1 Embedding Equity into Quality Health Care A Symposium to Share Leading Ideas, Practices, and Strategies May 13, 2014; 8:30 AM 4:00 PM SPEAKER BIOGRAPHIES Tera Beaulieu Supervisor, Aboriginal Engagement and Outreach Unit, CAMH Tera Beaulieu, MA, is of Métis ancestry and is the Supervisor of the Aboriginal Engagement and Outreach Unit with the Provincial System Support Program at CAMH. Tera formally worked with the Youth Outreach Service at CAMH as a Counsellor and led an ongoing community partnership with Native Child and Family Services of Toronto that provides clinical support to Aboriginal youth with substance use and concurrent disorders. She is currently completing her PhD in Clinical/Counselling Psychology at University of Toronto and is the recipient of the Vanier Canada Graduate Scholarship, awarded by the Canadian Institutes of Health Research. Her area of research is on the role of Métis traditional knowledge in addressing the life transition needs of urban Métis homeless people. Dr. Paula Braveman Professor of Family & Community Medicine and Director, Center on Social Disparities in Health University of California, San Francisco Paula Braveman, MD, MPH received her M.D. degree and completed a residency in Family Medicine at UCSF, and received an MPH in epidemiology from University of California, Berkeley. For more than 25 years, Dr. Braveman has studied and published extensively on health disparities, health equity, and the social determinants of health; and has actively engaged in bringing attention to these issues in the U.S. and internationally. Her research has focused on measuring, documenting, understanding, and addressing socioeconomic and racial/ethnic disparities in health. During the 1990s she worked with World Health Organization staff in Geneva to develop and implement a global initiative on equity in health and health care. Throughout her career, she has collaborated with local, state, federal, and international health agencies to see rigorous research translated into practice with the goal of achieving greater equity in health. She was elected to the Institute of Medicine of the National Academy of Sciences in 2002.
2 Russ Ford Executive Director, Lakeshore Area Multi-Service Project (LAMP) Community Health Centre Russ Ford is the Executive Director of LAMP Community Health Centre in Etobicoke. Russ has thirty years of senior management experience in the nonprofit and government sectors. He has written extensively on issues of equity and social justice and has spoken at both national and provincial conferences. Russ participated in Occupy Toronto and was honoured for this work in organizing health care providers to help defeat Rob Ford s first budget. Russ has a master s degree in social policy and is a candidate in the October municipal election. Angela Gayle Data Quality Coordinator, Mount Sinai Hospital Angela Gale received her diploma from Centennial College in Medical Office Administration in 1994 and has been with Mount Sinai Hospital since Beyond collecting admission and registration information from patients, Angela s role as Data Quality Coordinator at Mount Sinai also includes monitoring the quality of all data collected through Admitting (e.g. patient demographic information). Stephanie Gee Senior Project Manager, Research and Evaluation Centre for Innovation and Excellence in Child and Family-Centred Care, The Hospital for Sick Children As an applied researcher and evaluator, Stephanie has focused on issues of social inequality, health equity, sexuality, and violence against women. She is currently employed at The Hospital for Sick Children as the Senior Project Manager, Research and Evaluation for the Centre for Innovation and Excellence in Child and Family- Centred Care. In this position, she has led innovative projects aimed at improving cultural competence and health equity at both the organizational and system-level, and has engaged in projects the promote the provision of child and family-centred care. 2
3 Jan Lackstrom Senior Clinical Director, UHN Chair, Health Equity Council Assistant Professor, Department of Psychiatry, University of Toronto Jan has held the role of Senior Clinical Director at UHN for the past 8 years. Her portfolio includes the Departments of Mental Health and Addiction, Community Health, Allied Health and Spiritual Care. In an effort to improve quality of care and operational efficiency, Jan has focused on enhancing the skills and resiliency of the managerial staff. Positive Leadership is a key enabler of powerful leaders. Jan is Chair of the UHN Health Equity Council. The Council provides strategic direction and support to programs as they endeavour to improve access and quality of care to all patients. The Council has been instrumental in helping programs develop their health equity quality indicators. A social worker by profession, Jan has focused her clinical work on the treatment of eating disorders. Jan is a part time instructor at York University, School of Social Work. Nancy LaPlante Regional Decision Support Specialist, Central Region, Ontario's Community Health Centres Nancy LaPlante does Regional Decision Support for Greater Toronto Area Community Health Centres with much of her work focused on evaluation through the development of client experience surveys and indicatos; as well her work includes data standards, analytics, and oral health program data management in CHCs. Nancy supports the CHCs Performance Management and Standards Committees, both of which have approved the eight socio-demographic fields for use in CHCs across Ontario. Previous work included responsibility for Ontario Breast Screening Program affiliations, and continuing education for health professionals in smoking cessation. Undergraduate and graduate studies focused on medical anthropology. Dr. Fok-Han Leung Physician, St. Michael's Hospital Associate Program and Course Director, University of Toronto Fok-Han Leung, MD CCFP MHSc is a family physician practicing general family medicine, inner city health and low risk obstetrics out of St. Michael's Hospital. He is also the Associate Course Director of DOCH-I (undergraduate medical course), and the Associate Program Director (Evaluation and Assessment) for the University of Toronto family medicine residency. He was the previous physician lead of the St. Michael's Health Centre at 80 Bond and has been involved with the implementation of multiple technologies at his clinic and department. 3
4 Dr. Kwame McKenzie Medical Director, Centre for Addiction and Mental Health (CAMH) CEO, Wellesley Institute Professor of Psychiatry, University of Toronto Dr. Kwame McKenzie is an international expert on the social causes of mental illness, suicide and the development of effective, equitable health systems. As a psychiatrist, researcher and policy advisor, Dr. McKenzie is an active, funded researcher of social, community, clinical and policy issues and has nearly 200 academic publications including four books. After completing his medical training at the University of Southampton, Dr. McKenzie undertook psychiatric training at the Maudlsey Hospital and Institute of Psychiatry, UK and Harvard University, US. In addition to joining the Wellesley Institute as CEO in March, 2014, Dr. McKenzie is Medical Director responsible for Dual Diagnosis, Child Youth and Family and Geriatric services and Director Health Equity at CAMH. He is a full Professor and the Co-Director of the Division of Equity Gender and Population in the Department of Psychiatry, University of Toronto. Dr. McKenzie is President of the Canadian Mental Health Association, Toronto. Dr. Samir Sinha Director of Geriatrics, Mount Sinai and UHN Provincial Lead, Ontario s Seniors Strategy Assistant Professor, University of Toronto & Johns Hopkins School of Medicine Dr. Samir K. Sinha MD, DPhil, FRCPC is a passionate and respected advocate for the needs of older adults. He serves as the Chair of the Health Professionals Advisory Committee of the Toronto Central LHIN, is a Medical Advisor to the Toronto Central CCAC and an Associate Fellow with interrai. A Rhodes Scholar, after completing his undergraduate medical studies at the University of Western Ontario, he obtained a Masters in Medical History and a Doctorate in Sociology at the University of Oxford s Institute of Ageing. After returning to pursue postgraduate training in Internal Medicine at the University of Toronto, Dr. Sinha went to the United States where he served as the inaugural Erickson/Reynolds Fellow in Clinical Geriatrics, Education and Leadership at the Johns Hopkins University School of Medicine. Dr. Sinha's has consulted and advised hospitals and health authorities in Britain, Canada, the United States and China on the implementation and administration of unique, integrated and innovative models of geriatric care that reduce disease burden, improve access and capacity and ultimately promote health. 4
5 Jay Smooth New York-based hip-hop scholar and cultural commentator Video and Multimedia Producer, Race Forward Jay Smooth is a cultural commentator and hip-hop scholar known for the award-winning Ill Doctrine video series. Based in New York, Smooth also founded one of New York s most respected and longest running radio shows, Underground Railroad. Mixing political commentary through a hip-hop lens, the show is recognized for supporting break through artists and leading conversations on race, LGBT rights, and a range of social justice issues. Smooth has received several awards and wide acclaim for his incisive discussions of politics and culture, with his work being featured by NPR, MSNBC, Current TV, and other media outlets. Ill Doctrine videos such as How to Tell Someone They Sound Racist have been incorporated into school and university curricula. Laura Williams Director, Client and Family Integrated Care, Holland Bloorview Kids Rehabilitation Hospital Laura Williams, MSW, RSW, has worked extensively in the field of healthcare in both frontline and leadership roles. Laura has successfully implemented new programming in several healthcare settings, with a specialty in family supports and family-centred practices. While at Holland Bloorview, Laura has provided leadership in developing strategic partnerships to ensure the success of organizational client-centredness. Laura is an Adjunct Lecturer at the University of Toronto and is also the Co-Chair of the Canadian Family Advisory Network. Along with her professional knowledge, Laura brings the user experience to the table as the parent of a child with chronic illness and disability. Greg Zaffarano Operations Coordinator, Registration Services, St. Joseph's Health Centre, Toronto Greg Zaffarano is the Registration Services Operations Coordinator at St. Joseph s Health Centre. Greg has a background in consumer marketing research from the automotive sector. He has an MBA in the Management of Technology and Innovation from Ryerson University, and has recently made the transition to Healthcare. Since his debut at St. Joseph s Health Centre in 2012, Greg has managed successful projects such as the Patient Experience Survey by pilot study, the Ambulatory Care Centre registration kiosk, online staff scheduling, and the implementation of health equity data collection at registration. 5
