We will then elaborate on the specific A&R Intervention under Enhanced Service delivery--seniors Mobile Assess Restore Teams (SMART) implemented in the Hamilton, Niagara, Haldimand, and Brant (HNHB) Local Health Integration Network (LHIN). The development and operationalization of the SMART team at St. Joseph s Healthcare Hamilton is an example of one of these demonstration projects that will be discussed in detail. Learning Objectives To discuss the relationship between the Rehabilitative Care Alliance (RCA), The A&R initiatives and SMART. To describe the SMART outcomes related to the implementation of the A&R work in the HNHB LHIN. To describe the development, implementation and evaluation of an intervention project,

Session B: A NEEDS-BASED APPROACH TO REHABILITATION CAPACITY PLANNING Matthew Meyer, PhD Population Health Strategist, London Health Sciences Centre; Adjunct Professor, Western University (Schulich School of Medicine and Dentistry - Department of Epidemiology and Biostatistics, and Ivey Business School); Adjunct Professor, Ottawa University (School or Rehabilitation Science) Session Summary: Capacity planning is a well-established practice in the business world, but has only recently gained attention in healthcare; yet, effective capacity planning in healthcare arguably holds much greater importance. Over-planning capacity for one patient group, or in one region, can lead to significant waste and lost opportunities for treatment in other populations. Under-planning can lead to poor access to services or long wait times, contributing to poor patient outcomes or death. In this session we will discuss some of the basic principles of effective capacity planning and the strengths, weaknesses, and impacts of the approaches traditionally used in healthcare. We will then explore newly proposed approaches to healthcare capacity planning in Ontario including those being developed by the Ministry of Health and Long-Term Care, and the needs-based approach being developed by Ontario s Rehabilitative Care Alliance (RCA). It will also explore a framework for approaching chronic pain patients. Finally it will provide an overview of management strategies appropriate for chronic pain management, diving deeper into exercise and chronic pain and how to overcome the barriers associated with chronic pain patients abilities to engage in exercise.
