s Disease in the 0-year period (-00), % and % respectively. 8 BACKGROUND In order to understand and appropriately interpret the data, it is important to understand: data reporting criteria, cause of death determination, ranking procedures, and rate calculations. DATA REPORTING CRITERIA The guidelines for data release and cell suppression used by CDPH try to balance data accessibility with privacy concerns and confidence in the stability of the estimates that we publish. Rates, percentages, and ratios based on twenty or fewer events can vary widely just by random chance even when there is no meaningful statistical difference between measurements. Rare events occurring in large populations do not usually allow for the identification of individuals, but reporting small cell sizes for population subgroups increases the risk of breaching confidentiality. The guidelines for cell suppression used in this report (adapted from the Massachusetts Department of Public Health) are based on both the numerator (event) and denominator (population or group size) values. To prevent back calculation of suppressed numbers, secondary suppression of at least three other cells may be performed in conjunction with the primary suppression. See Table for specific reporting conditions. Health Status Index Series Vol XVII No II pg. CAUSE OF DEATH DETERMINATION All deaths are registered using death certificates. Death certificates are processed and analyzed by a standardized system that is used in most countries throughout the world, the International Classification of Diseases (ICD). On the death certificate, the certifier (a physician, coroner or medical examiner) enters information about the causes and circumstances of death in a specific sequence. When received by the vital statistics office, the causes are placed in standard categories, and the underlying cause of death is then determined. Underlying cause of death is not necessarily the first medical condition listed on the death certificate. All of the information on the certificate is examined systematically, using classification rules, to determine the most appropriate underlying cause of death. Table. The system is periodically revised to reflect advances in medical knowledge and public health priorities. The ninth revision, ICD-, was used for deaths in the United States for twenty years, -. The ICD- 0 is the most recent update, and is being used for US deaths effective with data from. There are important differences between ICD-0 and its predecessor, ICD-, including the following: The number of detailed categories nearly doubled. Important changes in some of rules for classifying underlying cause of death. Therefore, even with the same information being recorded on the death certificate, a death can be attributed to a different underlying cause of death. Due to these differences, mortality rates based on data classified under ICD-0 cannot be directly compared to ICD-. In order to compare mortality statistics based on deaths classified by ICD- to ICD-0, a comparability ratio can be applied to counts and rates. There is a different ratio for each cause of death, determined by a comparability study conducted by the National Center for Health Statistics. 9 RANKING Tabulation lists are part of the ICD system. These lists standardize the reporting of data which makes it easier to compare data from different times and places. The lists aggregate detailed categories into appropriate groupings, but also highlight specific causes of death that are of public health and medical importance. Ranking is performed on the basis of the tabulation lists. Not all causes of death are considered rankable. For example, if subcategories of a larger category are listed on a tabulation list (e.g. specific forms of cancer are listed in addition to all cause cancer), then only the larger category is considered for ranking purposes. Another important exception are causes of death that are remainder categories for larger groupings. As an example, the grouping Other diseases of the respiratory system is not considered in leading cause of death ranking. Further detail can be obtained from the National Vital Statistics Reports and other publications of the National Center for Health Statistics. RATE CALCULATIONS Rates for causes of death and all-cause mortality are age adjusted in this report. Age adjustment is a technique that is used to make it easier to interpret differences in disease rates between different populations. Most important health conditions vary with age. Different ethnic populations vary in their age composition. Similarly, a population can vary in its age composition over time. When comparing rates of death, we would like to be able to separate what differences are due to potentially modifiable risk factors and what differences are due to age. With direct age adjustment, a reference or standard population is identified, and weights are assigned to age-specific groups based on their representation in the standard population. The age adjusted rate for a population is a weighted average of the age group specific rates for the population, using the weights derived from the standard population. Age adjusted rates that are derived using the same standard population can be compared to each other, but they CANNOT be compared to age adjusted rates using different standard populations. Age adjusted rates are summary statistics for comparisons, and NOT actual rates - they cannot be applied to a specific population to get the actual number of events that occurred in that population. The projected US population for the year 000 is currently used as the reference (standard) population for age adjustment by CDPH. This makes our data consistent with reports issued by all agencies within the US Department of Health and Human Services, who uniformly adopted the projected US population for year 000 as their standard for age adjustment for health events or deaths occurring in or later. See the Appendix for the age-adjustment weights used in this publication. SPECIFIC METHODS FOR THIS REPORT DATA SOURCES All mortality data come from the Illinois Department of Public Health 00 and death datasets. Population denominators for calculating 00 and age-adjusted mortality rates were estimated based on population data from the 000 and 0 US Census and the 00 American Community Survey (ACS). RANKING For infants under, the number of deaths from each of the rankable causes of the List of 0 Selected Causes of Infant Death, as well as all-cause mortality, is tabulated. For all other age groups, the number of deaths from each of the rankable causes of the List of Selected Causes of Death, as well as all-cause mortality, is tabulated. The ten causes with the highest frequencies for each age group are then reported. In the case of a tie, deaths are listed in order of their appearance in the ICD-0. The rankable causes from each list are listed in Tables A- and A- in the Appendix. The abbreviations used in this publication can be found in Table. POPULATION ESTIMATES Intercensal population estimates were utilized in this report to improve the accuracy of the age-adjusted mortality rates. Linear interpolation modeling for was performed using 0 and 000 U.S. Census for all sex and ethnic categories. ACS data from 00 was used for All People, Hispanic and Non-Hispanic White populations. 10 ACS does not publish population estimates by sex and age for Hispanic subgroups, Non-Hispanic Asians, Non-Hispanic Blacks and Non- Hispanic Native Americans, only population totals. In order to estimate these populations Health Status Index Series Vol XVII No II pg. by age and sex, the proportion of the population in each age group (for females, males and all people) in 000 was multiplied by the total population of persons in 00.
