2 1.0 Purpose & Introduction The purpose of this document is to outline the content for acute oncology induction training for all relevant Trust staff within the Merseyside & Cheshire Cancer Network. Each designated Trust within the network should aim to ensure that all relevant staff members are aware of local/network arrangements and referral pathways associated with the service. The content of the training programme is dictated by the Manual for Cancer services: Acute Oncology including Metastatic Spinal Cord Compression Measures.V1.0 (2011), (Measures 11-1E-106y, 11-1E-107y, 11-3Y-102, 11-3Y-103, 11-3Y-403). Please note this specification does not address the training and education required for primary care teams. 2.0 Acute Oncology Patients Acute oncology patient groups as defined and agreed by the network are: - o Patients presenting with certain acute complications of cancer. This includes: - patients with known malignancy and - patients with previously unknown malignancy o Patients presenting with acute complications of chemotherapy o Patients presenting with acute complications of radiotherapy o Patients with features suggestive of possible metastatic spinal cord compression (MSCC) 3.0 Network Induction Training Delivery The following staff groups should receive induction training: o A&E consultants and NCCG medical staff o A&E nurses band 6 & above o Consultants and NCCG medical staff involved in acute medical take rota o Consultants and NCCG medical staff involved in MAU o Haematology nurses band 5 & above those involved in haematology 24/7 triage o New acute oncology team members 4.0 MCCN AOS Induction Training The Training should comprise the following: MCCN Acute Oncology Induction Training Specification V. 2.0 Page 2 of 15
3 4.1 The Network Configuration of the Acute Oncology Service All staff undertaking the induction training should be familiar with the configuration of the Acute Oncology Service within Merseyside & Cheshire Cancer Network. The following hospitals have been designated as providing an acute oncology service: Group 1 (Hospitals with an A&E Department and acute medical beds): o Aintree University Hospitals NHS Foundation Trust o Countess of Chester Hospital NHS Foundation Trust. o Royal Liverpool and Broadgreen University Hospitals NHS Trust o Southport & Ormskirk Hospital NHS Trust o St Helens & Knowsley Teaching Hospitals NHS Trust o Warrington and Halton Hospitals NHS Foundation Trust o Wirral University Teaching Hospital NHS Foundation Trust Group 2 (Hospitals with specialist oncology beds and NO A&E Department or acute medical beds): o The Clatterbridge Cancer Centre NHS Foundation Trust Metastatic Spinal Cord Compression Centres o Clatterbridge Centre for Oncology NHS Foundation Trust (Radiotherapy) o Walton Centre NHS Foundation Trust (Surgery) o Royal Liverpool and Broadgreen University Hospitals NHS Trust (Surgery) 4.2 Trust Configuration of the Acute Oncology Service All staff should be familiar with the configuration of the Acute Oncology Service and how the service operates within their Trust i.e. its role and remit within the acute pathway, membership, contact points and regularity of Cancer of Unknown Primary (CUP) and /or AO MDT meetings. These are detailed in the Trust AOT Operational Policy, a copy of the latest version of which is available within each Trust via Acute Oncology Team. 5 o Referral pathway for patients with suspected MSCC (Appendix B) 4.4 MCCN Acute Oncology Protocols All staff undertaking Acute Oncology Induction Training should familiarise themselves with the location and content of all acute oncology-related protocols within the Trust, comprising the following:- o Fast Track Referral Protocols, o Acute Oncology Service Communication Protocol o MCCN Acute Oncology Protocols o One hour to antibiotic pathway These protocols are located in each Trust s Acute Oncology Service Operational Policy. 4.5 MCCN Acute Oncology Group (NAOG) Staff should be aware of the Network Acute Oncology Group, its role, membership (local representative), contact points and frequency of meetings. This information is detailed in the NAOG Constitution; a copy of the latest version is available at /7 Chemotherapy Patient Advice Staff should be familiar with the Trust s 24/7 Chemotherapy Service, covering: o It s role within the Acute Oncology Service o Who provides the service, both in and out-of-hours i.e. names, contact numbers During working hours (Mon- Fri) telephone advice regarding patients with solid tumour cancers can be accessed directly from local acute oncology teams/local chemotherapy service The Chemotherapy Triage service at The Clatterbridge Cancer Centre NHS Foundation Trust can also be accessed by HCPs for telephone advice regarding patients with solid tumour cancers - this service is available 24hrs a day 7 days a week. All out-of-hours requests should be directed to the Chemotherapy Triage service at The Clatterbridge Centre for Oncology NHS Foundation Trust ( Bleep 5555). Following assessment should advice from a consultant oncologist be required the triage nurse will arrange for the on call medical or clinical oncologist to contact the HCP. 6 4.7 Trust Fast Track Referral to Outpatient Clinics All staff should familiarise themselves with the content and location of their Trust s Fast Track Clinic processes. These processes are located in the AOS Operational Policy for each Trust and should specify the following: o Patients presenting as an emergency with a previously diagnosed cancer (and where the primary site is known) should be referred to fast track slots in named site-specific clinics within one week of presentation, In support of this, o There should be named site-specific clinics offering this service, covering the cancer sites treated within the Trust. o For those not treated within the Trust, the patient s Oncologist s secretary at CCC should be contacted, to arrange a site specific appointment. o There is a list of these named site-specific clinics and slots readily available and visible within A&E. 4.8 MCCN MSCC Coordinator Service All staff should be familiar with the role and responsibilities of the MSCC Coordinator, and the contact number for the service, both in and out-of-hours. These are detailed within MCCN MSCC Clinical Guidelines & Pathway (2012), a copy of which is available within each Trust. A copy of the latest version is also available at The purpose of this role is to provide a single point of contact where advice can be obtained on the immediate management of a patient with suspected Metastatic Spinal Cord Compression (MSCC), with additional responsibility for ensuring that the appropriate people (i.e. Senior Clinical Advisor Service) come together urgently to discuss the treatment plan of a patient with suspected or confirmed MSCC. 4.9 MCCN MSCC Senior Clinical Advisor Service All staff should be familiar with the MSCC Senior Clinical Advisor role. The purpose of this role is to provide advice to secondary care clinicians and MSCC Coordinators on the management of MSCC patients who have been judged suitable for definitive treatment (spinal surgery or radiotherapy). Senior Clinical Advisors will be consultants in spinal surgery, clinical oncology and radiology. MCCN Acute Oncology Induction Training Specification V. 2.0 Page 6 of 15
7 5.0 Evidence of Completion There should be written evidence of all relevant staff who have completed the induction training, and training record should be maintained within each Trust.
