2 Hospital: Leicester Royal Infirmary Directorate: Womens and Childrens Grade: MTI Paediatric Oncology (ST3 for 6 months and then ST4-6 for 18 months) Hours Per Week: 48 Salary Scale: Salary payment is according to the MN37 pay scale at 30,002-47,115 according to level of previous experience Unit Description: For the current round of RCPCH applications to MTI-p, we are delighted to offer 6 months in Acute/General Paediatrics and 18 months in Paediatric Oncology/Haematology at University Hospitals Leicester NHS Trust. This job is under the Medical Training Initiative (MTI) scheme run by the Royal College of Paediatrics and Child Health. Please note this post has been quality assured by the MTI team at RCPCH The Leicester oncology and malignant haematology service is part of the East Midlands Children s and Young Persons Integrated Cancer Service (CYPICS). This service is a partnership between the University Hospitals of Leicester and Nottingham University Hospitals and clinicians work together as part of CYPICS. CYPICS manage new cancer and leukaemia patients per year (about 1/3 will originate from the southern part of the East Midlands and 2/3 from the northern part of the East Midlands). The service delivers comprehensive cancer care to patients from all the counties in the East Midlands and works closely with the District General Hospitals in the region. The CYPICS co-ordinating centre based at Nottingham Children s Hospital manages the patient journey, in close consultation with clinicians. There is a weekly regional Solid Tumour MDT and a weekly Neuro-oncology MDT (video-linked). A 2-weekly regional Paediatric Malignant Haematology MDT meeting includes clinical, laboratory and cytogenetic review of leukaemia cases (newly diagnosed and at critical time points during treatment). The Paediatric Oncology/Haematology service offers training opportunities for General Paediatricians, those training with an interest in related sub-specialities and those aiming for consultant roles in shared care Paediatric Oncology. Working within Paediatric Oncology/Haematology provides the opportunity to develop several skills: Working with children and families who are facing serious and potentially life-limiting illnesses Caring for acutely sick patients including HDU-level patients on the ward. Managing those with multisystem failure, complex fluid balance issues, acute sepsis, infection in the immunosuppressed child Developing communication skills e.g. breaking bad news and dealing with medically stressful situations Working within well-functioning multidisciplinary teams which deal with the diagnosis, clinical management, psychosocial and late effects of cancer in children Understanding the varied presentations of childhood cancer and how often cancer mimics common diseases in childhood Developing skills and an understanding of the management of cancer in children Experience of clinical research trials (the majority of children with cancer participate in clinical trials) and this is part of routine working within the Unit
3 Demographics The Leicester paediatric oncology/haematology service is based in the Leicester Children s Hospital on the Leicester Royal Infirmary site. We have an integrated paediatric ward and dedicated Teenage Cancer Trust unit providing services for children and young people from 0-24 years with a variety of malignant disorders such as leukaemias, lymphomas, soft tissue sarcomas, germ cell tumours, bone tumours & brain tumours. The department also looks after children with non-malignant haematological disorders like ITP, hereditary spherocytosis, thalassemia, sickle cell disease and bone marrow failure syndromes. The day-care unit is adjacent to the ward and we also run regular paediatric and teenage and young adult outpatient clinics covering: Non-malignant Haematology Solid tumours Leukaemia Brain Tumours Long term Follow Up Joint Endocrine Clinics for survivors of malignancy/bone marrow transplant Leicester Children s Hospital provides paediatric services to the just over one million residents of Leicestershire and Rutland. Tertiary services at the Leicester Children s Hospital also attract patients from further afield. All the general and specialist services, except for cardiac services, are centralised on the Leicester Royal Infirmary site. Cardiac services are based at the Glenfield Hospital site. The Leicester Children s Hospital currently manages around 130 beds and has a budget of over 35 Million. The Children s Hospital team includes more than 50 consultants and sees new out-patients annually. The Children s Hospital manages 3000 day case patients and emergency patients annually. The Children s Hospital forms part of University Hospitals Leicester Women and Children s Clinical Management Group (CMG). Services at Leicester Children s Hospital The Children s Hospital is based both in the Leicester Royal Infirmary LRI) and Glenfield Hospitals and manages a large number of paediatric specialities (At LRI unless listed otherwise) including: Cardiac Surgery (Glenfield) Cardiology (Glenfield) Clinical Immunology Dermatology Endocrine and Diabetes Gastroenterology General Paediatric Medicine General Paediatric Surgery Haematology Infectious Diseases (including HIV and TB) Intensive care (both LRI and Glenfield) Metabolic Neonatal and Paediatric ECMO (Glenfield)

For the current round of RCPCH applications to MTI-p, we are delighted to offer 6 months in Acute/General Paediatrics and 18 months in Paediatric Oncology/Haematology at University Hospitals Leicester NHS Trust. In the first 6 months you will spend doing general paediatrics. As general paediatrics interfaces with the majority of the specialties, it is the best base from which to get to know the children s hospital and our ways of working. During this time, you will start as an SHO (ST3) and within 6 months, when ready, move up to the registrar rota. In the following 18 months you will be working in the tertiary oncology/haematology service. Although the majority of your time will be spent in oncology/haematology you will continue to contribute to the general paediatric on-call rota (Please look at the rotas). Depending on the clinical need there will be an opportunity to either work on the CICU or general paediatric on-call rota. This 18 months will provide an excellent opportunity to gain specialist oncology/haematology training. The order of posts within a rotation can sometimes be changed. The trainee will be responsible for co-ordinating the care of paediatric oncology/haematology patients on the ward and attending as daycases. This includes all patients on treatment who have open access to the ward. There are regular opportunities to see new patietns presenting with suspected cancer in outpatients. More specific elements of training can be organised depending on the trainees experience and areas of interest. The trainee will have the opportunity to complete parts of the RCPCH recommende training in paediatric oncology/haematology.
5 The appointee will have the opportunity and funding to complete the RCPCH approved Postgraduate Diploma in UK Medical Practice that is coordinated by the Liverpool School of Tropical Medicine. Induction All MTI candidates will receive a comprehensive induction programme to familiarise them with the functioning of the hospital, the Trust s procedures and issues of clinical governance. In addition, there is a specific induction to Paediatrics and the relevant areas for the post. Consultant supervision and advice is available at all times. The Programme All MTI candidates will start training at ST3 level for the first 6 months. This is to allow acclimatisation with practice in the UK. The expectation is that the trainee will satisfy level 1 & 2 competencies. Further to meeting these criteria the candidate will be promoted to ST4. Some candidates may not meet the criteria for promotion and will be given a further 3-6 months to make progress. In the final 18 months of training candidates will be expected to gain level 3 competencies as discussed with their educational supervisor. E Portfolio Please note it is mandatory for all MTI neonatal fellows to maintain an e-portfolio record of their training from start of the programme. Use of E-portfolio is permitted through the RCPCH for a nominal fee. Working Pattern & Duty Roster To maintain acute general paediatric skills the appointee will take part in the general paediatric middle grade on call duty rota (EWTD compliant). The appointee may also have the option of covering the CICU middle grade rota on call duty rota as an alternative option. Sample rota is shown below. 7 Liverpool School of Tropical Medicine The Postgraduate Diploma in UK Medical Practice has been designed by the Liverpool School of Tropical Medicine in order to allow MTI doctors the opportunity to demonstrate the breadth of experience gained during their work in NHS Trusts. In order to be awarded the DipUKMP, doctors will be required to carefully document their training using the eportfolio system, demonstrate that they have met learning outcomes based on a range of NHS work, and submit a 2,500 words assignment, based on their involvement in one of clinical practice, medical education or quality improvement/ clinical governance work. Study Leave The trust study leave allowance for non- training grade doctors as the same as those in training and comprises of 10 days external and 20 days internal leave per year. The leave year runs from April to April and is pro rata if their contract does not run for the entire year. Annual Leave Annual leave is dependent on salary scale at 27 to 32 days per year and up to 8 public holidays.
