1 Building One Page Profiles with Transition- Age Youth Aniko Adany 2017 The Learning Community for Person Centered Practices envisions a world where all people have positive control over the lives they have chosen for themselves. Our efforts focus on people who have lost or may lose positive control because of society's response to the presence of a disability. We foster a global learning community that shares knowledge for that purpose. 2 Each One Page Profile is a snapshot What is most important for others to know and do? - What others like and admire - What is important to - How best to support - (think: balancing important TO and FOR) 3 1

2 One Page Profiles capture important information in a way that is easy to use. They are not comprehensive Can tell a story or communicate the most important information a person wants others to know and/or do to support the person best. We gather information, then prioritize what will go on the One Page Profile, depending on how it will be used. 5 The process of developing a One Page Profile together is powerful. It helps us understand each other better. More than just a form One page profiles can be general, or focused on a particular area of the person s life think: TRANSITION *Weekends, time alone, hanging out with friends *Activities getting a haircut, attending church, etc. *Health- doctor, dentist, therapist, hospital, medications *Home and family life, selfcare *Getting around in the community *School in general, a class, or part of a day *Work, share at a job interview *Becoming more independent, safety, money Developing one page profiles What others like and admire All those things that the person is great at, strengths, skills, personality they do that make us smile others love about them they like about themselves we are proud of, achievements make the person special etc 5

6 What others like & admire for example *Awesome tennis player *Creative and determined, problem solver *Stands up for what he believes in *Green thumb! *Willing to try new things *Great sense of humor *Compassionat e towards all creatures *Getting his diploma! *Responsible and safetyminded 16 Developing one page profiles Important TO Things in life which help us to be satisfied, content, comforted, fulfilled, and happy One Page Profiles need to have enough detail that someone who does not know the person could understand what matters to him/her. These are clues to the things that are Important TO the person and should be present in the person s life. Bad Days What contributes to bad days? These are clues to the things that are Important TO avoid and should not be present in the person s life. 20 Examples of Important To *Having money to buy tickets to my favorite concerts *One on one time with my mom *Taking the bus on my own *My dog Sodapop *Being able to trust my supporters *Having control in making my own decisions *Painting at least once a week *Feeling safe and being independent *Always having my ipad, charged 7

8 Developing one page profiles How to Best Support Balancing what s Important TO and FOR the person Power Over Power With Fixing vs. Supporting TLC-PCP Good and Bad Days give us clues to best supports How can we help the person feel safe, secure, and happy? When things are going wrong, who can help and how? What supports help turn around a bad day? 24 8

9 Examples of How to Best Support Someone *When I m mad, let me come and talk to you don t pressure me *Put favorite blanket in the laundry each week *Give me time and privacy with my friends *Help make a list to remember what needs to be done *Pack a snack No gluten! *Remind me to take a break if you see me overwhelmed *Remind him to take his phone when he goes with friends *I need about an hour by myself after work/school to relax *Be aware that talking about my health is scary and stressful 25 Person centered = information Leaving out any negative Important information about a person s health and safety must not be omitted because it is considered negative. However, how the information is presented is very important. OTAC 26 How do we include important information in a way that it s most RESPECTFUL to the person and USEFUL to others? 9

10 Even if the person needs total or complete assistance with daily life, the language we use must say the person is in control. Our language is powerful Respect, dignity, and control Think about how these statements reflect each person who has control?
