2 Page 2 ROBOTIC COURSES This series of half day courses is designed for consultants and trainees who have an interest in, or are already performing, robotic surgery. The introduction to robotics familiarises surgeons with the da Vinci System, instruments and set up as well as basic manoeuvres. Intermediate and advanced level courses allow surgeons to gain knowledge and handling skills with an observed and graded assessment. Surgeons can enter at introductory level and progress or at intermediate/advanced level to improve skills and perfect techniques. The robotic training courses introduce surgeons to safe robotic surgical practice helping to develop a skill set and competency to commence in robotic surgery. The course is designed to complement the existing training methods of working with experienced surgeons under supervision and mentorship when extending practice to real life scenarios. Established and advanced level robotic surgeons can access these training sessions to enable them to enhance areas of practice such as intricate anastomosis, assessing new instruments and sutures etc. This generic course is not specific to any one speciality and can be tailored to the needs of the individual. The Facility Despite the fast growth of robotic surgery in the UK and its clinical application, there still remains a lack of accessible regular training opportunities. The Chitra Sethia Centre for Robotics and minimal access surgery is the first centre in the UK to offer Hands on year round practical skills based training. The Chitra Sethia Centre has two permanent training da Vinci four arm surgical robots. It is considered that robotic surgery is the future of minimal access surgery and more centres within the UK and Europe are developing robotic surgery practice due to the high precision possible with robotic surgery. 3 Page 3 The Principles of Robotics Robotic surgery is one of the leading medical advances in minimal access surgery and is the next real future of advanced surgery. The da Vinci Surgical System is an enhanced computer which offers the surgeon high-definition 3D vision, greater precision and dexterity, and superior ergonomics, allowing the surgeon to perform beyond the limitations of human performance. Our hands on training courses are designed for surgeons interested in learning techniques for performing robotic-assisted laparoscopic procedures. The techniques demonstrated are applicable to various surgical fields such as pediatric surgery, urology, gynaecology, colorectal, hepato bilary and general surgery. The training is structured around dry lab sessions using suture jigs, porcine models, videos, surgery observation and sufficient Hands on time on the robotic systems to improve technique.

Port placement and operative safety. To attain a basic skill level in robotic suturing technique. Port placement Understand port positioning relevant to procedure Understand risk associated with port placement Discussion of port selection Robotic cart docking Demonstration of docking procedure Docking troubleshooting Camera manipulation Focus working knowledge of focus control Manipulation working knowledge of safe movement of camera including camera clutch Selection understanding of selection and application of 0 and 30 scopes Instrument knowledge Manipulation working knowledge of safe instrument movement, including instrument clutch control Basic suture skills Interrupted, continuous sutures and slip knots on a variety of models

5 Page 5 INTERMEDIATE MODULE Course aims To give the participant a working knowledge of robotic surgery and to be confident about set up with an in-depth understanding of the robotic console, use of clutch mechanism, camera, instrumentation. To attain an intermediate skill level in suturing technique, tissue handling and basic procedures. Camera manipulation Camera selection understanding of different scope angles and their application Focus working knowledge of focus control Manipulation working knowledge of safe movement of camera including camera clutch Hands on knowledge of changing scopes Instrument knowledge Selection rationale for instrument selection Manipulation working knowledge of safe instrument movement, including instrument clutch control Port placement trouble shooting Hands on knowledge of instrument changing Basic suture skills Interrupted and continuous sutures and slip knots on a variety of models Porcine model Cystotomy and closure with interrupted sutures Ileo-ileo anastomosis Introduction to urethro-vesicle anastamosis using continuous suture (This module can be adapted to suit individual specialities)

6 Page 6 ADVANCED MODULE Course aims A structured training process with rapid assimilation of specialist skills and assessment through video feedback and scoring of competencies. Porcine model Urethro-vesicle anastamosis, using continuous suture, V-lock and Quill Completion of anastomosis in the narrow pelvis model Trouble shooting and managing a broken suture Competency assessment after a minimum of 10 anastomosis Bowel model Ileo ileo anastomosis Urterero-ileo anastomosis and stent placement Formation of intracoporeal neobladder, tips and tricks to construct a neobladder Stent placement and catheter drainage Completion of cystectomy and neobladder assessed in porcine model Vascular model End to end anastamosis Patch graft Stent suturing Video download of training available
